This article outlines the history and development of the Snoezelen Centre at Whittington Hall Hospital in Chesterfield. Snoezelen originates from Holland. It is a leisure and recreation facility for people with profound and multiple handicaps. The concept involves creating high quality, purpose built environments which are used in a sensitive and caring way to provide pleasurable sensory stimulation and relaxation. The article describes the environments at Whittington Hall and the activities within them, together with details of the approach to staff development for Snoezelen workers. It also gives the background of a research project associated with the development of the facility, which seeks to evaluate the impact of Snoezelen on the daily lives of the people who use it.

摘要:
This article addresses the issues concerning the experience of adolescence for young people with mental handicaps. A developmental model is presented, which states that various tasks need to be accomplished if they are to attain adult status.Evidence is put forward which suggests that, for the great majority of adolescents with mental handicaps, these tasks are never accomplished. It is hypothesised that there is an unwitting conspiracy on the part of carers, both parents and professionals, to thwart the accomplishment of these tasks: unwitting in as much as there is no conscious conspiracy, but a communication through postures, attitudes, and styles of interaction. It is suggested that reform of service provision is the first step towards a more developmentally appropriate adolescence for people with mental handicaps and greater opportunities for the attainment of adult status.

Community services are increasingly concerned with people who have profound and multiple handicaps, who often show problem behaviours. This article outlines the nature and causes of such behaviours and summarises the main kinds of helping techniques which are available. These are based on the ABC model of assessment and include methods of increasing more desirable behaviours as alternatives to problem behaviours, altering the environmental circumstances and events thought to be promoting problem behaviours, and decreasing problem behaviours directly. Readers are referred to sources which offer practical guidance on implementing these approaches.

People aged over 65 years living in a mental handicap hospital on July 1st, 1989 were rated using a standardised psychiatric interview and other assessments. The results were compared with a similar survey carried out in 1976. Although the number of people had declined slightly, from 38 to 36, they constituted an increased proportion of the hospital population. Basic ability ratings were lower in 1989 than in 1976, but the difference was not statistically significant. Levels of psychiatric symptomatology were similar in both surveys.
