SUMMARYIn 1980 St. Lawrence's set out to make a positive contribution to the International Year of Disabled People (IYDP) in 1981. This was done in two ways: by focusing attention upon mentally handicapped people in hospital; and raising money to provide better facilities. In extending our own awareness of the needs of the more severely handicapped, we realised that there was a need to improve access and mobility for residents with wheelchairs in St. Lawrence's itself.This article outlines how, over and above their normal work commitment, multiprofessional groups of staff came together to achieve these aims. Reference to the IYDP amounts to no more than a diary of main events. The aspects of access and mobility which were tackled are described in more detail. All of the improvements negotiated have been or are being achieved through the normal Works Department programme, mostly by direct labour.

Professional staff in the National Health Service tend to assume without question that the patients they meet are genuine, honest, and well‐intentioned in the problems that they present. The same applies to the parents and relatives of patients, and to professional associates. The large majority are, so that any rare exceptions appear almost incredible. In this respect interviews with doctors and other health workers differ from those with lawyers and the police where the veracity and goodwill of the client is not automatically taken for granted.Downright deliberate dishonesty or premeditated manipulation of truth on the part of patients, their relatives, and people caring for them may be construed as reflecting some personality or psychiatric disorder. Milder forms and shades of “near truth”, “selected truth”, “graduated truth”, “white lies”, “exaggerated facts”, and data chosen for maximum impact, are more frequent and are probably not always suspected or detected.In all communications between people, degrees of licence and flexibility are found, and are institutionalised in different societies and cultures. For example, it has been good manners in Britain not to be too outspoken or blunt, and to use understatement. This is reminiscent of Voltaire's “Speech was given to man to conceal his thoughts.”The term “graduated truth” is used in medical work when it is thought to be in the interests of the patient to deliberately and knowingly conceal or modify the truth. This paper considers some facets of services for mentally handicapped people where “graduated truth” is used in practice. This article seeks to direct attention towards the need to educate the community to care for mentally handicapped people prior to the establishment of further community care systems. It is argued that mentally handicapped people living in the community are isolated in terms of both social contacts and occupation; and that certain assumptions are made, without supportive evidence, about the community's willingness to care.

摘要:
Living in a small residential unit within the community should mean a greatly improved quality of life for severely mentally handicapped people. It can, however, mean isolation, regression, severe problem behaviour, and much unhappiness.This article argues for a better understanding of the complex processes involved when residents of large institutions are relocated in a community situation. By concentrating upon factors which mainly concern the educational situation, some of the major problems are discussed in greater detail.It is hoped that, whilst the impetus for community placements continues unabated, we should not, through lack of adequate foresight and planning, relegate the handicapped community resident to the status of a second class citizen.

Over 160 adults attending day workshops for people with mental handicap were interviewed regarding the persons they had talked to in the community and who was usually with them on these occasions. The number of community contacts was heartening, with most occurring around the neighbourhood, fewer in the city, and least of all at home. However, there were only three groups of people with whom the majority of respondents had recent contact – local shopkeepers, bus conductors, and chemists. In general, community contacts were rarely in the company of friends – cinema‐going was the only exception. Usually they were with their family, or alone.Results from surveys of this type can help to establish the effectiveness of our social education programmes, can challenge parental assumptions about their son or daughter's abilities, and can identify groups within the community who would best benefit from guidance on how to interact with a person who is mentally handic

SUMMARYA home visiting service for parents whose mentally handicapped children were exhibiting difficult behaviour at home is described, with details of providing the service to 33 families with children aged from 3 to 15 years. The most frequent problems were socially unacceptable behaviour and incontinence, but self‐help, communication, and play skills were also tackled. Visits were usually made every 3 to 4 weeks for periods from 3 to 27 months. Targets were achieved or some progress was gained in over two‐thirds of the problems tackled.
