摘要:
A format is presented for the discussion and assessment of the sociosexual needs of an individual living in a residential setting. It has a three‐fold purpose: firstly to focus on interpersonal skills and sexuality, particularly during the preparation of individual programme plans, and highlighting areas where help is required; secondly to heighten direct care staff's awareness of clients' sociosexual needs; thirdly to consider their social environment. It is at an early stage of development and is put forward here as a pilot project which might be adapted for use in a variety of situations and developed by other workers. There are an increasing number of registers for individuals with learning difficulties. This paper briefly summarises the need for and requirement of such registers. An account is given of the development of a new set of assessment scales which are hopefully well suited as a basis for gathering information for use in a computerised local learning difficulty register.

摘要:
Many people with a profound degree of mental handicap remain wet by day despite routine attempts at toilet training and despite favourable environments. Some recent work, which attempted to replicate Azrin and Foxx's (1971) intensive toilet training programme, but in a community setting, concluded that this programme is not feasible in such a setting. The present paper presents data for just such an intensive toilet training programme carried out with an eleven‐year‐old boy with a profound mental handicap in the community. The programme has been found to be highly effective, both in institutional and community settings. Differences between the use of parents as trainers and ‘professional’ trainers are discussed, as are problems of generalisation and maintenance of newly learned skills in a community

摘要:
The introduction of the National Vocational Qualifications (NVQ) for care staff should mean existing skills are formally recognised. Managers should look to develop their training role and investigate novel training methods. Four areas for examination are discussed. (1)Shift planningwhich we believe is valuable but shift plans are a tool for staff to use, not a book of rules. Direct management intervention is essential. (2)Household&community activitiesare the key to successful residential services and staff training is a priority. Three main training methods are considered: role‐modelling, verbal instruction, taking staff aside. (3)Teaching skills—whilst preparation can be done in staff meetings etc., actual teaching is truly ‘on‐the‐job’. Demonstration is often invaluable. Direct observation and feedback is paramount. (4)Coping strategies—challenging behaviour is an obstacle to participation. Staff find it hard to value clients who behave inappropriately. Managers should ‘drop everything’ to support staff in

This article describes a five year follow‐up survey of severity of mental handicap, behavioural disturbance, psychotropic medication and psychiatric illness in residents of a mental handicap hospital in South East London. A survey of carers of people with a mental handicap was carried out in the Malvern Hills area of Hereford and Worcester in conjunction with the local MENCAP and Family Support Groups to establish the carers' views of the service. Carers wanted a service that was local and provided flexible, mostly short‐term care on both a planned and emergency basis. Carers of adults wanted a more frequent and regular service than those caring for children. The exact nature of required schemes differed between carers of adults and children. 摘要:
From September to December 1989 Ipswich TAFE (an adult education facility near Brisbane, Queensland) hosted a Communication Course for five adults who each have a profound intellectual disability and physical disability (in the UK this is the equivalent of profound and multiple learning difficulties). None of the five students possess skills of intentional communication. The course is believed to be the first one of its kind in Australia and was of an experimental nature. The course content is described. The main aim of this presentation is to share information and to invite collaboration from others in similar areas of work.
