摘要:
It has been recognised for many years that the amount of health care received by young adults with physical and mental handicaps rapidly declines after they leave the paediatric services. However, little is known about their specific health needs, or how these needs may be best met by the statutory services. This study indicates a considerable degree of unmet need and points to a failure by the Health Service to maintain a continuity of health care for these young people. A formal policy is called for in which multidisciplinary Teams for Adults with Handicaps are set up at district level, to provide a clinical, support, and coordination service for all adults with disabilities.

摘要:
The issues that come into play when young women and girls with mental handicaps are referred for sterilisation are discussed in the light of ethical arguments. The story of sterilisation is traced over the century. An attempt is made to understand the evolution of the legal aspects of sterilisation, from a rather paternalistic/eugenic standpoint in the early 1900's to a more humanistic one in the present day, as evidenced in contemporary court decisions. A comprehensive system of analysing the various components of sterilisation referral is suggested.

摘要:
This article reports on some of the main findings from a survey of CMHTs in Wales. Although a wide range of team models exists, many common features were found. The high level of commitment and importance of the All Wales Strategy as a reference point were highlighted. The setting up of CMHTs and the development of new services by team members has increased the possibility of providing appropriate and individualised service packages. Many teams, however, are still in the early stages of development and major gaps sometimes exist, such as establishing individual programme plans and review systems. Major issues emerging are the role of community nurses in CMHTs, the management and accountability of multidisciplinary teams, the level of team autonomy, and changes in professional roles.

Spilsby Road Centre was very fortunate during the winter of 1987 to be given a computer: a BBC ‘B’ with many extras, including a printer, concept keyboard, micro‐mike, turtle, AMX Mono, and a selection of software. When it arrived almost everyone was excited at the prospect of using a computer for the first time and curious to find out what it could do.A group of staff and students soon became confident in use of the computer, and gradually the circle of knowledge widened. Pat Hearn, a 24‐year‐old student who has been labelled since childhood as having a severe mental disability and being “difficult”, was initially uninterested. One day, however, Pat was watching two students (hindered by myself) using the AMX art package and mouse, and producing some rather uninspired art work. After watching for some time, and with much encouragement, Pat eventually “had a go” herself and produced the Centre's first piece of “computer art”.Pat's confidence in the use of the computer gradually increased from then on and several people have since noticed that her general self‐confidence has increased also. It seems that for the first time Pat had found something she could do successfully onher own.She rapidly mastered the keyboard and soon learned how to plug in, switch on, insert a disc, and load the programme. This was when my problems started: how was I to share out computer time, giving everyone a fair chance?After doing some highly complicated mathematics with the number of students and the amount of teaching time, I worked out that each student would be able to spend only 20 minutes each week using the computer. Obviously this was not enough, so a Computer Club was formed enabling a small group of students, including Pat, to snatch extra computer time during lunch breaks. This has been a great success.Soon after the Computer Club started anEdwordwas fitted, (a simple word processing package). One young man wanted to do an up‐to‐dateTop 40to put on the notice board each week. Pat watched, fascinated, while I worked with him — deleting and inserting words, moving text around the page, and finally printing it out. She then surprised me by saying, “Can I use that? I want to write a book for my little sister”.In no time at all Pat was proficient in the use ofEdword.The delighted expression on her face is a joy to watch when she deletes a mistake or corrects an error. She still needs a little help with grammar, but her spelling and punctuation has improved tremendously; I think because she can now see a word, know that it is wrong, and try innumerable variations until it looks right without having a page full of untidy‐looking crossings‐out. The fact that a perfect print‐out is possible has been the major motivation for Pat to persevere and succeed in an area where

Although many adults with mental handicaps have the skills to hold down jobs in the community, they often fail to present themselves adequately when they go for an interview or a meeting to discuss a job with a prospective employer. This paper argues for the feasibility of interview training. Twelve adults with mental handicaps, who attended day services, were selected randomly and introduced to a range of community based recreational activities with the help of young people from the community who acted as their partners. The activities took the form of a summer project which aimed to offer a variety of recreational experiences to each adult, and to test the benefits of working alongside community helpers.The implications for identifying ways to increase participation levels of individuals and for providing for the carryover of any new levels of activity achieved are discussed. The outcome of the project suggests that a partnership of professional, frontline, and voluntary workers can effectively promote the integration into the community of adults with mental handicaps.
