SummaryAllergens in extracts of the ‘green nimitti’ midge,Cladotanytarsus lewisiFreeman (Diptera: Chironomidae), a cause of widespread hypersensitivity in the Sudan, were isolated and partially characterized by assays which depend on the binding of125I‐anti‐IgE to allergen‐IgE complexes. These methods included RAST inhibition, crossed radioimmunoelectrophoresis (CRIE) and rocket radioimmunoelectrophoresis (RRIE). Following Sephadex G100 chromatography the ‘major peak’ of allergcnicity as determined by RAST inhibition. RRIE and SDS‐PAGE was associated with molecules of approximately 17 000 daltons. The peak eluting at Vo contained material of molecular weight 66000 daltons which also bound125‐I‐anti‐IgE. but had only 61% of the activity of the ‘major peak’ by RAST inhibition. By isoelectric focusing and RRIE of fractions obtained by chromatofocusing with polybuffer exchanger 94, the ‘major peak’ was associated with multiple bands with a pI range of 3.5‐5.5. SummarySera from forty patients with house‐dust‐mite‐sensitive bronchial asthma were examined for the presence of circulating immune complexes (CIC) by the sensitive and quantitative CIq solid‐phase radioimmunoassay (Clq‐SP) and a monoclonal rheumatoid factor solid‐phase radioimmunoassay (mRF‐SP). Compared to fifteen normal individuals, the asthmatic patients showed significantly higher mean values of Clq‐reactive materials; however, there was no difference between the results from the patients treated by immunotherapy usingDermatophagoides farinaeextract and those not so treated. Moreover, immune complexes in eight patients before and after immunotherapy showed that the amount of the complexes tend to decrease during immunotherapy. Furthermore, the presence of complexes had no relationship with the amounts of mite‐specific IgG antibody. Similar results were also obtained in tests using mRF‐SP. SummaryThe effect of increasing concentrations of inhaled sodium cromoglycate (SCG) (2, 10, 20 and 40 g/l) on histamine and methacholine bronchial reactivity was studied in nine patients with extrinsic bronchial asthma. SummaryMost clinical signs and symptoms of systemic mastocytosis (SM) are attributed to histamine release. We report here a 5‐year‐old male child with SM, who suffered from the age of 4 months from disseminated skin lesions, vomiting, diarrhoea, abdominal pain, flushing, tachycardia, hypotension, somnolence, and transient blindness, triggered by heat and egg ingestion. Oral disodium cromoglycate (DSCG) or placebo were started in a single blind trial at a dose of 100 mg/kg/day in four divided doses. The child was studied for 21 months during the administration of three courses of DSCG, each of 6 months’ duration, interspersed with three 1‐month courses of placebo. SummarySputum samples from patients with chronic bronchitis were assayed for histamine content using a bioassay, and/or fluorometric assay and single or double radioenzymatic techniques. Comparison showed that all the assays produced significant differences in the measurement of sputum histamine. Similarly, it was observed that the assays all showed significant differences in the measurement of histamine released from human basophils. In general the fluorometric assay give the highest values for histamine in sputum and the double‐isotope radioenzymatic assay the lowest. In contrast, no differences were found between the bioassay, fluorometric assay and the single‐isotope radioenzymatic assay for the measurement of histamine added to Tyrode's solution. These results suggest that in biological samples some of the assays may additionally be measuring metabolites of histamine or other components present in the samples and underline the need to measure the levels of histamine and its metabolites separately in spu

SummaryIn May 1981 a new disease, with epidemic characteristics, appeared in Spain. Initially it was thought that an infectious agent was responsible for the disease, a theory that was later discarded when the disease was found to be associated with the ingestion of adulterated olive oil contaminated with potentially toxic chemicals. More than 20 000 people have been affected, and whilst official reports put the death toll at 250, the true figure may exceed 300. The disease is multi‐systemic in presentation, affecting several organs and systems of the body. The disease has a poor prognosis in its chronic stages, which affect approximately 10% of victims, and its ultimate natural history is still to be observed. SummaryA number of workers packing ipecacuanha tablets were complaining of allergic symptoms. Skin‐prick tests, RAST and an assessment of clinical history by interview were used to confirm the presence of work‐related allergy. Specific IgE antibodies to ipecacuanha were detected and the occurrence of antibodies correlated with the incidence of allergic symptoms. SummarySpecific IgE antibodies were demonstrated in the sera of six bakers with respiratory symptoms of asthma or bronchitis by the radioallergosorbent (RAST) assay. The specificity of the antibodies was found to be directed not only against the common flours (rye, wheat) used in the bakeries but also against triticale, barley, oat. corn and rice in some of the bakers. By using the RAST inhibition tests, cross‐antigenicity was shown to exist between different cereal grains. The degree of cross‐reactivity closely paralleled their taxonomic relationship and appeared to be in the following order of decreasing closeness: wheat, triticale, rye. barley, oat. rice and corn. SummarySodium cromoglycate (SCG) has recently been formulated as a pressurized aerosol. Twenty‐five asthmatic outpatients taking SCG via the Spinhaler were studied in a double‐dummy, double‐blind cross‐over trial in which subjects took Spincaps containing 20 mg of SCG for 2 months, and for the alternate 2 months were treated with SCG aerosol delivering 1 mg/actuation.There was no evidence of clinical deterioration occurring during the period on aerosol SCG. During this period however cough frequency was significantly lower and when data for the 8‐13 and 14‐46 year age groups were analysed separately, this decrease was seen to be confined to the younger group. Furthermore, this group but not the adults, showed a significant improvement in morning peak expiratory flow rate.We conclude that the aerosol preparation of SCG is a useful alternative to delivery by Spinhaler for patients who are able to coordinate, and that it may have special advantages f
