SummaryA double‐blind trial comparing two dosage schedules of beclomethasone dipropionate, 200 μg and 400 μg, with a placebo in the treatment of perennial rhinitis for 12 months has been undertaken in 108 patients. Both schedules of beclomethasone dipropionate were therapeutically effective but the improvement in both the nasal and conjunctival symptoms was more marked with the higher dosage. Clinical candidiasis was not observed on inspection of the nose in any of the patients before or during the trial and in only one patient was clinical candidiasis observed on inspection of the throat. Epistaxes occurred in twelve of forty patients allocated to beclomethasone dipropionate and four of twenty‐three allocated to placebo who had not had them before the trial. SummaryA double‐blind crossover study has compared intra‐nasal sodium cromoglycate (SCG) with beclomethasone dipropionate (BDP), and both drugs with placebo, in fifty‐two chronic perennial rhinitis patients, BDP was significantly more effective in relieving symptoms than SCG (76.9% and 50% of the patients improved respectively,P<0.01). Both drugs were more active than placebos but while BDP was very clearly more effective (P<0.0005) SCG was only marginally better than its placebo (P<0.05, Fisher;P=0.068, χ2). SummaryIn a double‐blind trial, beclomethasone dipropionate inhaled as a dry powder in doses of 200 μg three times a day was compared with the conventional aerosol of 100 μg three times a day, each for a period of 4 weeks. SummaryMeat wrapper's asthma is a typical example of occupational asthma due to emissions from chemical products. SummaryProxicromil is a compound shown in biological tests to have anti‐allergic properties. It is well absorbed in humans after oral administration. In order to assess its anti‐allergic activity in humans, experiments were carried out in asthmatic patients to investigate its inhibitory effects upon the bronchoconstrictor response to antigen challenge.Two controlled experiments were carried out according to randomized double blind crossover designs, in each of which ten selected patients were pretreated orally with proxicromil before being challenged with inhaled doses of antigen. Inhibition of the immediate response to antigen challenge by single and repeated doses was measured by intermittent recordings of forced expiratory volume in one second (FEV1) for 30 minutes after challenge. SummaryAn examination was made of 221 children with bronchial asthma, who were divided into six groups according to serum and saliva Ig levels and the number of circulating T cells. Absence or small amounts of IgA and low or low‐normal numbers of T cells were associated with (1) atopic dermatitis, (2) hypersensitivity to house dust mite and animal danders, (3) previous hospital admissions due to respiratory tract infections with pathogenic bacteria and (4) a high family incidence of allergic diseases. In a group of patients with IgA deficiency and elevated serum and saliva IgM, respiratory tract infections were not common, and furthermore, in another group of IgA‐deficient patients with normal numbers of circulating T cells, atopic dermatitis was rare. In the latter patients, allergic rhinitis occurred very frequently, and in that respect they resembled a group of patients with combined high IgM/high IgE levels. Another group of asthmatic children with normal Ig levels represented an intermediate type of patient with regard to hypersensitivity to different allergens and family incidence of allergy on the one hand, and the occurrence of atopic dermatitis and allergic rhinitis on the other. SummaryAn analysis of skin prick reactions in 500 consecutive patients referred for investigation and management of asthma showed that 240 patients had multiple positive reactions. Of these patients 46% had positive reactions to Aspergillus and they differed from patients with multiple positive reactions excluding Aspergillus by having onset of asthma at a younger age and being more atopic. Precipitins to Aspergillus were present in the serum of twenty‐six of seventy‐one consecutive patients in whom these were looked for. Thirteen of the patients with positive precipitins had chest radiograph shadows compatible with allergic bronchopulmonary aspergillosis (ABPA) and in eight of these there was sufficient evidence to make a diagnosis of ABPA. SummarySera from forty‐two children orally challenged with ovalbumin were analysed for the presence of IgG‐ovalbumin immune complexes. The technique used is based on the adsorption of the IgG of the complex to immobilized protein‐A and detection of the ovalbumin of the complex on the solid phase, using isotope‐labelled, pepsin‐digested antibodies to ovalbumin. Children with high serum levels of antibody to ovalbumin had significantly higher concentrations of IgG‐ovalbumin complexes than children with low antibody levels. In both groups the appearance of the complexes in the serum was accompanied by a significant decrease of the complement factor C3 (P<0.05). SummaryExtracts of cotton dust were tested for their ability to activate the alternative complement pathway in fresh normal human serum (NHS). Alternative pathway activation was determined by a haemolytic assay utilizing glutathione‐sensitized human erythrocytes, consumption of alternative pathway components in terms of alternative pathway CH50 units and an immunoelectrophoretic assay to detect split products of activation of factor B. All assays were performed under conditions that have been shown to block the initial steps of classical pathway activation but permit activation of the alternative complement pathway. Results demonstrate that the cotton dust extracts could consume alternative complement pathway proteins in a dose‐response manner.
