SummaryTen patients (Group 1) withStaphylococcus aureussuperinfected atopic dermatitis (SAD) were compared with three control groups without SAD. One control group (Group 2) was comprised of ten atopic patients with total IgE levels of>1000 u/ml, another control group, of six atopic patients (Group 3) with total IgE levels of<1000. u/ml and a third group, often non‐atopics (Group 4), five of whom were infected withStaphylococcus aureus.Total and specific anti‐staphylococcal IgE levels and anti‐human dander IgE were high in Group 1. Specific IgE levels toS. aureuswere slightly increased in some Group 2 patients but were normal (<7%) for Group 3. No specific IgE to human dander antigen was detected in either Group 2 or Group 3.In the non‐atopic control group (Group 4), six out of ten patients had both low total, and specific IgE antibodies to both Staphylococcal, and human dander antigens. Two had increased total IgE and specific anti‐staphylococcal IgE antibodies, and two more with a severe, chronic infection, also had anti‐staphylococcal IgE antibodies.In SAD patients, a good correlation was seen between cutaneous basophil hypersensitivity and high specific IgE antibodies against Staphylococcal and human dander allergens. SummaryThe interaction of [125I]‐human myeloma IgE with the human basophilic IgE receptor of normal and allergic individuals was quantitated by direct ligand binding analysis after removal of endogenous IgE. Using a mixed leucocyte preparation containing 2 to 5% basophils, the [125I]‐IgE binding was specific for human IgE, paralleled the presence of basophils in the cell preparation, and was described by a time course similar to that reported for passive sensitization experiments. The Kdvalues were 4.0 to 13.2 nM for the normal individuals with 1.5 to 8.4 × 105receptors per basophil, while the IgE receptor of allergic individuals was described by Kdvalues of 4.2 to 11.7 nM and 3.0 to 4.7 × 105sites per basophil. SummaryOf 231 patients evaluated for chronic urticaria and angioedema (CUA), 192 were diagnosed as having an idiopathic condition. The roles of serum IgE, complement (CH50), and immune complexes (IC) were investigated in 112 patients with idiopathic CUA. Immediate skin tests were not helpful, but total IgE was elevated in 13%, equally divided between dermographic (D) and non‐dermographic (ND) patients. Depressed haemolytic complement (CH50) was noted in 10% of CUA, all of whom were D. Serum IC were elevated in 38%, equally divided between D and ND patients. There was no relationship between depressed CH50and elevated IC. Skin biopsies, evaluated by both light and immunofluorescent techniques, were negative for all specimens tested.The pathophysiology of idiopathic CUA is multifactorial, with a variety of immunological mechanisms involving serum IgE, CH50, and IC. SummaryIn vitropolymorph movement, phagocytosis and bactericidal activity were compared in asthmatics, patients with bacterial infection and control subjects. No gross defects were observed, but statistical analysis indicated that asthmatics have depressed polymorph movement (P>0.05) and lower augmentation of an intracellular enzyme activity (NBT reduction), associated with bactericidal function (P>0–01). As expected, polymorphs from patients with bacterial infection are capable of increased NBT reduction (P>0.001).Correlations between a wide range of clinical and laboratory data were also performed by computer analysis. Among significant findings were indications that atopic extrinsic asthmatics, in comparison with intrinsic asthmatics, have raised unstimulated NBT activity (P>0.05) and lower augmentation in response to endotoxin (P<0.05), as well as increased IgE and cosinophil counts (P<0.002). These findings suggest that in extrinsic asthmatics there may be some impairment of polymorph function which would account for an increased tendency to respiratory infections or to a prolonged recovery from such infection

SummaryThe case report of an angler with delayed onset asthma, following fishing with Calliphora (Blue Bottle) maggots, is presented. The investigations showed that the symptoms were associated with the presence of a circulating IgG antibody to a crude water soluble maggot extract. The patient progressed to develop symptoms suggestive of immune complex disease.

SummaryThe various species of mites in the bedding dust and in a number of quilts of twenty patients in Hong Kong with house‐dust allergy have been identified and counted and skin tests performed with extracts ofDematophagoidesspp. and six storage‐mites. These results are compared with a similar study in London. The particular environmental conditions which influence the numbers of mites and exposure to them have also been studied.D. pteronyssinusis the most frequently occurring species in bedding dust in Hong Kong, as it is in London, and is found in higher numbers in the dust from quilts which appear to be the main source of allergy, as are pillows, blankets, eiderdowns and surface dust of mattresses in the U.K. All the patients of both groups gave positive skin‐tests to this species.Temperature, humidity and rainfall are all at their highest during the summer in Hong Kong, the time when quilts are discarded and stored, usually several together, in cupboards or drawers providing optimal conditions for breeding of mites and spread of infestation.In a second group of eleven patients serial monthlyD. pteronyssinusspecific IgE‐values, estimated by RAST, showed a peak in January approximately 2 months after increased exposure to the mites resulting from the quilts being put back onto the beds in October‐November and an increase in the severity of patients' symptoms in November. There was a smaller peak in July possibly related to conditions caused by the high rainfall and cyclones in the preceding months.Storage mites were found in higher numbers in Hong Kong than in London house‐dust probably because sleeping rooms were often used for eating and cooking. The predominant species wasBlomia tropicaliswhich produced the highest number of positive skin tests of the six storage‐species. SummaryPigeon bloom, a waxy powder which coats the feathers of racing pigeons, has been investigated as a potential antigen in pigeon fanciers' lung. Precipitins to a soluble bloom‐extract were demonstrated in fifty‐nine of ninety‐seven people studied at two Scottish pigeon shows, whilst precipitins to pigeon serum were found in thirty‐seven. A radioimmunoassay was developed to allow an estimation of the intensity of antibody response to this antigen in nineteen pigeon fanciers being investigated for pigeon fanciers' lung. The mean antibody level to bloom extract was significantly higher (P>0.05) in those with acute symptoms compared to those with less severe symptoms. Pigeon bloom is a highly antigenic material and provides an alternative to pigeon droppings as a source for the inhalant antigens regularly encountered by those keeping

SummaryAlthough propylene‐and‐polyethylene‐glycol and saline have been used in clinical studies as placebos, their possible therapeutic role as welling agents in the treatment of perennial rhinitis was investigated. Clinical and laboratory response to these agents was measured in eighteen patients during a 2‐week baseline period and with 4 weeks of active treatment in a double blind randomized study. After 2 and 4 weeks there was a significant improvement compared to baseline in nasal function (P<0.05) and blockage index (P<0.01) combining both groups, with no difference between treatments. Patients had less sneezing at 2 and 4 weeks (P<0.01), and less stuffiness at 4 weeks (P<0.01). There was a significant correlation between improvement in blockage index and nasal biopsies when both were judged independently of the other. This study has demonstrated that wetting agents offer both subjective and objective improvement in the treatment of perennial rhinitis and merit consideration prior to (or along with) other agents with known systemic side
