SummaryThree cases are described showing a seasonal exacerbation of their nephrotic syndrome in association with an atopic trait and grass pollen allergy. The first patient has a history of four consecutive seasonal relapses each requiring steroid therapy. Following a course of desensitization injections he has now been free of relapse for 3 consecutive years. The second patient has also had a recurrent steroid‐sensitive nephrotic syndrome often associated with the pollen season and allergic rhinitis. In this patient a course of cyclophosphamide has reduced his tendency to relapse. The third patient who has been on continuous prednisone therapy shows a seasonal increase in proteinuria.Serum changes in the first two patients include: a seasonal rise in total and grass pollen specific IgE; the continued presence of grass pollen specific IgG throughout the year but with a reduction during the pollen season in association with a more pronounced fall in the total IgG level; a depression in the C3 level in association with each major relapse; a mild rise in the I‐K titre and a positive result in the C1q test for circulating complexes. A renal biopsy performed on the first patient when in relapse showed minor histological changes only and IgG. IgM, IgA, IgD, IgE, C3 and fibrinogen were undetectable by immunofluorescent examination. SummaryNineteen patients with perennial rhinitis were studied in a double blind crossover trial of 8 weeks duration using a 2% solution of sodium cromoglycate and placebo.Nasal smears, blood counts and serum IgE levels were carried out to determine whether any changes occurred with either the active or placebo formulations.Clinically, thirteen patients were greatly improved with the active solution, four showed no change and two became worse.Eosinophils in the nasal secretions were generally reduced during the administration of active solution but remained unchanged with the placebo.

SummaryCutaneous and bronchial immediate allergic reactivity and the serum level of IgE specific for each of four common inhalant allergens were determined for 153 asthmatic patients. Positive bronchial reactivity to an extract was not detected in any of the patients with a negative prick test reaction to that extract but did occur in 9% of the patients in whom the serum tests results for IgE specific to that allergen fell within the range regarded as negative. Highly significant correlation coefficients between the degree of bronchial and cutaneous allergic reactivity to the allergen extracts were found and these were slightly but significantly less than the correlation between the serum level of allergen specific IgE and the degree of bronchial allergic reactivity to the allergen extracts. These findings indicate that the results both of prick testing with appropriate extracts and estimation of the serum level of allergen specific IgE can be used to predict not only the presence of detectable bronchial reactivity to inhalant allergens but also the degree of this reactivity. However, for the routine investigation of asthmatic patients measurement of the serum level of allergen specific IgE would appear to have little advantage over properly performed prick tests.

SummaryIn a double‐blind trial thirty‐five patients with moderately‐severe nasal polyposis were treated with intranasal beclomethasone dipropionate aerosol for 3 weeks. The dose given (400 γg/day) had only local effect on the symptoms. Judged by diary card scores the nasal symptoms were reduced to 52% of the pre‐trial level for the whole group. Corrected for the placebo effect the percentage was 68. The reduction of symptoms was equally apportioned to the three symptoms, sneezing, nasal secretion and blockage.The treatment was tolerated well, and it is concluded that intranasal treatment with beclomethasone dipropionate aerosol offers most patients with nasal polyps a good response without any risk of systemic steroid side

SummaryThe technique of counterimmunoelectrophoresis (CIE) has been adapted for detection of serum precipitins to calf thymus (CT) DNA in patients with SLE, discoid LE, miscellaneous connective tissue and infectious diseases, and control populations. Of seventy‐eight LE patients, 58% demonstrated anti‐ss DNA precipitins, and 20% exhibited anti‐ds DNA precipitins. Good correlation was noted between the presence of ss DNA precipitins and ss DNA binding values determined by the more sensitive ammonium sulphate precipitation assay. Depressed total serum haemolytic complement activity in CH50u/ml was noted in 64% of sera exhibiting ss DNA precipitins and 38% of those with negative ss DNA precipitins. There was a strong association, however, between ds DNA precipitins and depressed serum complement levels.Although less sensitive than primary binding assays, CIE can be used as a rapid and simple screening test for detection of circulating anti‐native and denatured CT DNA precipitins. SummaryIn an examination of a variety of diseases associated with a peripheral blood eosinophilia it is evident that the eosinophilia is not necessarily accompanied by increased amounts of serum IgE. Tissue eosinophilia occurring with mast cell hyperplasia and usually increased amounts of JgE, indicate atopic allergy.It is considered that the eosinophilic granuloma of soft tissue (Kimura's disease) is an atopic disease and quite different from histiocytosis X.

SummaryThe morphological changes in blood basophils, from patients with bronchial asthma, were studiedin vitroduring immunological reactions, with particular reference to interaction with anti‐IgE. SummaryIn a controlled trial in Hong Kong, 575 Chinese patients with pulmonary tuberculosis whose treatment with first‐line regimens had failed were allocated at random to the following retreatment regimens of chemotherapy.(1) Rifampicin plus ethambutol daily (ER7).(2) Rifampicin plus ethambutol twice a week (ER2).(3) Rifampicin plus ethambutol once a week (ER1).(4) Rifampicin plus ethambutol daily for 2 months and then once a week (ER7ER1).(5) Ethionamide plus pyrazinamide plus cycloserine daily for 6 months and then ethionamide plus pyrazinamide daily (EtZC), as a control regimen.Answers to a questionnaire on allergic disease, the results of prick tests with standard allergens, ABO blood grouping, size of tuberculin response during chemotherapy, and a rifampicin patch test showed no associations with the occurrence of adverse reactions to daily or intermittent rifampicin.Mantoux testing during chemotherapy provided no evidence of an immuno‐suppressive effect of rifampicin.Mean platelet counts at 12 months were significantly lower than those at 3 months on the two once‐weekly regimens (ER1, ER7ER1) and on the control regimen (EtZC), although still within normal limits. SummaryTwo hundred and thirty‐one randomly selected schoolchildren and adults from rural Gambian villages showed no evidence of asthma, nor could any case of asthma be found in three rural villages with a total population of over 1200. In contrast, asthmatic patients were identified without difficulty in the capital town. The geometric mean serum IgE level of 131 rural schoolchildren was 962 u/ml, whereas that of sixty age‐matched urban schoolchildren was 368 u/ml (P<0.001). Forty‐four asthmatics, all from the capital town, had a mean serum IgE level of 405 u/ml, not significantly different from that of urban schoolchildren.Twenty‐one asthmatics showed skin sensitivity to house dust mite antigen, but their IgE levels were not significantly higher than asthmatics without skin sensitivity.
