SummaryThe possible role of leukotriene D4(LTD4)in nasal allergy was investigated in healthy volunteers. Nasal blood flow, nasal airway resistance, nasal discharge and nasal itching and sneezing were examined. LTD4was found to induce a dose‐response related increase in nasal mucosal blood flow as measured by laser‐Doppler flowmetry. Histamine exhibited similar effects on blood flow in the same concentration range. Nasal airway resistance as recorded by rhinomanometry, increased in a dose‐related manner after topical LTD4. Nasal secretion was obtained by nasal lavage and estimated from a dilution principle. Topical LTD4did not increase the amount of nasal secretion, whereas a dose‐related increase was found after topical histamine. LTD4did not cause itching, sneezing or other irritative symptoms. In conclusion, LTD4may play a role in nasal allergy by increasing blood flow and nasal airway resistance. SummaryFollowing the promotion of anti‐air pollution measures in steam power stations using crude oil, atmospheric concentrations of sulphur dioxide, nitrogen dioxide and suspended particulates have declined. These changes in atmospheric concentrations were accompanied by a decline in the prevalence of respiratory symptoms among schoolchildren living near the power stations. In analysing the correlation between air pollutant concentrations and the prevalence of respiratory symptoms, those with positive skin reactions to house dust extract (the positive group) showed a more significant correlation coefficient compared to those who had never had a positive skin reaction (the negative group). In addition, schoolchildren more heavily exposed to air pollutants showed the higher prevalence of respiratory symptoms than the others. With regard to the kind of air pollutants, both sulphur dioxide and nitrogen dioxide were more closely associated with the prevalence of respiratory symptoms than suspended particulates. SummaryWe examined the effect of nedocromil sodium on histamine airway responsiveness in twelve grass‐pollen sensitive patients during the 1984 pollen season. The study was a randomized double‐blind crossover comparison of nedocromil sodium administered by a pressurized aerosol (4 mg b.d.) with placebo. Crossovers were made at 14‐day intervals throughout 8 weeks of the grass pollen season. Histamine airway responsiveness was assessed twice before the pollen season and at the end of each 14‐day treatment period. Results were expressed as the provocation concentration (PC) producing a 10% fall in FEV1(PC10FEV1) and a 40% fall in flow at 30% of the vital capacity (PC40V̇30(P)). During the pollen season all patients developed hay fever and seven had symptoms of asthma. The observed lowest values of PC10FEV1and PC40V̇30(P)during the placebo treatment periods were significantly lower than mean preseasonal values although not significantly lower than theoretical expected values. Geometric means PC10FEV1and PC40V̇30(P)were significantly higher during nedocromil sodium treatment compared with placebo. SummaryThe IgG sub‐class antibody response to bee venom in the four sub‐classes was investigated in ten patients during and after venom immunotherapy. All patients tolerated a bee sting challenge 1, 2 and 3 years after the start of treatment as well as 1 and 2 years after treatment was stopped. Anti‐phospholipase A2(PLA2) antibodies were of IgG1 and IgG4 sub‐class and rose early in treatment, IgG1 anti‐PLA2fell to pre‐treatment levels after 3 years in contrast to IgG4 anti‐PLA2levels, which remained high during maintenance therapy and declined relatively little in the 2 years after the termination of treatment. This data shows that IgG4 antibodies are maintained in the absence of monthly maintenance injections and suggests that they may provide long lasting clinical protection from

SummaryA sandwich ELISA has been developed, using an affinity purified monospecific antiserum as a capture antibody, to detect specific IgG and IgG sub‐classes to a major antigen (Ag 7) ofAspergillus fumigatusin the sera of patients with allergic bronchopulmonary aspergillosis (ABPA). Significantly elevated levels of specific IgG to Ag 7 were detected in 97% of ABPA sera tested, as compared to control sera and to sera fromA. fumigatusskin‐prick test positive individuals. IgG sub‐class antibody levels to Ag 7 were also determined in a similar sandwich ELISA, but using specific monoclonal antisera instead of the polyclonal anti‐IgG. Both Ag 7 specific IgG1 and IgG4 levels were found to be significantly raised in the ABPA sera compared to controls. It is proposed that this antigen‐specific ELISA may provide a more specific diagnostic test for IgG antibody detection in sera of ABPA

SummaryWe report the case of an 8·5‐year‐old girl with oro‐facial granulomatosis associated with clinical atopy, in whom relapse of her granulomatous disorder was shown to be related to exposure to specific food additives, viz. carmoisine, sunset yellow and monosodium glutamate. SummaryPermanent immunoglobulin substitution therapy was performed in a 44‐year‐old patient with common variable immunodeficiency, recurrent respiratory tract infections, total absence of serum IgA and a high titre of class‐specific anti‐IgA antibodies. An IgA‐depleted i.v. immunoglobulin (IG) preparation was used. Infusions were well tolerated by the patient although minor anaphylactoid symptoms regularly occurred. Anti‐IgA antibody titres rose during the first 4 months of treatment and gradually fell during the following 8 months. SummaryTwelve egg‐allergic patients were selected on the basis of a clinical history of egg allergy and a positive skin‐prick test (SPT) to whole egg. A study was then carried out on the ability of the patients’ washed leucocytes to release histamine in the presence of whole egg, ovomucoid, ovalbumin and ovotransferrin. Histamine release (HR) from washed leucocytes was demonstrated in ten out of twelve patients, but only four out of ten released over 40% of their total histamine. Spontaneous HR ranged from 2·1–14·5% with a mean of 7·5%. There was good agreement between positive and negative HR, skin test and radioallergosorbent test (RAST) results. Concordance between the HR and skin test was found in 83%, HR and RAST in 71% and skin test and RAST in 78% of patients. However there was poor quantitative agreement between these three tests. When skin‐prick tests and HR thresholds were compared, ovomucoid elicited the greatest skin test sensitivity in five out of six patients, whereas five of the same six were more sensitive to ovalbumin when

SummaryThe serum of patients with pigeon breeder's lung (PBL) contains IgG‐class antibodies against pigeon serum proteins. These antibodies cross‐react strongly with the sera of other avian species. Cross‐reactivity, as demonstrated by precipitation, complement fixation, haemagglutination and microELISA, is also exhibited with the water‐soluble proteins in hen's egg yolk and egg white. The phenomenon appears to be due to the presence of hen serum proteins in the eggs, which occur particularly concentrated among the water‐soluble proteins in the yolk (‘livetins’).
