SummarySkin tests were performed on 210 patients with house dust allergy and bronchial asthma or perennial rhinitis using extracts ofDermatophagoides pteronyssinusand of four storage mites most commonly found in house dust in the United Kingdom—Acarus siro, Tyrophagus putrescentiae, Lepidoglyphus(formerlyGlycyphagus)destructorandGlycyphagus domesticus.The results of the skin tests were related to certain occupations and living conditions of the patients which could have exposed them to storage mites and some patients were included because their environment seemed especially likely to expose them to these species in order to assess the importance of these conditions.D. pteronyssinuswas the most potent of the mite allergens and provoked the largest number of positive tests but a proportion of the storage mite species gave skin reactions which were larger or as large as those ofD. pteronyssinus.No significant statistical correlation was found between reactions toD. pteronyssinusand any storage mite but highly significant correlations were found between some storage species. The frequency and strength of reactions to these species were unexpectedly high in view of their irregular occurrence and relative scarcity in house dust.It is suggested that sensitisation to these species occurs through exposure either to localised sources of infestation overlooked during the random collection of floor or bedding dust or to infested materials encountered at work or other activities or to infested food or bedding of certain domestic pets. It is concluded that allergy to storage mites is more important and widespread than hitherto realised and is a considerable occupational hazard in farming communities and to those in occupations handling infested materials. SummaryTen of fifteen cystic fibrosis children with positive skin prick tests to common antigens gave an immediate bronchial reaction to the antigen inhaled, five of them also gave a late reaction; however only one gave a history of asthma. The antigen most commonly eliciting a positive skin reaction in cystic fibrosis patients isAspergillus fumigatus.In six children tested to this antigen the bronchial response varied, two were negative, one gave an immediate reaction and three gave a dual (immediate and late) reaction. None of the children showed the characteristic pattern of response to exercise seen in asthmatic patients, an initial rise in Peak Expiratory Flow Rate followed by a fall of greater than 14% below the resting level. Two patients showed an abnormal rise in Peak Expiratory Flow Rate during exercise, a pattern described previously in cystic fibrosis.The results suggest that bronchial allergy, immediate or late does not completely explain susceptibility to asthma, and that other factors including perhaps the type of bronchial reactivity shown by bronchoconstriction after exercise may be required. However the majority of the children tested had bronchial allergy and anti‐allergy therapy such as inhaled sodium cromoglycate may have a place in the management of selected patients with cystic fibrosi

SummaryA case of bronchial asthma and dermatitis due to spiramycin is described in a non‐atopic woman who worked as a chick breeder and handling poultry feed containing antibiotic or chemoterapic drugs including spiramycin, chlortetracycline and sulfadimethoxine. Patch tests showed a vigorous delayed reaction to spiramycin.Inhalation challenge test with the chick feed containing spiramycin reproduced symptoms of late asthmatic response (FEV1fell by 14% and FEF25–75by 25% within the 4th and 6th hour) with leucocytosis. No significant modification of FEV1and FEF25–75was observed within 24 hours after challenge with chick feeds containing other antibiotic or chemotherapeutic agents but not spiramycin.On leaving her job, the patient had no further skin reactions or bronchial asthma episodes.This case suggests that allergic reactions to a chemical product may involve both type III and IV hypersensit

SummaryIn twenty adult asthmatics studied in a stable state, histamine bronchial challenges were carried out at one‐week intervals with the Wright nebulizer and with the dosimeter‐DeVilbiss apparatus. Dose‐response curves were analysed for sensitivity, defined as the minimal histamine dose causing a 20% fall in FEV1(PC20), and for reactivity, defined as the slope of the dose‐response curve once the reaction starts to occur. A significant relationship (r=0·80) was found for the sensitivity obtained with the two nebulizers. PC20was indeed reproducible within a two folds concentration of histamine in sixteen of the twenty patients. A significant difference (P<0·01) was found in the reactivity to the two different apparatus, reactivity being greater with the Wright nebulizer in twelve out of fifteen patients so tested. Sensitivity and reactivity showed a borderline relationship (r=0·47) with the Wright nebulizer but not with the dosimeter. A significant correlation (P<0·05) was found to exist between the initial FEV1(in % of the predicted value) and the observed sensitivity and reactivity as assessed with the Wright nebulizer, but not as assessed with the dosimeter. We conclude that histamine bronchial challenges with the Wright nebulizer and with the dosimeter yield reproducible results if the threshold of a 20% fall in FEV1is taken as positive. SummaryA 24‐year‐old asthmatic patient reported acute wheezing immediately after using an aerosol of beclomethasone dipropionate (Beclovent). She was hospitalised for bronchial challenges. Immediate bronchoconstriction was shown after two puffs of this aerosol and after inhalation of the placebo containing all the products of the aerosol less the beclomethasone. Another brand of beclomethasone aerosol, Vanceril, did not produce any reaction. SummaryImmunological investigations have been carried out on a haemophiliac patient who had developed severe anaphylaxis to bovine factor VIII administration. Results have shown that the development of anaphylaxis in this patient was associated with a sudden increase in total and specific IgG4 antibodies. The significance of this observation is discussed in relation to current evidence suggesting the involvement of IgG4 antibodies in some anaphylactic conditions.
