AbstractPsychiatric illness among mothers of infants and young children is believed to have serious implications for the child's cognitive and socio‐emotional development, and presents a continuing crisis in the provision of reliable parental care: frequent rehospitalizations are disruptive both for the parent‐child tie, and for the stability of the family unit. This chapter reviews findings regarding the impact of maternal psychiatric disorders upon the child's development, and intervention programs designed to reduce the impact of these disorders upon both child and family. In addition to considering families in which there is major psychopathology, problems are noted among families in which “an ordinary devoted mother” shows markedly lowered morale, but does not become a part of the mental health system. AbstractLittle is known about the relationship between mother and infant within multiproblem, so‐called “hard to reach” families. In an effort to understand factors contributing to problems in caring for the young children of these parents, a group of 47 families was recruited for study by the Clinical Infant Development Program of the National Institute of Mental Health. Serious social pathology was found within 75 percent of families. The lives of mothers in these families was marked by long term disruptions: 64 percent were from families characterized by recurring poverty and psychiatric illness, and 69 percent reported disruptions in parental care prior to age twelve, while more than two‐thirds reported a history of being physically and/or sexually abused as children. AbstractThe present report reviews some findings of a 20‐year longitudinal study of Danish children at risk for schizophrenia. Early factors predictive of later schizophrenia in high‐risk children, including infant temperament and the effect of perinatal complications, are reviewed. AbstractThe development of children (ages 2 to 49 months) of mentally ill mothers was compared with that of children of matched “well” mothers. The children of mentally ill mothers scored lower on measures of intelligence and had a less adaptive coping style in the testing situation. The children of “well” mothers were more socially competent; that is, they sought the attention of adults more often, responded more to social initiatives, displayed more positive affect, and sent clearer signals. AbstractChildren with mothers who had a variety of psychiatric disturbances were studied in a longitudinal investigation from birth to four years of age. The children were assessed for cognitive and social‐emotional functioning at each age and the scores related to maternal characteristics. The results were that specific maternal diagnoses of schizophrenia, neurotic depression, and personality disorder had few unique effects on child performance. AbstractThe purpose of this study was to examine character and behavior in a group of mentally ill and well mothers.Mentally ill mothers differed from well mothers in mother‐child interaction in their positive involvement in the feeding context and in overall optimal mothering during feeding. On measures of intrapsychic conflict and concern, mentally ill mothers demonstrated significantly more conflict in areas of trust, anger, self‐esteem, mutuality and nurturance and more concern with issues of self‐esteem, and giving and caring than well mothers. AbstractVideotapes of 20 mentally ill and 20 matched well mothers with their three month through 36‐month‐old‐children were analyzed in a two‐part study. Part I tested the usefulness of the Maternal Sensitivity Scale and the Maternal Effectiveness Scale in discriminating mentally ill and well mother‐child dyads. Part II analyzed the interactions of those dyads that were rated low on the scales in Part I (n = 13) to explore maladaptive patterns of behavior which may explain how the insensitivity and ineffectiveness unfolded.
