AbstractA survey was distributed to service providers in Connecticut working with newborn to 6‐year‐old handicapped children to examine the degree to which service activities currently focus on the family. Most service‐provision goals elicited by the survey were child‐focused clinical goals, but family‐focused goals were frequently cited However, 30% reported spending no time with families during a typical week. Published programs or curricula were rarely used. The providers felt they were successfully achieving goals set for families but that they encountered some problems, including a lack of time to work with families. Comparisons were made between those working with children from birth to 3 years of age and those working with children 3 to 6 years of age. AbstractA model for preventive intervention based on the use of the Neonatal Behavioral Assessment Scale (NBAS) is presented, and the conceptual framework underlying this approach is made explicit. The approach is based on the assumption that the neonatal period presents clinicians with a unique opportunity to influence parent‐infant relations positively from the beginning. It is argued that especially under conditions of stress, this infant‐centered but family‐focused form of intervention can serve to prevent the compounding of problems that may occur when the caregiving environment is unable to adjust to the needs of the young infant. The aim of the NBAS‐based approach is to sensitize parents to the communication cues and the unique adaptive capacities of their infants. It is the individualized nature of the NBAS‐based approach that places it outside the domain of main‐effects treatment modalities and renders it responsive to the particular needs of individual infants and their families. The model of intervention described herein is based on the development of a supportive therapeutic relationship between clinicians and parents and as such may serve as the first stage in a comprehensive follow‐up program of support for infants and families. AbstractA quantitative single case study is presented to illustrate how an early intervention program used two therapeutic modalities to treat a depressed mother and her 2‐month‐old son. Drawing upon an ecological, transactional model of development, the case study utilized a multimethod, longitudinal approach to assess the infant's developmental competence and attachment status, the mother's history and current psychosocial functioning, patterns of mother‐infant interaction, and components of the family's social ecology. Measures were administered during a baseline period to assess pre‐intervention functioning and were systematically repeated throughout the 2‐year period of intervention. The treatment modalities included psychodynamically oriented individual therapy and Parent‐Infant Relationship Treatment (PIRT) in which the dyad was also seen by a second therapist to foster more adaptive mother‐infant transactional patterns. The findings indicated increased infant developmental competence and a shift from an insecure to a secure attachment classification, improved maternal psychosocial functioning, and a decline in the dyadic interactional pattern of maternal intrusiveness and infant withdrawal. AbstractThis study investigated the relationship of risk status at birth and attachment in the second year of life to measures of socioemotional adaptation at age 3. The subjects were 52 preterm infants stratified into three groups according to the severity and chronicity of their respiratory illness. Two videotaped laboratory procedures were used to code both the quality of mother‐child interactions in a structured teaching task situation and the child's functioning under stress. The results show that although early risk history alone does not manifest itself as a pervasive disruption to the mother‐child relationship, attachment history was associated with differential dyadic interactions.
