AbstractLittle systematic attention has been given to the intense involvement of nursing and medical staff, and its emotional consequences for them, whilst caring for long stay very sick infants. This survey of 12 doctors and 14 nurses involved in such work confirmed the presence of consistent attachment behaviours, and examined the contributory elements such as direct contact and interaction in determining that attachment. Staff were found to relate to infants on an individualized basis, and clearly recognized temperamental differences. Differences in attachment patterns between nurses, consultants and registrars were evident, and appeared to again reflect specific opportunities for contact and interaction. Implications for hospital policy were raised.

摘要:
AbstractNursing and research coexist in a‘troubled’relationship. The culture of nursing and the discourses of research together generate tensions and conflicts which often work to constrain nurses’attempts to engage in research activity. In this paper I attempt to expose possible reasons why this might be so. Drawing on critical feminist post-structuralist theories and recent empirical research with clinical nurses some of the‘challenges’to thinking and doing research in nursing are given voice and brought to critical expression.

AbstractIn the light of evidence which linked heparin induced thrombocytopenia (HIT) to the prolonged flushing of intermittent IV cannula with heparinised saline, a randomised control trial was mounted to determine whether this solution was a superior flushing agent to normal saline. 152 inpatients between the ages of 2 months and 18 years were allocated to a control group which had heparinised saline prescribed as a flushing agent, or an experimental group which was prescribed normal saline. Each day IV sites were examined for signs of phlebitis and on removal each cannula was flushed to check for fibrin. In total 200 cannulae representing 510 IV days were assessed. No significant differences were detected in either the duration of IV therapy, the patency of the IV line or the incidence of phlebitis when normal saline was used to flush the IV cannulae. On chi square testing however, highly significant associations emerged between the incidence of phlebitis and the use of the IV antibiotics tobramycin (p=0.05) and/or Timentin (p=0.008). In addition, young children were found to have a significantly higher incidence of blocked cannulae—regardless of the flushing solution used.

AbstractThis article tells of the author’s experience of suffering a cerebrovascular accident (stroke) at the age of eighteen. The author’s feelings, thoughts, and sensations are explored, as well as the psychological and physical effects of a stroke in both the acute and rehabilitation phases. Details are given of a support group started by the author to help other young survivors of cerebrovascular accidents.

摘要:
AbstractChanges to output-based funding for health care as occurred recently in Victoria, and the goal of continuous improvement in quality of care, have provoked a rationalization of resource utilization and a greater focus on the process and outcomes of care in acute hospitals. The use of clinical management plans (CMPs) to monitor the efficiency of processes and the achievement of outcomes (managed care systems) is one approach to these challenges. A pilot study undertaken in the cardiac surgical unit at the Royal Melbourne Hospital is used to highlight the major considerations that need to be addressed when administering CMPs. These were the multidisciplinary, patient-focused approach of the managed care system, integration of the plans into the patient documentation system and the opportunities offered to research by variance analysis.
