摘要:
With the escalation of the cost of providing health care in Ontario, politicians have begun to attempt to stay the increase by curtailing hospital funding. This article reports on an interview survey of 15 randomly chosen Ontario hospitals that are attempting to cope in the current funding cutbacks. Overall, we found our sample hospitals to be an innovative group on the whole, which were willing to try out new ideas and adapt to the changing environment while protecting quality of patient care.

The U.S. health care sector is following the path of the revitalized U.S. economy with the creation of focused health care factories that provide coordinated care primarily for high-cost chronic diseases, disabilities, and surgical procedures, and with increased response to the demands of hard-working, well-educated Americans for convenience and support. Everything-for-everybody vertically integrated systems and managed care organizations will be sorely tested by these efficient, consumer-friendly innovations. This managerial revolution will create substantial opportunities for important health care administration research.

Medical practice guidelines are being developed rapidly and advocated as a crucial component in transforming and improving the delivery of health care services. However, little is known about the ability of hospitals to institute guidelines outside of experimental settings. This article approaches guideline implementation from the viewpoint of process innovation assimilation and presents specific recommendations for organizational tactics leading to successful implementation of practice guidelines.

摘要:
For the health care manager, correct words are essential to describe work and can determine if performance appraisals are high quality, efficient, effective, objective, and fair. Work descriptors were assessed from performance appraisal forms, textbooks, and current literature with word lists presented, assessed, and reviewed. It was found that work descriptors are not always carefully selected and that health care managers should wisely select appropriate words to describe work in performance appraisals.

There is growing interest among hospitals in reengineering. It promises dramatic improvements in performance: Costs will be reduced while work processes, productivity, and patient care will all improve. A review of the health care literature on reengineering shows that little evidence exists to support its claims. This article critiques the existing literature on reengineering and addresses the conundrum hospital executives encounter when faced with the decision to adopt a new management technique-such as reengineering-in the absence of proof of its efficacy.

In an environment of change, innovators or entrepreneurs emerge to develop new methods and processes of delivering health care in a way that lowers the overall costs of care while improving outcomes. This qualitative study looks at the factors that motivate nurse entrepreneurs as well as the characteristics of nurse entrepreneurs and their business ventures. The rationale for health care managers to capitalize on nurse entrepreneurship is discussed as an effective method of strategic adaptation.

The referral process is an important means of obtaining patients and it is necessary to determine ways of influencing the referral process to increase the patient base. This article reports research based on a survey of the referral habits of 806 primary care physicians.
