Using an index to measure the scope and complexity of services provided by a hospital is an effective way to explore the relationship between the extent of a hospital's programs and its annual cost of operation. Ways to use the index to support a hospital's program development are presented.

Hospitals often have large amounts of information available about their human resources, but this information is usually inadequate. The nature of existing human resource information is examined in one large hospital, and the concept of a human resource indicator system is developed. Methods for deriving meaning from the indicators are discussed.

The productivity incentives of TEFRA are compared, using a sample that includes ten patients, four on Medicare and the remainder being either private pay patients or patients covered by commercial insurance. The effect of a ten percent increase or decrease in productivity on Medicare reimbursement before and after TEFRA is examined.

In advising health care providers about care of the terminally ill in hospitals, risk managers generally take a legally conservative, narrow, risk avoidance approach. This orientation is generally unnecessary legally, since the legal risks in this context are exaggerated, and often lead to suboptimum medical treatment. A more humanistic approach to risk management should be taken where the terminally ill are involved.

Profound changes have taken place in the treatment of seriously handicapped infants in hospitals. These changes are examined from the perspective of the hospital manager, looking at hospitals' response to the new challenges of handling “Baby Doe” types of cases, including the formation of Infant Care Review Committees in many hospitals.
