Many assets and projects provide value to organizations in excess of the direct and indirect cash flows that they generate. Under pure capitation, the link between individual investment decisions and future cash flows is completely severed. Including the value of strategic options enhances the usefulness of the capital budgeting process for health care organizations and provides a means of adapting that process to managed care settings.

This article addresses the introduction of competition into the Medicare prospective payment system (PPS), the application of an expanded PPS to ensure that Medicare is not paying more than the market price for comparable services, the expansion of the Medicare PPS to outpatient services, and the expansion of the Medicare PPS to physician fees for inpatient care.

Risk analysis techniques used in valuing medical practices are discussed. Employing these techniques provides superior information to the analyst than using a single estimate for uncertain variables. Examples of scenario analysis, sensitivity analysis, and simulation are presented. These tools can be easily implemented with spreadsheet software.

While hospital outshopping is becoming a significant problem for hospital administrators, little research, exists in this area. Some health care studies have focused on the attributes deemed important by the consumer in choosing one hospital over another; however, not all service attributes are equally important in determining consumer preferences for hospitals. The authors describe an approach for calculating determinant attributes that rural consumers deem important and those in which these consumers perceive differences among hospitals.

Health care administrators have been faced with the challenge of developing performance appraisal instruments that balance continuous quality improvement (CQI) initiatives with the potential legal liabilities surrounding employee performance. Although the literature offers many examples of performance appraisal instruments, there is no real roadmap of how those charged with developing such an instrument can successfully go through the process. This article documents one pediatrie hospital's experience in developing this needed device.

Hospital planners rely on a number of graphic and quantitative devices to facilitate hospital strategic planning and each has its place. It is the comprehensive, user-friendly, environmental assessment models that are missing, however. This article seeks to remedy this condition through the introduction of a portfolio approach to strategic analysis. The authors review the analytical tools currently used by hospital planners to place this model in context and provide an exposition of the model and an explanation of how it is constructed and interpreted.

A patient classification system was developed integrating a patient acuity instrument with a computerized nursing distribution method based on an optimization algorithm. The objective was to minimize the total number of nursing personnel used by optimally assigning the nursing staff to meet the acuity needs of the various clinical units in the hospital. The validity of the model to assign staff was established by a 30-day comparison with the existing manual method of assignment.
