ABSTRACT:Research and clinical experience continue to demonstrate the impact of a traumatic brain injury on the entire family system and the importance of incorporating the family in the treatment process. Many authors have advocated for family education and support; however, there is less discussion of family therapy for families of individuals with traumatic brain injury. As a result, it has been difficult to apply research findings and clinical impressions to family treatment strategies. This article describes key treatment issues—specifically, power and authority—from a family systems perspective. Treatment recommendations are discussed that incorporate the special needs of families of individuals with traumatic brain injury within a family systems model.

This article describes the Head Injury Family Interview (HI-FI), a five-part structured interview designed to gather clinical and research data on persons with head injury and their family members. The HI-FI was developed over a 10-year period at the Research and Training Center on Head Injury and Stroke of New York University Medical Center to track outcomes of persons recovering from traumatic brain injury, with input from both the injured person and significant others, and to document the impact of the injury on the family system. The structure, contents, rationale, and development of the interviews are described, as well as its clinical utility and research potential. In addition, the statistical properties of one particular instrument within the interview, the Problem Checklist, are detailed and data presented from a multicenter study.

ABSTRACT:This study used injury, patient, and family information to statistically predict family needs after brain Injury. Predictor variables were selected based on previous research and clinical availability (eg, neuropsychological tests). The criterion variable, family needs, was chosen because of its specificity to brain injury and its relevance to prevention and treatment. Methodologically, the study included a large (N=180) and heterogeneous sample and standardized instruments and focused on outcome after two years post-injury. Multiple regression identified different sets of predictor variables for parents and spouses. Moreover, relatives' appraisal of neurobehavioral status was a stronger predictor of family needs than were neuropsychological test results. Study findings are used to develop clinical recommendations as well as future directions for family outcome research.

ABSTRACT:Mentoring has been used successfully by leaders in business and education organizations to facilitate professional training and development of employees who show promise. Many of the principles and practices used in these mentoring programs can be applied to families of individuals with traumatic brain injury (TBI). This article defines mentoring, explores several commonly used mentoring practices, and shows how these practices can be used to develop specific skills of family members. Suggestions are offered for encouraging and implementing a mentoring orientation in treatment programs for individuals with TBI. Through mentoring processes, professionals can increase the inclusion and involvement of families in planning and treatment.

ABSTRACT:This article discusses some of the influences that managed care policies and practices exert on family adjustment to traumatic brain injury. The task of serving families within the real constraints of time and financial resources challenges professionals to reevaluate and revise current thinking, technologies, and family service practices. This article provides theoretical and practical strategies as to how families and professionals can work within managed care practices through the rehabilitation process. Strategies are also proposed as to how families can work with their insurance carriers to maximize benefits and achieve optimal rehabilitation outcomes.

ABSTRACT:This article presents information regarding the epidemiology of traumatic brain injury (TBI) in culturally diverse populations and factors that must be considered when providing services to these persons and their families (eg, language, level of acculturation, family values). Issues of assessment and treatment are also discussed. Recommendations for education and training of rehabilitation professionals and for providing effective clinical interventions to culturally diverse people with TBI and their families are provided.

ABSTRACT:This article describes an analysis of the Symptom Checklist-90-Revised (SCL-90-R), a self-report measure of emotional distress, demonstrating potentially misleading scale elevations (ie, psychopathology) in data from 23 patients with mild to moderate traumatic brain injury (initial Glasgow Coma Scale score range=9-15). Average scores were significantly higher than those for the normative control group on 88 of 90 symptoms, seven of nine individual dimensions, and three summary indices. A considerable portion of these elevations was attributable to endorsement of 14 items (15.5%) identified by experts as “usual consequences” of traumatic head injury. The Obsessive-Compulsive, Hostility, and Depression scales were especially weighted with brain injury symptoms. These findings corroborate previous reports concerning the hazards of applying to neurological patients psychological instruments that were normed on physically healthy populations. Scores that would indicate pathological abnormality in the latter group may hold substantially different diagnostic significance for the former group. Clinicians should inspect individual responses to self-report questionnaires and seek confirmation from additional sources.
