State governments have responded to the need to develop services for persons with head injuries within the context of their unique structures. This article describes the development of public policies in four states, selected because they illustrate distinct approaches to the provision of services for people with traumatic brain injuries and their families. At the same time, some common strategies have emerged that may serve as models for other states to follow.

This article describes the development of public policy in the state of New York regarding the design of a comprehensive system of rehabilitation services for persons with traumatic brain injuries. Based on the experience of the existing brain injury rehabilitation programs, the data available from the Model Systems Programs, and the assistance of a technical advisory group, the policy has evolved to embrace a concept of a regionally integrated continuum of care.

摘要:
This article explores the changing expectations, practices, and policies supporting self-sufficiency and community membership on the part of people with traumatic brain injuries (TBIs). It introduces relevant developments in community living for other population groups and reviews their application in relationship to people with TBIs. It also delineates 10 philosophical principles that support community membership and argue that these principles apply to all people with disabilities, including those with cognitive and behavioral disabilities. In addition, the article discusses three emerging sets of practices, including person-centered planning, flexible and personalized services and supports, and the use of regular housing.

This article presents an overview of current public and private financing mechanisms for long-term care and support services (LTCSS) with an analysis of their deficiencies. It then summarizes recent efforts at the federal level to establish an adequate financing mechanism to improve the availability of LTCSS for persons with functional impairments. Current federal regulatory and legislative LTCSS proposals are then analyzed, with particular attention to the provisions in the Clinton administration's health care reform plan. The article closes with a brief discussion of the political factors affecting the probability of new LTCSS financing in the next few years.

Tills article examines key federal laws that play an important role in the vocational rehabilitation of persons with traumatic brain Injury. The Title I Vocational Rehabilitation Program under the Rehabilitation Act Amendments of 1992 is the most significant resource available to assist persons with traumatic brain injury (TBI) to return to employment and independent living. Although the amendments encompass policy changes relevant for all persons seeking access to the system, the provisions addressing rehabilitation technology and supported employment services are particularly important for individuals with brain injury. Long-term support services, funding issues, and suggested future directions in the research and public policy arenas are also discussed.

The 1980s proved to be a fertile time span in which awareness and recognition of traumatic brain injury (TBI) flourished. From a handful of specialized programs, an industry was created, public policy was changed, research and training became priority in some federal agencies, and many state governments took on the TBI agenda. Many individuals set forth ideas based on facts, data, and anecdotal experiences, all of which contributed to the development of a state and national policy agenda for traumatic head injury. There were as many factors contributing to the development of such policy as there were needs and levels of interest. Throughout this time period, the National Head Injury Foundation (NHIF) represented a diverse constituency of families, consumers, academicians, providers, and policy makers who came together to improve the quality of life for survivors of head injury and their families. Because of this focus and tenacity, our government began to respond to the NHIF. Historically, federal and state governments do respond to the efforts of singularly focused and well-led advocacy movements. This article will review events that impacted the development of such policy and opened the gates to research, training, and service delivery still in effect today.

Passive P300 (PP300) brain evoked responses were compared in 10 normal subjects and 13 subjects with severe traumatic brain Injury (TBI). Electrophysiological responses in patients with TBI were slower for peaks designated as N250, P300, and N400 and generated lower voltages for amplitudes N250-P300 and P3OO-N4OO. A significant anterior-posterior (Fz'-Pz') voltage difference was found in normal subjects but not in subjects with TBI. Tills difference may be both a marker of severity of brain impairment as well as recovery potential in patients with TBI. The finding of a PP300 response in patients with severe TBI, most of whom were in coma or near-coma, raises questions about the “cognitive” nature of the P300 response. Its presence suggests antecedent pre-cognitive subcortical or cortical activity such as primitive orienting to unexpected stimuli that affects higher level cognitive responses when head injury is not severe and such responses are possible.
