Increasingly, organizations that provide services to individuals with brain injuries are being asked to explain the value of their services. By focusing on quality, managers believe that their organizations can be differentiated in the marketplace. The article discusses the difficulties encountered when organizations attempt to improve quality. Important concerns discussed are the roles of the client, caregiver, administrator, and purchaser of services; the relationship of cost to quality; the marketing of quality; and the influence of team care. Prerequisites of an organization improving quality include a readiness to engage in change and a commitment by key members.

From a financial provider's perspective, the desired outcome is that a person with traumatic brain injury move through the continuum of care to independence in activities of daily living and reintegrate into society in as reasonable an amount of time as possible. The accountability of service providers to outcomes achieved according to established timetables and mutually agreed upon costs ensures the value desired by third party payers. Realistic projections by service providers of future treatment plans and resulting medical expenses allow the financial provider to reserve monies for the costs of care throughout the life of the survivor.

This article addresses the need for providers and consumers of rehabilitation services to align their expectations about eventual rehabilitation outcomes. Rehabilitation outcomes from 536 cases are presented with specific reference to global functioning measures and consumer satisfaction assessments. It is strongly suggested that all of the “customers” of rehabilitation services align their expectations at the earliest possible moment in order to ensure clinical efficacy and customer satisfaction

This article takes a focused look at outcome and value from the survivor perspective. Starting with a general review of how people with disabilities are societally devalued, the article next turns to examine three recent studies of survivor perspectives, gleaning their thoughts and feelings about the rehabilitation process through the use of different surveys. The authors summarize the key findings of these surveys, drawing some basic conclusions and suggesting means for incorporating the survivor perspective into philosophy and action within the rehabilitation setting.

The 1992Joint Commission Accreditation Manual for Hospitalscontains an elaborate section on patient rights standards that includes scoring criteria. Organizations seeking accreditation will have to implement policies and procedures congruent with these standards, especially as they apply to decision making practices for resolving ethical issues. Tills article discusses how traumatic brain injury teams might prepare to function as ethics committees so as to integrate the Joint Commission on Accreditation of Healthcare Organizations' position on patient rights with treatment decision making.

This article explores the definition of outcome, proposing that the expressed needs of rehabilitation service consumers and sponsors call for expansion of the boundaries of outcome and process to include parameters of quality, service, and value. The role of the internal case manager, in the dedicated management of process and outcome within the service delivery setting, is discussed. Mechanisms for facilitating and measuring process and outcome excellence are suggested.

Program evaluation (PE) and quality improvement (QI) systems are needed to assure and improve the delivery of high-quality, effective services to persons with traumatic brain injury (TBI). This article describes three alternative PE/QI designs based on the last decade of experience with such systems in TBI rehabilitation. The first is a conventional PE model similar to that used in many hospital-based programs. The second is a more individualized, client-centered model that reflects consumer-driven trends. The third is a more elaborate system that connects outcomes to treatment processes but requires substantial research and development. Each approach is limited, but each also can be useful. Industrywide efforts are needed to further develop PE and QI systems in TBI rehabilitation.

Uncertainty regarding the effectiveness of memory retraining with traumatically brain-injured adults continues to plague the field of cognitive rehabilitation. Although single-case experimental designs offer treatment providers a means by which to assess treatment efficacy, methodological flaws and short treatment intervals have seriously hampered previous studies. In the present investigation, a multiple-form memory assessment device was employed in an evaluation of two long-term memory retraining interventions. The results indicated significant improvement in word-list recall in a subject with mild to moderate closed head Injury, while no improvement was found in a more severely injured patient. The merits of the repeated measures methodology and the results of each single-case experimental design are discussed.

This article describes three case studies in which Direct Instruction techniques were used to teach students with brain injuries who participated in a 6-week tutoring program. The subjects were three children with closed head injuries who exhibited significant learning problems: two boys, ages 8 and 10, and a girl, age 6. After approximately 12 hourly instructional sessions, all three students made substantial academic progress as demonstrated by probe data collected in their targeted instructional areas (reading, language, math, and keyboarding). The gains were seen in both discrete and more complex skills, and some of the gains made represented new learning. In addition, the aggressive outbursts of one student were decreased substantially through use of a self-monitoring technique in the context of academic instruction. Results from these case studies suggest that Direct Instruction is a promising approach for teaching both academic and behavioral skills to students with TBI.
