Thirty-eight consecutive separations from employment in a supported employment program for persons with traumatic brain injuries were examined. Five categories of reasons for separation were identified, in order of frequency reported: (1) issues related to the employment setting, (2) interpersonal relationship problems, (3) mental health problems and criminal activity, (4) other problems, and (5) economic layoffs. Excluding true layoffs (temporary suspension of work for economic reasons only), over three fourths of the separations involved more than one reason. Potential strategies for reducing the occurrence of the problem areas were discussed.

Substance abuse is a relatively common problem facing persons with traumatic brain injury and often contributes to difficulties gaining and maintaining employment. To assist vocational rehabilitation professionals, information is presented regarding commonly used terminology, pre-and postinjury levels of substance abuse, and assessment methodologies. Guidelines for policy development and implementation as well as education and prevention efforts are described.

This article describes return-to work outcomes for 130 persons with traumatic brain injury (TBI) who participated in a vocational rehabilitation job development and placement program. There was a 68% placement rate in paid employment, a 71% employment retention rate, and a 75% employment stability rate. By contrast, engagement in paid employment was less than 40% for a group of graduates of a TBI day-treatment program and a TBI group who only received inpatient rehabilitation. This preliminary report describes the 3-year study procedures, service program components, and work experience of clients served.

摘要:
The authors present several intervention strategies that have facilitated vocational reentry after traumatic brain injury. These include assessment of learning style, creating simulated work environments, domain specific training, role playing, and using prosthetic aids and cognitive orthotic devices. Several barriers to reemployment are also presented, along with suggestions that obviate these problems. A “return loop syndrome” summarizes the head injured person's cycle of prevocational training, employment, and change of job duties and the need to return for additional vocational rehabilitation services.

Vocational evaluation in our country today remains an imperfect science, which is only complicated by the multitude of variables (relating to cognition, communication, other system injuries, psychosocial issues, etc) that are involved in the assessment of traumatic brain injured clients. In consideration of the number and complexity of these variables, however, it becomes critical to utilize uniform vocational evaluation or goal-setting procedures. This article reviews the range of factors that need to be addressed in the vocational evaluation of these clients and emphasizes the need for efficient procedures within timelines that may be expanded in contrast to many other disability groups. Procedures are described under the categories of transferable skills assessment, and basic and intermediate levels of evaluation. There is a focus on progressing to situational and/or community-based assessment as soon as possible. The specific recommendations that should result from a basic evaluation are outlined, whereas the intermediate level of evaluation should end in the choice of a placement model, if competitive work is feasible. Case studies are presented that illustrate the different evaluation categories.

Unemployment is a chronic problem for many persons with traumatic brain injury. The value of many existing vocational rehabilitation programs is uncertain, and there is relatively little long-term outcome data to help gu ide rational program development. An evaluation of neuropsychological, behavioral, and emotional status was obtained on a sample of 102 consecutive individuals referred for supported employment services. A diversity of serious cognitive and behavioral problems were found for a substantial majority of the sample. Observed characteristics were similar to those reported in other long-term investigations of severe head injury. The seriousness of observed problems helps explain chronic employment difficulties and provides important information relevant to serving clients' needs.

Employment outcome was predicted for fifty-nine clients with traumatic brain injury participating in a holistic neuropsychological rehabilitation program using data collected prior to and following treatment. Group process measures and staff ratings were added to demographic, neurological, and psychological variables; factor scores were derived, and used to predict postprogram employability and actual work status six months after program completion. Postprogram measures were more accurate than preprogram measures in both predictions. Neurological, neuropsychological, and group process measures all contributed to predicting outcome; group process measures of awareness and acceptance added significantly to the prediction, and were the most efficient postprogram predictors of both employability and work status.
