This study evaluated relationships of the Mayo-Portland Adaptability Inventory (MPAI) to the Disability Rating Scale (DRS), the Ranchos Los Amigos Levels of Cognitive Functioning Scale (Rancho), and cognitive performance measures. The Spearman correlation between the MPAI and the DRS was high (rho = .81). Both the MPAI and the DRS differed significantly among patient groups based on independent Rancho ratings (P<.001 for the Kruskal-Wallis test). Spearman correlations between MPAI scores and cognitive performance measures were generally moderate to good (rho >.40), although cognitive performance measures correlated more highly with MPAI cognitive items than noncognitive items. Contrary to expectation, MPAI noncognitive total score was significantly related to a measure of verbal intelligence, but not to measures of verbal declarative memory. These results suggest the possible importance of the fund of academic and social knowledge and the relatively smaller contribution made by verbal declarative memory to overall functional adaptation after brain injury. Results provide initial evidence of the usefulness and concurrent validity of the MPAI as a measure of adaptive functioning after brain injury.

A questionnaire assessing orientation; awareness of brain injury; and physical, communication, functional and sensory/cognitive impairment was administered to nine institutionalized survivors of brain injury, who were at least 4 years postinjury. All survivors were totally dependent in basic activities of daily living. Six were found to produce reliable test-retest estimates of their self-awareness of impairment, and their responses were compared with staff ratings. Two patterns of self-awareness of impairment were found. Three survivors were more disoriented and showed little awareness of the presence of brain injury or of striking organic symptoms (such as the inability to talk). The other three survivors showed greater awareness of overt physical and functional impairment while still underestimating their level of impairment compared to staff estimations.

This study evaluated a computer-based program (TEACHware™) for remediating cognitive-communication skills in individuals with traumatic brain injury (TIM). TEACHware™ consists of a screening module (pretest/ /posttest measure) and six interlinked remediation modules. A randomized controlled experimental design was employed using two groups of six subjects with TBI. The remediation group made significantly more gains than the control group on both the screening module and standardized assessment measures. Results suggest that TEACHware™ is an effective enhancement to traditional rehabilitation and special education programs. Further Investigations will seek to establish its usefulness in assisting individuals with TBI to achieve greater success in academic, vocational, and interpersonal activities.

摘要:
Parents of children with acquired brain injury experience severe stress. Many experience periods of emotional distress related to their child's injury and its sequelae. This study compared two kinds of support groups for parents of children with sequelae of severe brain injury. One group participated in a psychoeducational stress management program that emphasized instruction in coping skills and group discussions of coping efforts. The program included regular homework assignments, practice exercises, and follow-up discussions of the use of skills. The second group was an informational support group. Parents identified topics that they wanted to discuss and were also encouraged to share their feelings with group members. The impact of the two groups was measured with standardized self-report measures of depression and anxiety. Parents in the stress management group experienced significant reductions in depressive and anxious symptoms. Parents in the informational and sharing group, as a group, showed increases in depressive and anxious symptoms.

摘要:
A group of professionals, survivors of brain injury, and family members met to achieve consensus on a number of issues in neuropsychological rehabilitation. These included the definition of neuropsychological rehabilitation, the role and value of cognitive rehabilitation within neuropsychological rehabilitation, neuropsychological evaluation as a predictor of vocational success, enhancing functional evaluation to aid in vocational planning, and identifying factors that affect quality of life. Summaries of these consensus discussions and future considerations for the field are presented.
