AbstractReconstruction of the arch of the mandible has always been a difficult problem. The problem of reconstruction is compounded by bony defect and also the loss of soft tissue after resection of tumors of the floor of the mouth. A variety of different methods have been utilized for immediate and delayed reconstruction of the arch of the mandible with limited success. There are various post‐operative complications and problems related to infection, soft tissue loss, and recurrence of the tumor. A free microvascular reconstruction is considered the best and the state of the art. However, it requires specialized training and expertise in microvascular surgery. Other methods have been successful, such as the pectoralis myocutaneous flap, pectoralis osteomyocutaneous flap. We have utilized the pectoralis osteomyocutaneous flap in six instances. Two of these patients have been followed for a long time, ranging from 7‐8 years and we have noticed certain changes in the soft tissue and the rib. We are reporting here the long term results of the two patients who are alive from 7‐8 years. The major finding in these patients was rib resorption and deteriorating cosmetic appearance and development of “Andy Gump”

AbstractIntestinal hemorrhage due to arteriodigestive fistula is described to be a rare and serious complication of radiotherapy. Three cases of this type of fistula in gastric cancer patients treated with gastrectomy, intraoperative electron boost (15 Gy), and fractionated external beam irradiation (46 Gy) are reported. Selective angiography is most helpful in the diagnosis and immediate surgery could be lifesaving. An attempt to explain the etiopathogenesis is made.

AbstractSix patients with adenosquamous carcinoma (ASqC) of the pancreas were studied clinicopathologically and immunohistochemically. In five of six ASqC tumors, both malignant squamous and glandular elements were reactive with CA 19‐9, ST 439, and keratin antibodies. In contrast, a portion of the glandular element in the remaining one ASqC was reactive with CA 19‐9 and ST 439 antibodies, but that of the squamous cell carcinoma (SqCC) was not reactive. However, SqCC of this tumor was intensely reactive with keratin antibody. These immunohistochemical results suggest that the histogenesis in one ASqC tumor was different from that of the other 5 ASqCs, and that this tumor may be a collision tumor rather than transformation to SqCC from adenocarcinoma, which is a very rare pattern of histogensis in ASqC. The patients with ASqC of the pancreas showed shorter survivals following operations because of systemic metastasis including liver metasta

AbstractThree patients with external small bowel fistulas were successfully treated with bowel rest, total parenteral nutrition, and the somatostatin analog, octreotide. Two of the patients had received prior multimodality therapy, including radiation, for gynecologic cancer. The time intervals to fistula closure were 2 days, 10 days, and 5 weeks after initiation of octreotide therapy. The efficacy of octreotide combined with total parenteral nutrition in the treatment of external enteric fistulas supports its routine use, especially in previously irradiated patients.

AbstractClear cell adenocarcinoma in a urethral diverticulum in the female is a very rare and unusual tumor. We recently treated two patients with this tumor. Their presentation, histologic evaluation, and management are reviewed in light of the limited experience in the literature.
