AbstractA case of follicular carcinoma of the thyroid is presented with two unusual features: 1) metastatic tumor in a phalanx; and 2) all iodophilic metastases, making possible treatment with radioiodine. The possible mechanism of metastases to reach the systemic arterial circulation is discussed.

AbstractThirty cases of lip cancer in 28 patients, 23 of which were extensive or very extensive, were treated with local urea injections in combination with curettage. Out of the 30 cases there were 26 squamous cell carcinomas, three basal cell carcinomas extending to the mucosa from the skin of the lips, and one adenocarcinoma of the mucosa of the interior surface of the upper lip. This treatment was effective in all our patients. Recurrences appeared in four patients. These were easily treated in three out of the four patients who returned for treatment, by the same method. In three cases metastases appeared in the lymphnodes of the neck, which were surgically treated.

AbstractA case of neuroectodermal tumor, a malignant schwannoma, of the cervical vagus nerve is presented. It had been previously misdiagnosed and was a recurrence. AbstractA prospective study was carried out to determine the results of chemotherapy as an adjuvant to surgery in the initial treatment of adult primary soft tissue sarcomas. The results were compared with those in a group of patients with similar histologic types of primary sarcoma treated by surgery alone. The chemotherapy regimen consisted of adriamycin, 60 mg/m2intravenously on Day 1, and DTIC, 250 mg/m2on Days 1 through 5. The cycle was repeated every 22 days. The total dose of adriamycin was 500 mg/m2; the DTIC was continued for 1 year. The adjuvant chemotherapy group consisted of 113 patients (group one) and the concurrent surgical resection group consisted of 144 patients (group two). In group one, 53 tumors were T1and 60 tumors were T2; 67 of the tumors were grade 3 and 46 were grade 4. In group two, 65 had T1and 79 had T2tumors; 82 tumors were grade 3 and 62 were grade 4. The anatomic location and histologic types were similar in both groups. Seventy‐seven (77%) of the 113 patients in group one lived disease free for 2 years, compared with 59% of the 144 patients treated by resection alone. Of the patients eligible for 5‐year survival analysis, 74% were disease free in the adjuvant chemotherapy group, compared to 50% in the surgical group. The incidence of local recurrence was about the same in both groups. From a histologic standpoint, malignant fibrous histiocytomas and myogenic sarcomas appeared to benefit most with use of the use of the two‐drug regimen used here, and the least favorable response was in liposarcomas and fibro‐sarcomas. AbstractAn oat cell carcinoma occurring in the esophagus of a 59‐year‐old man is described. A review of another 22 cases of such a tumor from the English literature is made. These tumors may have three histologic patterns: pure oat cell carcinoma, oat cell carcinoma with squamous cell carcinoma, and oat cell carcinoma with adenocarcinoma. AbstractFrom 1955 to 1976, 129 patients were seen with metastatic malignant melanoma from an unknown primary site, comprising 8.1% of all the referred patients with malignant melanoma. Eighty‐two percent of the patients presented with a history of mass or lump.Overall median survival and 5‐year survival rates after onset of symptoms were 10 months and 13%, respectively. Patients with lesions of the head and neck area had 5‐year disease‐free survival of 25%, whereas those with lymph node disease had a survival of 18%. Twenty‐nine patients with regional disease had wide excision (usually lymphadenectomy) and their 5‐year disease‐free survival was 58%.Regional disease in patients with malignant melanoma of unknown primary site should be treated with radical surgical excision, since the survival is comparable or better to that of patients with regional disease having known and excised

AbstractThe humoral immune response to tumor‐associated antigens (TAA) in cancer patients undergoing immunotherapy can vary considerably. This variability is inherent from patient to patient and is also dependent on the type of assay used to measure patient antibody response to tumors. In this investigation four serological techniques, complement‐dependent antibody‐mediated cytotoxicity, immune adherence, complement fixation, and indirect membrane immunofluorescence, were used to assess the humoral immune response in melanoma patients receiving specific immunization with cultured allogeneic melanoma cell vaccine.Two different patterns of antibody response were elicited. AbstractTwo hundred and thirty‐one patients had 270 bone scans. Increased rib uptake was correlated with radiological findings. Interpretation of results whether due to benign or to metastatic disease depends on other observations. These include whether the uptake is localized or along the rib outline, single or multiple, and associated with other abnormal uptake in the rest of the skeleton. Abnormal rib uptake was encountered in 23% of patients scanned for malignant disease and 10% of patients scanned for benign conditions.In the first group 34% were true positive and of these, rib lesions were multiple in 32% and solitary in 2%. In 20% the rib uptake was due to benign conditions and in 46% it was indefinite because of absent radiological findings. In this last difficult group 17% were due to probably early rib metastases and 29% were due to benign causes. Rib metastases were missed in only two cases, 1.1%. AbstractSixty patients with Stage I or Stage II malignant melanoma were treated with wide local excision of the primary and dissection of the regional lymph nodes prior to being randomized to chemoimmunotherapy with DTIC plus intradermal BCG versus methyl‐CCNU plus intradermal BCG. For the 32 Stage I patients, there were no differences in survival between the two treatment groups. Survival was similar to a concomitant control group of 45 Stage I patients. For Stage II melanoma both chemoimmunotherapy groups had a significantly better survival than the 11 patients in the untreated control group. These results do not indicate that this form of chemoimmunotherapy is valuable in Stage I malignant melanoma but further trials are needed for patients with Stage II diseas
