AbstractThe efficacy of surgery alone or combined with preoperative radiation was examined using isotransplants of the spontaneous fibrosarcoma (FSaII) growing in the right leg of syngeneic C3Hf/Sed mice and employing as end‐points both local tumor control probability and normal tissue damage. Complete assays have been performed on small (4–5 mm) and large (8 mm) tumors. The TCD50s (the dose required to control half the irradiated tumors) for radiation alone were 63.2 and 82.7 Gy for small and large tumors, respectively. Surgery was performed 4 days after the radiation treatment. Surgical resections were performed under an 8X dissecting microscope. In small tumors, radical (enbloc resection) and conservative (local resection) surgery alone achieved 69.8% (30/43) and 21.9% (7/32) tumor control. The corresponding figures for large tumor were 25% (8/32) and 6.1% (2/33), respectively. The combination of radiation and surgery increased tumor control frequency up to 100% with doses lower than TCD50 for radiation alone. The TGF is expresseed as therapeutic gain factor for conservative and for radical surgery for 4–5‐mm and for 8‐mm tumors from analyses of dose response curves for tumor control and leg shortening. In small tumors, the TGFs at the 80% tumor control (TCD80) and leg shortening of 5 mm were 1.4 and 1.8 for radical and conservative resection, respectively. The TGFs at the TCD50 level for large tumors were 1.4 and 1.7 in respective resections. In this particular experimental system, conservative surgery yielded higher TGFs than radical surgery for both tumor sizes. AbstractThe most common complication of total thyroidectomy is hypocalcemia. Following thyroidectomy, especially total thyroidectomy, the serum calcium usually falls gradually and patients do not usually require supplementary medication before 24 hours. Two cases of total thyroidectomy are presented in which the preoperative serum calcium levels were normal and hypocalcemic tetany developed in the recovery room immediately after the operation. The hypocalcemia was a temporary phenomenon, and neither patient requires supplementary calcium at the present time. There is no good explanation for this precipitous drop in the serum calcium levels in these two patients.

AbstractThe lymph node biopsies of 12 patients with lymphadenopathy due to reactive hyperplasia contained lymphocyte populations with increased percentages of B‐cells and abnormal numbers of cells with single class surface immunoglobulins. Ten patients were followed for periods of 3–30 months: one developed a non‐Hodgkin's lymphoma and one Hodgkin's disease. One other has persisting lymph node enlargement. Two patients had nonlymphoid malignancies. The other six remain well without lymphadenopathy. Twenty‐five of 40 patients with reactive lymph node hyperplasia and normal kappa/lambda ratios followed for 9–36 months had a similar incidence of lymphoid malignancy and persistent lymph node enlargement. The follow‐up data so far available do not support a hypothesis that reactive lymphoid hyperplasia with increased numbers of cells with single class surface immunoglobulin is associated with higher morbidity than reactive hyperplasia containing the usual polytypic lymphocyte

AbstractOur initial experience with diagnostic ultrasound mammography (UM) showed it to be of high diagnostic accuracy and improved specificity when compared with x‐ray mammography (XRM) in certain categories of patients. To evaluate this modality further, we reviewed our experience with 600 consecutive patients who underwent ultrasound mammography as part of their initial evaluation for breast disease. Five categories of patients were reviewed: I: age 35 or less—129; II: DY or P‐2 pattern on XRM—174; III: negative XRM but symptoms requiring additional evaluation—81; IV: high risk remaining breast in patients previously having mastectomy—48; V: refusal of repeat XRM because of pregnancy, lactation, or fear of radiation exposure—168.Fifty‐five solid lesions were diagnosed, including 36 carcinomas. Seventeen of these were not diagnosed on x‐ray mammography, giving an improved pick‐up of carcinoma in this overall group of 2.83%. Of particular interest was that, of the 27 carcinomas that were found in the group having XRM, 8 or 29.6% were missed on that modality alone. Five of these (62.5%) were patients having a DY pattern. All patients have been followed carefully and re‐examined at 3‐month intervals, with no evidence of a false negative examination. AbstractThe value of serial biochemical and hematological tests of blood in differentiating stages and in detecting recurrence of breast carcinoma was studied in 500 patients. Patients who had mastectomy and were with no evidence of disease (NED group) had higher hemoglobin, albumin, total protein, and lower LDH than patients with any amount of breast carcinoma. These four tests became more abnormal in patients with metastatic disease (group IV). In addition, peripheral lymphocyte count was significantly decreased, and SGOT, AP, alpha‐1 globulin, and CEA significantly increased, when patients developed recurrent or metastatic disease. Patients with disseminated soft tissue or lung metastasis had less abnormality of such blood tests than those with metastasis in bone or other visceras. Immunoglobulins (IgG, IgA, IgM) were not different between early or late stages, nor specific sites of metastatic involvement. Among patients with a greater amount of loco‐regional disease (group III) and patients with metastatic disease (group IV), those who had hemoglobin ⩾ 12.5 gm%, lymphocyte count of 15–2,500/cmm, albumin ⩾ 4 gm%, AP ⩽ 25 units, or LDH ⩽ 400 units had significantly better 5‐year
