AbstractMeningioma can be a diagnostic problem for the head and neck surgeon and the surgical pathologist by reason of extracranial manifestation and the varying histological patterns seen in small biopsy specimens submitted from various extracranial sites. During the thirty year period 1940–1970, 405 cases of meningioma were seen at the New York Hospital and the Memorial Hospital and the clinical findings have been reviewed together with recent pertinent literature. Thirty‐four cases were spinal in origin: 9 cervical, 22 dorsal, and 2 lumbar. An external mass was present in 20% of the intracranial tumors: the orbit was involved in 30 patients, the outer table and scalp in 25, the paranasal sinuses and nasal cavity in 10, and the parotid and parapharyngeal region in 5. Three cases are documented primary extracranial meningiomas arising is soft parts; one free in the orbit and two in the neck masquerading as deep lobe parotid gland tumors. The pathological features of extra‐cranial meningioma were identical with the intracranial variety and a standard classification of the main histological types is outlined. A variety of differential diagnoses were entertained before the true nature of the neoplasm was established. AbstractAdriamycin was given by arterial infusion to 32 patients with different types of advanced cancer. The drug was used for inducing the first remission. In responsive cases maintenance treatment was given with conventional agents. Twenty‐six patients were adequately evaluable. Seventeen patients adequately treated showed objective improvement and 7/17 regression>50%. Head and neck cancer was the disease most sensitive to adriamycin by arterial infusion. Of 32 patients receiving adriamycin, 23 had one or more side effects, 14 had morbidity, and 2 patients died. In comparison with other conventional drugs, adriamycin by prolonged arterial infusion produces a higher percentage of side effects and not a higher percentage of therapeutic respons

AbstractThirty‐three horror basal cell carcinomas (18 large, six destructive, five locally uncontrollable, and four metastatic) were studied in detail. In comparison with a series of non‐horror cases, it was found that there were more horror cases in men and that the age of onset of horror tumors was earlier. The anatomical site was the same. Basic etiological factors included habitual sun exposure in 13, mustard gas burns in two, and one each of radioermatitis, trivalent arsenic ingestion and naevoid basal cell carcinoma syndrome. Fifteen had no obvious cause. It could not be shown that initial treatment with radiotherapy caused any change in the biological nature of the cancer. In 17 cases no medical treatment was sought until after the cancer had been present for over five years. It is concluded that these rare cancers, histologically indistinguishable from the ordinary basal cell carcinoma, are, from their onset, either biologically more aggressive or occur in patients with a lower tumor host resistance. AbstractThis is a retrospective and all‐inclusive study of 527 patients with malignant lymphoma. A total of 178 major surgical procedures were carried out on 139 patients (an incidence rate of 18.4% for the 163 patients with Hodgkin's disease and 29.9% for the 364 with other lymphomas). There were 101 procedures done initially for diagnosis and 77 carried out after the diagnosis was established. Only 15 patients had laparotomies for staging purposes. The other operations included 59 celiotomies for diagnosis (21 had resection of organs), 42 abdominal operations for complications (malignant or nonmalignant), 11 radical node dissections, 6 radical mastectomies, 5 laminectomies, 4 thoracotomies, 3 bone operations, and 33 various other procedures.With the wider application of the exploratory laparotomy for staging and the improved survival with radiotherapy and chemotherapy, it is obvious that the future role of surgery will be quite different from the past. However, certain patients will still require major surgery for diagnosis. AbstractIn the search for a simple, objective, reliable and sequential diagnostic hepatography, an oily contrast medium, Ethiodol {ethiodized oil), was purposely injected into the portal vein system of dogs. In the absence of a porto‐systemic shunt, the smallest amount of Ethiodol which produced faint roentgenographic visualization of pulmonary oil embolization was about 2.50 ml/kg. The average “trapping capacity” of the livers of six dogs was 2.89 ml/kg. The average acute lethal dose was 5.10 ml/kg. It was not possible to extrapolate the lethal dose of intraportal Ethiodol in humans. Also, the consideration of a threshold dose level (corresponding to the trapping capacity of the liver), near or above which the problem of pulmonary oil embolism will become significant, is only a rough concept. However, the results of this study serve to emphasize that the potential hazards of using intraportal oil contrast media for clinical purposes should be kept in

AbstractProstatic cancer with metastatic disease generally is associated with a short survival. A 68‐year‐old man has survived 10 years following total prostatectomy for a clinical Stage B lesion. Repeatedly positive bone marrow aspirates reverted to normal following a short course of intravenous nitrogen mustard and diethylstilbestrol disphosphate chemotherapy. AbstractAmputated branches of the cervical plexus may develop neuromas within the area of a radical neck dissection. At times these are palpable and are thought to be recurrent tumor. The distinction between recurrent tumor and neuroma cannot be made easily. Many patients who have extensive maxillofacial surgery have discomfort or neuromas within the operated area postoperatively. Five patients have been found to have palpable neuromas within their cervical operative field, but 84% (42/50) had painful neurological symptoms or unusually sensitive areas within the area of previous surgery. Only a third of this latter group had recurrent tumor.

AbstractRats were partially hepatectomized, injected sc daily for the first 7 days with oil solutions of several mineralo‐ and glucocorticoids, sacrificed on the tenth post‐operative day, and the extent of liver regeneration ascertained. Cortisone and prednisolone acetates at overall levels of 70 and 7.5 mg/kg, respectively, depressed regeneration, as was also noted with aqueous solutions of hydrocortisone and dexamethasone sodium phosphates at about 85 and 0.7 mg/kg, in the order stated; aldosterone (0.5 mg/kg) or fludrocortisone (15 mg/kg) were ineffective. The injection of deoxycorticosterone acetate (130 mg/kg) or the feeding of a diet with spironolactone supplemented at 0.060% accelerated liver regeneration and these, in addition to very massive levels of progesterone, counteracted the effect of cortisone; testosterone propionate negated the action of the latter in the adult female. An inhibitory response was also evident when cortisone acetate was fed at 50 ppm. The extent of regeneration was little affected in rats adrenalectomized at least 5 days prior to partial hepatectomy and, except for dexamethasone (0.7 mg/kg), the inhibitory influence of the above glucocorticoids was minimal; the stimulatory action of spironolactone continued. In normal or adrenalectomized rats with intact liver, the spironolactone feeding and, even more so, the sc injection of fludrocortisone (15 mg/kg) elicited increases in liver‐body weight ratios, cortisone acetate at levels even as high as 190 mg/kg being without a

AbstractSpontaneously observed and exogenously induced renal tumors in mice and rats have been described. Occasionally occurring tumors in other species have been reviewed. The relationships between the spontaneously observed and induced tumors and endocrine factors are reviewed. Practical applications for the use of induced tumors as screening techniques for chemotherapy of advanced renal cancer in man are discussed. Limitations in the present models are apparent. Both hormonal and nonhormonal environments can effect relative growth rates in several species of the induced tumors. Common metabolic pathways in several of the alkylating agents for the inductions of renal tumors are described. The virus relationships to frog tumors have been appreciated for some time and have been further studied. Present clinical problems and controversies, however, remain. Important areas for further study of these models can provide helpful clinical information and have been discussed.
