AbstractRegressive computer analysis was used for prognostic evaluation of immediate and long‐term results of radical treatment of gastrointestinal cancer of different localization. The data were derived from 1,630 patients with gastric, colonic, and rectal cancer who were radically operated upon at N.N. AbstractDouble partial hepatectomy (hepx) evokes an elevation in serum erythropoietin (Ep) levels in anephric hypoxic animals when compared to non‐hypoxic or sham hepx controls. But this Ep response is significantly lower than that found in singly hepx, anephric hypoxic rats.Double hepx also induces numerous cytological changes in the liver. Extravascular accumulation of fat, fibrous scarring, localized necroses, and multiple abscesses, as well as decreased vascularity, occur following the second hepx.A humoral factor was detected in the serum of these animals that is capable of inducing hepatic Ep production when injected into normal rats 18 hours before nephrectomy and hypoxia. This factor, termed hepatopoietin (Hp), was previously demonstrated in the venous serum of singly hepx rats. The serum from animals subjected to double partial hepx is not as potent in inducing Ep production as the serum from singly hepx animals. AbstractTo evaluate the erythropoietic potential of rats during pathogenesis of Shay chloroleukemia (SCL), SCL animals were exposed to hypoxia initiated within two hours postinduction of SCL. Increases in hematocrit (HCT) occurred in both SCL and nonleukemic controls during the first seven days of exposure. This was followed by declines in HCT in hypoxic SCL rats similar to that observed in nonhypoxic SCL animals. HCT of nonleukemic controls remained elevated throughout hypoxic exposure. Results suggested that in SCL, erythropoietic homeostatic response mechanisms are functional prior to terminal phases of this disease, whereas anemia‐producing mechanisms exceed maximum erythropoietic capacity of these animals during later stages of SCL. The HCT of most SCL rats exposed to hypoxia did not fall below 40% before death. This was in contrast to nonhypoxic SCL animals, which became severely anemic in terminal stages. No survival value was observed despite maintenance of red cell mass at or above normal. The pathogenesis of this leukemia was independent of the leukemic animal's erythropoietic potentia

AbstractFour hundred fifty‐three patients are evaluated regarding 10‐year survival rates. Females comprise 66%, and males the remainder (34%). The absolute 10‐year survival rate was 55.2% post therapy. Females had a remarkably better survival (64%) than the males (42%). Age did not affect the prognosis. Staging was most important in that 167 patients with Stage I had a 10‐year survival of 66%, which dropped for the 263 patients in Stage II (24%), and of the 23 patients classified as Stage III, only one (4%) survived 10 years. Of the 202 patients in clinical Stage I, 102 were subjected to an elective groin dissection which revealed occult metastases to lymph nodes in 35, whose 10‐year survival rate was 49%. In 100 patients, no elective groin dissection was performed, and 41 of these developed metastases to inguinal nodes at a later date. The 10‐year survival for that group was 32% — somewhat lower than the 49%. An elective groin dissection is indicated for invasive melanomas. AbstractThe primary mode of spread of basal cell carcinoma is by direct extension. From 1894 to June 1977, 119 cases of metastatic basal cell carcinoma were reported in the world literature. We wish to report five additional cases. It is our feeling that the spread of these tumors is by the interstitial route. It is standard teaching that the interstitial embolic spread of cancer does not occur, but these five cases give strong supporting evidence to the previously reported suggestion that this route of spread may be common. The concept has considerable implications regarding treatment of most cancers.

AbstractOf 77 patients with mesotheliomas, 63.6% had pleural involvement, 31.2% had peritoneal involvement, and 5.2% had involvement at other sites. The presenting symptoms varied with the location of the primary. Surgery and irradiation did not appear to influence the course of the diffuse type. Of 41 patients given systemic chemotherapy, three patients had complete response and four had greater than 50% objective response (median survival 42.5 months); 18 patients had stabilization (median 12.8 months), and 16 showed no response (median 4.5 months). The difference in survival between responders and non‐responders was statistically significant (P<0.001). Of 16 patients who received secondary chemotherapy, four had complete response (median survival 17.5 months), six had stabilization (median 10.5 months), and six had no response (median 2.5 months). Chemotherapy appears moderately effective in the treatment of mesothelioma

AbstractThe malignant carcinoid syndrome (MCS) is a rare complication of malignant carcinoid neoplasms, fewer than 200 cases having been reported since its description by Thorson in 1954. Its signs and symptoms vary with the site of the primary neoplasm, and MCS is now recognized as one member of the large family of endocrine tumors ‐ APUDOMAS).Although MCS remains uniformly fatal, significant palliation can be provided some patients with current surgical and chemotherapeutic management. All signs and symptoms in MCS, except evidence of the destruction of the tricuspid and pulmonic valves, can be reproduced experimentally. The factor causing this lethal complication has not been identified.Two previously unreported cases of MCS, each from the gastrointestinal (GI) tract, are presented. AbstractA simple reliable technique for reconstruction of low‐lying anterior thoracic wall defects following trauma or excision of neoplasms is illustrated by a case presentation and described. The bipedicle transverse abdominal cutaneofascial flap is based on the lateral cutaneous branches of the lower intercostal arteries, which offer an excellent blood supply allowing it to be raised and used even in the presence of radiated tissue.
