AbstractOne‐hundred selected breast cancer patients underwent a laparoscopic liver examination to evaluate this procedure in the management of breast cancer. Group I contained patients with primary breast tumors considered to be localized. Group II consisted of cases which had disseminated disease at initial evaluation or had recurrent disease. In group I, 8.5% showed macrometastases less than 1 cm in size at laparoscopy in clinically uninvolved livers. This converted a presumed localized lesion to stage IV disease and was managed accordingly. Cases evaluated as high‐risk cases due to local factors, however, were not found to have liver involvement at laparoscopy, and hence adequate local therapy could be undertaken. In group II, metastatic liver involvement detected at laparoscopy was 22.5%. AbstractApproximately 1.3% of patients with lymphoma develop obstructive jaundice secondary to lymphomatous involvement of the extrahepatic biliary system. This may occur either as an initial or as a late manifestation of disease. Clinically and radiographically the condition may mimic a variety of more common causes of obstructive jaundice. Surgical exploration may be necessary to confirm the diagnosis, but local radiotherapy would appear to be the preferred mode of treatment. Rapid progression to systemic disease occurs in the majority of patients, necessitating multidrug chemotherapy. Control of jaundice by radiotherapy is good, but long‐term prognosis is poor. Supervening sepsis and gastrointestinal bleeding caused the majority of deaths, suggesting that adjunctive nutritional support, immunologic enhancement, and stress ulcer prophylaxis may be necessary if survival is to be improve

AbstractThe failure of chemotherapy often results in the outgrowth of drug‐resistant tumor cells, and, since chemotherapy is often combined with immunotherapy, the question arises as to whether immunity directed against the original tumor offers any protection against the drug‐resistant tumor cells. To approach this problem, the immunological relationships between a mouse glioma (GL26) and two FUdR‐resistant transplantable tumor sublines were studied. Immunity was induced in C57BL/6 mice with neuraminidase‐altered GL26 tumor cells. Immunized mice challenged with viable GL26 tumor cells were completely protected, and no tumors grew. Immune mice, when rechallenged with cells from the two FUdR‐resistant tumors, gave greater protection against one of the tumors than the other, but not as much as against the original tumor. The results indicate the drug‐resistant tumor cells have some immunological properties similar to the original tumor line and that further studies such as these might be of value to the staging of chemoim

AbstractExtramedullary plasmacytoma occurs infrequently as compared to osseous myoloma. A single case of extramedullary plasmacytoma was seen during the last 10 years whose initial presentation was that of a soft tissue sarcoma of the chest wall. AbstractA case of renal cell carcinoma presenting as a nosebleed of three‐week duration is described. Light microscopy of a nasal lesion showed a richly vascular tumor, forming glands and uniformly consisting of clear cells. Cytochemically and ultrastructurally, the presence of abundant lipid droplets and glycogen within the neoplastic cells indicated a renal origin. AbstractSialyltransferase enzyme levels (Sialtx), CEA, and 10 standard laboratory tests were studied in 50 patients with treated, recurrent, or disseminated breast carcinoma. All groups of patients had elevated mean Sialtx activity (CPM/mg protein) as compared with normal controls. Sialtx levels (expressed as % of normal controls) of patients with disseminated lesions were significantly elevated as compared to patients without evidence of disease or with only soft tissue recurrences, whereas CEA and alkaline phosphatase levels were significantly elevated (peripheral blood lymphocyte count, total protein and albumin levels significantly decreased) in patients with recurrent disease of all sites. Among patients with disseminated breast cancer and normal CEA levels, about 50% had markedly elevated Sialtx activities. From our limited experience, it appears that Sialtx study is of value when CEA failed to indicate the presence of breast malignancy. Further testing including serial studies should be done to better define its clinical usefulness.

AbstractPreoperative serum immunoglobulins were measured in 102 patients with colorectal carcinoma. There were no differences in the mean values of IgA, IgM or IgG between stages. There was an association, however, between advanced stage of disease and a combination of low levels of IgA (≤200 mg/dl) or high levels of IgM (≥125 mg/dl). AbstractThe classical concepts pertaining to the natural history of mammary carcinoma and its treatment must be reviewed if survival rate is to be improved. By the time diagnosis is established, a fair amount of cases of carcinoma of the breast present distant, clinically undetectable micrometastases, which will not be affected by local therapy (surgery or radiation). Increasing evidence shows that mammary carcinoma nowadays must be looked upon as a diffuse disease affecting both breasts. Postoperative radiation does not improve the 5‐ and 10‐year survival rates and has a negative effect upon the immunological defense of the patient. Immunology plays an important role in the evolution and cure of the patient with carcinoma of the breast. The mutilation produced by radical or ultraradical mastectomy may throw the patient into mental depression which, as a consequence, may decrease the immunological competence of the patient. Multiple drug chemotherapy, started at the time of surgery and continued for several months thereafter, may kill micrometastases.
