AbstractGeneral immunocompetence was examined in 125 patients with various stages of breast cancer. Tests include peripheral blood lymphocyte count, serum protein electrophoresis, quantitative immunoglobulins, CEA level, and delayed cutaneous hypersensitivity (DCH) reaction to six recall antigens (PPD, Monilia, mumps, Varidase, histoplasmin, and coccidioidin). About one third of the patients responded positively to each of the first four antigens, whereas 12% reacted to histoplasmin and 4% to coccidioidin. In this study, DCH to recall antigens had no prognostic value because the distributions of skin test reactivity were similar among patients with different stages of breast carcinoma. And among patients with similar stage of disease the relapse rates were similar for those who reacted and those who were anergic. But peripheral lymphocyte count had definite prognostic value because patients with advanced stage (group III/IV) and those who were anergic had significantly lower lymphocyte counts. AbstractHyperthermia, either alone or combined with chemotherapy, has been shown to be effective in treating cancer. Because some investigators believe that regional hyperthermia may be more effective than whole body hyperthermia, we developed a technique to heat only the liver to 42‐43 degrees for 4 hr. The procedure was adapted from a previously described animal model and was performed in four humans. Vascular isolation of the liver was accomplished by cannulating the hepatic artery, the portal vein, and the inferior vena cava followed by occluding the suprahepatic vena cava and the liver was then perfused with blood and nutrients from an oxygenated reservoir. Preliminary results show radiologic and histologic evidence of tumor necrosis or cessation of tumor growth in three of the patients. AbstractEight patients with metastatic carcinoid tumors and the carcinoid syndrome were treated with gelatin foam embolization of the hepatic arterial tree. The aims were to reduce the tumor mass in the liver and to eliminate the carcinoid syndrome. The effects of the treatment were judged from arteriograms, CT scans, and the levels of serotonin in blood and 5‐HIAA in urine, as well as from the clinical symptoms. The mean follow‐up time was 12.5 months. In all patients the liver tumor mass was reduced after embolization, and this reduction persisted for at least 6 months in seven patients. After treatment, reduced serotonin levels in blood were measured in four patients and reduced 5‐HIAA levels in urine in seven patients. In five patients the carcinoid syndrome disappeared after embolization, but after 6 months two of these five patients had regained symptoms. Adverse reactions were minor consisting of a slight fever, reversibly increased serum levels of liver enzymes, and abdominal pain. AbstractLeukocyte adherence inhibition (LAI) has been demonstrated to be a simple, rapid, and reliable technique in the diagnosis of various malignancies including hepatocellular carcinoma. The LAI test was carried out employing modified tube LAI technique in patients with primary hepatocellular carcinoma (HCC) and secondary carcinomas of liver. Positive LAI response to HCC antigen was obtained in all six (100%) cases of hepatocellular carcinoma tested. None of the control cases, which included 8 healthy subjects and 16 cases of benign liver diseases, gave positive LAI response. Two out of 19 cases of secondary carcinoma gave a positive LAI reaction to HCC antigen. In secondary carcinomas, 19 out of 21 cases (90.48%) gave positive LAI reaction to secondary adenocarcinoma antigen. There were two false positives in controls (1 each of cirrhosis and amebic liver abscess), and 1 out of 8 cases of HCC also gave positive response to secondary carcinoma antigen. Thus, LAI test was found to be useful in the diagnosis of hepatic malignancies.

AbstractTwenty‐nine patients treated with postoperative radiotherapy for malignant tumors of the parotid gland were reviewed at the Joint Center for Radiation Therapy. Most patients were treated between 5,000 and 6,500 rad. All were treated because of microscopic residual disease, extra capsular extension, or tumor close to the facial nerve. The overall results showed one in‐field failure, two marginal recurrences, and eight patients failed distantly. Poor prognostic factors included high‐grade, extracapsular extension, and nodal involvement. We conclude that patients with malignant tumors of the parotid should be treated with postoperative radiation therapy if any of the bad prognostic signs are present. AbstractAlthough the entire ureter and renal collecting system are accessible for endoscopic treatment with a neodymium:YAG, (Nd:YAG) laser, specifications for safe laser application within the ureter are not established. After various doses of Nd:YAG laser energy were delivered, the midureters of six mongrel dogs were studied to determine tissue effects. Doses which are appropriate within the bladder (35 watts × 2 seconds) resulted in perforation and urine leak from the ureters. AbstractThe immune status of rats with chemically induced colorectal tumors was investigated for 8 weeks following treatment by resection, resection plus 5‐fluorouracil (5‐FU), and resection plus levamisole. Normal rats without tumors were given identical treatment and acted as controls.Tumor rats regained their ability to respond to an allograft of allogeneic lymphocytes after surgical excision of the tumor for a short period. Adjuvant Levamisole treatment enhanced this responsiveness, but adjuvant 5‐FU depressed it.The spleen and lymph node cells of tumor‐bearing rats showed greater spontaneous and PHA‐induced cytotoxicity for chicken red blood cells than control rats (P<0.001), and adjuvant therapy did not alter this response. However in control animals, levamisole treatment did augment the antibody‐dependent cytotoxicity of their spleen and lymph node cells. Rats receiving tumor resection alone were immune by leukocyte migration inhibition at 8 weeks only, while those receiving either adjuvant exhibited an immune response to the same homogenate for 4

AbstractA rare case of malignant fibrous histiocytoma of the retroperitoneum with scalp metastases as the initial presentation is described. The scalp lesions were mistaken for atypical schwannomas, however subsequent investigation revealed a large retroperitoneal tumor and multiple hepatic and pulmonary metastases.

AbstractFrom 1961 to 1980 17 patients with nasopharyngeal carcinoma who were less than 30 years old at time of diagnosis were treated at the two oncologic hospitals in Puerto Rico. Fourteen of the tumors were lymphoepitheliomas and the remaining three were anaplastic carcinomas. Ninety‐four percent of the patients were found to have neck adenopathy at the initial examination and in 65% of them the adenopathy was bilateral. The 5‐year actuarial survival was 14. The main cause of failure was the distant metastasis. Seventy‐one percent of our patients developed distant metastasis sometime during the course of their disease.Carcinoma of the nasopharynx is uncommon in Puerto Rico as in other western countries. The post‐adolescent age incidence peak reported from different countries is also present among Puerto
