AbstractLymphocyte bioenergetics in cancer patients undergoing surgery is largely unexplored. The effect of surgery on total lymphocyte counts and adenosine triphosphate (ATP) production is reported. Lymphocyte ATP was assayed by the coupled enzymatic method using hexokinase and glucose 6‐phosphate dehydrogenase and lymphocyte counts were calculated from total leukocyte counts and differential counts.Lymphocyte ATP assay and counts were carried out in 13 patients pre‐ and postoperatively, and lymphocyte counts were scored in six more patients similarly. In 69% of the patients lymphocyte ATP production was depressed postoperatively, and this condition appeared to be associated with cancer spread and immunosuppressive tumors. Postoperative increase in lymphocyte ATP was observed following hysterectomy, colostomy, and also after blood transfusion. The significance of the findings are discussed. AbstractFrom September 1969 through 1979, 68 patients with already locally metastasized malignant melanomas of the extremities were treated by hyperthermic regional perfusion and local excision(s). Of these, 53 patients could be evaluated after a follow‐up of at least 2 years: 31 had been perfused with melphalan and 22 with melphalan in combination with actinomycin D. Local recurrence rates were about the same in both groups: 32 versus 33%. The same applied to the symptom‐free interval between perfusion and local recurrence: about 12 versus about 13 months. Virtually all recurrences in both groups (90 versus 85%) were seen within 2 years. The interval between perfusion and systemic metastization was also roughly the same: about 25 versus about 23 months. These results indicate the need for better chemotherapeutics in order to achieve further improvement. AbstractThe relationship of acute appendicitis occurring previous to cancer in colon and rectum was studied in the consecutive records of 561 patients, of 40 years of age and older, operated upon with an appendectomy because of acute appendicitis. Sixteen (2.9%) of these patients were readmitted within three years because of a carcinoma in colon or rectum. The incidence of carcinoma in the colon and rectum in the population, of the same age, is only 0.1%, according to the Swedish Cancer Registry (1). This difference is statistically significant. Where acute appendicitis and colon carcinoma co‐exist, the danger is that the carcinoma may be missed. AbstractThe purpose of this study was to determine whether or not alternations in tumor growth induced by changes in thyroid status were mediated through changes in key enzymes, whose activity is known to be influenced by thyroid hormones. The activities of three lysosomal enzymes (cathepsin B1, cathepsin D, and acid phosphatase) and thymidylate synthetase were measured in implanted mammary tumors as well as in the livers of host animals that were either euthyroid, hypothyroid, or hyperthyroid. Hypothyroidism produced no significant change in enzyme activity in the tumors. Hyperthyroidism, on the other hand, did cause a significant increase in the activity of all lysosomal enzymes in the tumors, but did not affect thymidylate synthetase levels. In the livers of the host animals, hypothyroidism produced a significant decrease in cathepsin B1and a significant increase in acid phosphatase but did not change cathepsin D or thymidylate synthetase levels. Hyperthyroidism produced‐a significant increase in all enzymes measured in the livers of the host animals. The significant decrease in tumor weight with hypothyroidism did not correlate with the insignificant changes in the enzymes tested. AbstractThe utilization of Breslow‐Clark microstaging has proved helpful in deciding whether or not to perform elective regional node dissection for patients with melanoma. The marked biologic diversity of this disease, however, mandates a strict therapeutic policy of individualization with respect to prognostic variables and special clinical presentation. Specifically, melanomas associated with ulcerations, truncal location, and regional node involvement appear to be more aggressive. Lesions with cutaneous penetration more than 1.5 mm in depth, and showing blood and lymphatic vessel invasion are more often associated with regional node involvement and worse prognosis. The association of any of these adverse prognostic signs with an intermediate thickness lesion (0.76–1.5 mm) may justify elective node dissection. AbstractPatients with a diagnosis of squamous cancer of the upper aerodigestive (UAD) system were tested with extracts of squamous cancer arising from several regions of the UAD tract and compared to various control populations, using the slide leukocyte adherence inhibition (LAI) assay. Seven tumor extracts were tested. Twenty‐five of 77 tests (32%) were positive in unselected UAD squamous cancer patients. However, matching patients with tumor extract by the site of origin within the UAD tract, 19 of 22 tests (86%) were positive when the site of origin was identical for leukocyte donor and extract. Two of four patients tested with an extract prepared from a different site within the same region of the UAD system were positive; whereas only 4 of 51 tests (8%) were positive in patients tested with an extract from a cancer arising from a different region within the UAD tract (P<0.001). One of 33 tests using patients with either no cancer (11 tests), squamous cancer of other regions (15), or adenocarcinoma (7) reacted to the extracts of UAD tract cancers. AbstractSerum immunoglobulins (IgG, IgA, and IgM) have been studied in 70 patients with squamous cell carcinoma of oral cavity and in 40 age‐matched normal controls. The results were correlated with clinical stage, tumor size, lymph node status, tumor differentiation, and treatment modalities. There was a significant rise in serum IgM and IgA in oral cancer patients as compared to controls. It was further raised in advanced clinical stage, but tumor size and histological differentiation and treatment modalities (surgery and chemotherapy) do not seem to affect serum immunoglobulin levels. The observations implies that the extension and dissemination of tumor mass is the important factor for the change in serum immunoglobulin level in squamous cell carcinoma of the oral cavit
