AbstractBetween 1977 and 1984, 50 patients with Hodgkin's disease underwent a staging laparotomy performed by nine surgeons in a community hospital. Adequate procedures were performed in 80% of cases compared to staging laparotomies done between 1969 and 1976 when only 40% were properly performed. Abdominal lymphangiogram had a false‐negative rate of 0 but a false positive rate of 70%. Clinical stage III disease was significantly downstaged at laparotomy (65% of cases). Postoperative complication rate was 4% and there were no operative deaths. A subset of patients not requiring laparotomy have been identified. Because the quality of staging laparotomy and lymphangiography was variable, we encourage all centres treating patients with Hodgkin's disease to review their own experience with these techniques before making individual patient treatment decision

AbstractTumor‐infiltrating lymphocytes (TIL) and tumor‐associated macrophages (TAM) were recovered from 22 patients with primary lung cancer. The TIL did not exhibit any cytolytic activity against various target cells. The average percentage of TIL recovered was 6.7 ± 1.3% in patients without recurrence, as compared to 3.6 ± 0.6% in those with recurrence within 2 years after complete resection of lung cancer (P<0.05). The average cytostatic activity of TAM was 38.5 ± 6.8% in patients without recurrence, although it was 25.2 ± 4.6% in those with recurrence within 2 years after complete resection (P<0.1). The activity of macrophage activating factor (MAF) induced by TIL was also higher in patients showing no recurrence. A significant positive correlation was observed between MAF activity induced by TIL and the cytostatic activity of individual TAM. AbstractFrom January 1978 to April 1983, 53 patients were treated with cisplatin‐vinblastine‐bleomycin chemotherapy because of advanced nonseminomatous testicular tumor (NSTT). After the chemotherapy, the serum tumor markers were back to normal in 41 patients, of whom 35 were eligible for surgical removal of the residual tumor. In four patients, vital tumor tissue was found in the residual tumor. Salvage chemotherapy resulted in complete remission. Residual mature teratoma was encountered after the chemotherapy in 15 of the 25 patients with a teratomatous component and in one of the ten patients without a teratomatous component in the primary tumor. On completion of the study, 38 of the 53 patients (72%) are still alive, with a median follow‐up of 65 months. Subdivided by tumor volume, survival is found to amount to 92% for small‐volume disease, 67% for large‐volume disease, and 64% for very‐large‐volume disease. Six patients (11%) developed a recurrence in the course of the follow‐up. Exploratory laparotomy after remission induction chemotherapy is necessary in all patients with a teratomatous component in the primary testicular tumor who have become tumor marker negative, irrespective of the roent‐genographic findings of the retroperitoneum. Patients without a teratomatous component in the primary tumor should have exploratory laparotomy only in case of roentgenographic evidence of retroperitoneal residual tumor. AbstractWe have evaluated the effect of adjuvant chemotherapy on time to recurrence and survival in two prospective trials of women with stage I uterine sarcomas. The first trial compared surgery only to surgery plus Adriamycin. The 5‐year estimated survival rate was 36% for surgery alone and 63% for surgery plus Adriamycin. The 5‐year recurrence free rate for surgery alone was 46% as compared to 75% for surgery plus Adriamycin. The second trial, without a concurrent control group, included patients with stage I uterine sarcoma and adjuvant cyclaphosphamide, vincristine, Adriamycin, and dacarbazine (CYVADIC) chemotherapy. The 5‐year survival rate was 89% and the recurrence‐free rate was 80%. AbstractOrnithine decarboxylase (ODC) activity in biopsy specimens of normalappearing rectal mucosa was measured in 15 control patients, 5 patients with colon adenomas, and 11 patients with colon cancer. While women had significantly higher ODC activity than men, ODC activity was increased regardless of gender in the rectal mucosal biopsies of patients with benign or malignant colonic neoplasia compared with those of controls. AbstractTopical 5‐fluorouracil (Efudex®) has been shown to be of great value in the treatment of skin cancers, Marjolin ulcers, and advanced squamous cell carcinoma of the mouth and esophagus. It has been advocated for use on patients with melanocytic dysplasia and lentigo maligna melanoma. This report confirms the use of topical 5‐FU for lentigo maligna melanoma and melanocytic dysplasia. AbstractWe employed southern analysis to examine homology between yeast genes RAD 51, RAD 52, RAD 54, and RAD 55 for possible gamma repair genes in radioresistant or repair proficient human tumor cell lines and normal placental DNA. No homology was observed; however, other strategies including further gene restriction and transfection are underway to identify repair genes in human tumors. Understanding mechanisms of radiation repair might lead to more effective clinical radiation treatment protocols.

AbstractA cyst in the left lower lobe of the lung was removed from a 15‐month‐old girl and showed evidence suggesting a preexistent congenital cystic adenomatoid malformation; in addition a rhabdomyosarcoma was identified. Primitive mesenchyme and mesenchymal elements also were seen. The origin of the rhabdomyosarcoma is discussed. AbstractLiver involvement with Hodgkin's disease is determined at our institution by a combination of wedge biopsy and needle biopsy at the time of staging laparotomy. This retrospective study was undertaken to determine whether the wedge or the needle biopsy method is the more accurate. All staging laparotomies (n = 116) during the period 1972–1986 were reviewed. Twelve patients were found to have liver involvement with Hodgkin's disease; 2 had had wedge biopsies only and ten had had both wedge and needle biopsies. In all ten with both wedge and needle biopsies, the wedge biopsies were positive for liver involvement, but in only five were the needle biopsies positive. This study indicates that wedge biopsy is the more accurate method for detecting liver involvement in these patients, and we believe it should always be a part of the staging laparotomy for Hodgkin's diseas
