AbstractA case is reported in which multiple bilateral pulmonary metastasectiomies for metastatic adrenal cortical carcinoma were performed and which resulted in an unusually long survival of over 25 years from the first thoracotomy and over 28 years from removal of the primary adrenal cortical carcinoma. AbstractA total of 85 patients with recurrent cervical cancer were reviewed: 17 patients with recurrences were treated by radical surgery, 18 by radiotherapy, 29 by chemotherapy, and 21 cases received no further treatment. Survival was presented according to the site of recurrence and the mode of therapy. All patients were followed for a minimum of 24 months after recurrence.Of the total group, 14% are living without evidence of disease, 29% died of metastatic disease with no involvement in the pelvis, and 45% died of pelvic cancer. Overall, 22% were living more than 2 years, and only 2% lived over 5 years after recurrence.The NED (no evidence of disease) rate for radical surgery group was 47 and 44% for the radiotherapy group. There were no significant differences in median survival between the chemotherapy group and the notreatment group (6.8 versus 4.8 months). New chemotherapy agents and adjuvant systemic therapy are discussed.

AbstractA retrospective chart review was undertaken on all Hartford Hospital Tumor Registry patients under age 40 with a diagnosed adenocarcinoma of the colon or rectum between the years 1955 and 1980. Of the 45 patients who could be accurately staged by the Dukes classification, only about one‐third had disease localized to bowel. No unique histological pattern was observed. An excess of multiple primary tumors was observed (13%). Stage of disease at the time of presentation, rather than histology, was the most important prognostic factor in this study. AbstractThe lymphoplasmacytic infiltrates of 400 bening, borderline, and malignant skin tumors were assessed by light microscopy to determine the relationship between the presence of tumor ulceration and the histologic character of the infiltrate. Squamous cell carcinoma had the highest overall number of infiltrating lymphoplasmacytic cells, while seborrheic keratoses had the lowest. Plasma cells in the infiltrates were related significantly to tumor ulceration (P<.001). This suggests that the plasma cell infiltrates may be more related to tumor ulceration than to a tumorspecific response in these carcinomas. Small lymppocytes were most evident adjacent to areas of irregularity in the peripheral palisade or of squamous differentiation of the basal cell carciomas. This suggests that the presence of small lymphocytes, not plasma cells, is a response by the immunocompetent host to areas of tumor invasion.

AbstractUnusual lung tumours are not simply pathological curiosities. They demonstrate features of major significance in diagnosis, treatment, and prognosis. Six of these tumours are discussed: (1) Carcinosarcoma is rarely found in the lung. The histogenis of the lesion is unclear and the prognosis is poor. (2) Only three cases of pleomorphic adenoma have previously been described. Differentiation from other “mixed tumours” of the lung is essential. (3) A rare case of bronchial adenoma producing ectopic ACTH is described. Early recognition of these polypeptide hormone‐secreting tumours is streessed. (4) Oat cell carcinoma with the myasthenic (Eaton‐Lambert) syndrome shows the clinical features which should permit early tumour diagnosis. The hazards of muscle relaxants must be recognized. (5) Prostatic carcinoma with endobronchial metastases is discussed. The importance of localization of the primary tumour is emphasized. (6) An example of double primary carcinoma is presented. AbstractA 48‐year‐old woman is described with a pancreatic islet cell carcinoma that had no clinical evidence of hormone production. This neoplasm was removed by radical distal pancreatectomy. Immunohistochemical and extraction studies performed on the tumor tissue revealed that it was producing serotonin and βHCG. Circulating levels of these hormones were elevated in preoperative serum samples that had been frozen. βHCG, a known tumor marker, has remained normal following operation, and this mirrors her clinical course since she has no evidence of disease 1.5 yr after operation. This patient also had focal nodular hyperplasia of the liver. AbstractIn this preliminary report, evaluation of nontraditional agents has been used to evaluate control of metastasis. This androgen‐sensitive rat prostic adenocarcinoma has been subject to the treatments of heparin, indocin, and castration, as well as combinations of castration, heparin, indocin, and cyclophosphamide. Tumor volume, metastasis and determination of regression of primary tumor have been evaluated. Heparin and indocin both reduced metastasis without affecting tumor volume in this study. The combination of castration and cyclophosphamide was most effective in reducing tumor volume when compared to the other group

AbstractAlthough gut‐associated lymphoid tissue in the form of discrete lymphoid patches (LP‐GALT) in mammalian intestine is most prominent in the distal ileum, appendix, and, in some species, the cecal appendage, LP‐GALT can be found throughout the intestinal tract. LP‐GALT appears as single or multiple subepithelial lymphoid follicles covered by a specialized, structurally unique epithelium. In the colon of the Wistar/Furth(W/Fu) rat, LP‐GALT appears as aggregates of follicles, or lymphoid patches, that can be detected macroscopically. We studied the relationship between 1,2‐dimethylhydrazine‐ (DMH) induced colon carcinomas and the lymphoid patch associated epithelium in these animals. In addition, we defined the normal distribution of colonic lymphoid patches in both DMH‐treated and control rats. Patches were found macroscopically and confirmed by histologic examination at five constant sites: lower pole of cecum, proximal ascending colon, the major colonic flexure, mid descending colon, and the rectosigmoid. These are also the predominant sites of DMH induced carcinomas in W/Fu rats. In 120 DMH‐treated animals, 109 colon carcinomas were found. Eight percent were in the lower pole of the cecum, 56% in the proximal ascending colon, 16% at the major flexure, 15% in the mid descending colon, and 5% in the rectosigmoid. Lymphoid patches could often be detected histologically in association with DMH‐induced tumors. The depth of tumor invasion was found to correlate inversely with our ability to identify tumor‐associated lymphoid patches suggesting that tumors arising at the anatomical sites where lymphoid patches occur progressively destroyed them. Of colon tumors confirmed histologically to be associated with lymphoid patches, 88% were superficial lesions confined to the submucosa and 12% were more extensive but confined to the bowel wall. No lymphoid patches could be found associated with tumors that extended through the bowel wall. AbstractTen cases of male breast carcinoma are reviewed. Male patients with breast cancer on the average are 10 years older than females, and they delay twice as long as females after noting a breast mass before seeking medical attention. Because of this patient's delay, male breast cancer tends to be more advanced at the time of diagnosis and initial therapy than female breast cancer.

AbstractA Retrospective Review Is Presented Of Nine Patients Presenting With Solitary Plasmacytoma. Between 1963 And 1980, These Patients Were Treated With Radiation At The Department Of Radiation Oncology, University Of Louisville. Criteria For Admission To This Particular Study Include (1) A Solitary Focus Of Plasmacytoma Proven By Biopsy, (2) Normal Bone Marrow Findings (Less Than 10% Plasma Cells), And (3) No Evidence Of Disseminated Disease. In Six Patients The Primary Site Was Osseous, And In Three Extramedullary, Two Of Which Were Located In The Nasopharynx And Nasal Cavity And The Third In The Stomach. All Of The Extramedullary Plasmacytomas Are Disease Free For Periods Ranging From 4 To 10 Years. Of The Six Patients With Osseous Lesions, Two Developed Multiple Myeloma In 2 And 3 Years, Two Are Ned (No Evidence Of Disorder) After 9 Years, One Died Of Intercurrent Disease, And The Remaining Patient Was Ned For 2 Years, After Which He Was Lost To Follow‐Up. These Results Suggest The More Favorable Prognosis Of Extramedullary Plasmacytoma And Support The Theory That The Solitary Plasmacytoma Of Bone And Extramedullary Plasmacytoma Are Distinct Disease Entitie
