AbstractThe authors investigated social and occupational rehabilitation of patients treated at N.N. Petrov Research Institute of Oncology for cancer recurrences in colon and rectum (96 cases), for metachronic neoplasms (40 cases), and also for patients accepted for coloplasty after Hartmann operation (11 cases). The term of hospitalization in cancer recurrences was 15.9 ± 2.5 months; in metachronic tumors, 77.1 ± 9.9 months; and for the patients accepted for coloplasty, from 6 to 12 months. The period of patients' survival after radical operations in cases of metachronic tumors is twice as long as in recurrences and approaches, on the average, five years. The patients operated upon for their metachronic tumors are characterized by a high degree of social and occupational readaptation (about 80%) in comparison with patients with tumor recurrences (30%). All the patients who had undergone coloplasty were subsequently completely readapted from a social point of view, which was due to the elimination of a fecal fistul

AbstractSurgical methods of diagnosis make it possible to determine in patients with lung cancer the actual local extension of tumour and its metastatic foci. The diagnostic thoracotomy plays an important role in the complex diagnosis of lung cancer.Diagnostic thoracotomy has been performed in 109 out of 1215 patients with lung cancer and other lung lesions suspicious for cancer. Primary lung cancer, mostly at early stage, was detected in 50 out of 109 patients. Among 308 patients who had been subjected to Daniels operation, metastases were revealed in 130 cases (42.2%); prescalene lymph nodes were found inpalpable in 12.4% and palpable in 64.4% of cases.Mediastinoscopy carried out in 262 patients revealed metastases in 36.2% of cases. Parasternal mediastinotomy allowed definition of the degree of tumor extension in 49.5% of cases. Application of laparoscopy and laparotomy revealed metastases in the organs of the abdominal cavity and retroperitoneal space in 13.5% of patients.Results of studies show a definite relationship between the frequency of metastases development in the lymph nodes and distant organs and tumor site as well as clinico‐anatomical forms and morphological structure of tumor. AbstractA simple method of temporary reconstitution of the cervical esophagus in major head and neck surgery is detailed in a case report. Accompanying diagrams illustrate the mechanics of inserting a number 26 nasopharyngeal airway from the hypopharynx to the esophagus. This arrangement simplifies the management of oral secretions until permanent reconstruction of the cervical esophagus can be accomplished.

AbstractFour thousand seven hundred twenty‐eight autopsies performed at the Roswell Park Memorial Institute were reviewed to determine causes of death from cancer. The duration of the disease from the date of diagnosis to death, the primary site of the tumor, a detailed description of the presence or absence of metastatic tumor at a series of sites, and an estimate of the total tumor mass were included in the evaluation. Twenty‐three common primary sites were selected for investigation. These sites represented 99% of the total series. Nine causes of death were described. These were respiratory failure, infection, toxic drug reaction, hepatic failure, renal failure, shock or heart failure, adrenal gland failure, electrolyte imbalance, and central nervous system failure. AbstractThe records of 43 patients with histologically proved primary gastric sarcoma treated at M. D. Anderson Hospital and Tumor Institute between 1945 and 1975 were reviewed. Weight loss, abdominal pain, and hematemesis or melena were the most common symptoms, and palpable epigastric mass was the most common physical sign. Ninety‐five percent of the gastric tumors were leiomyosarcomas. The median survival time and the five‐year survival for the 29 patients who had curative gastrectomy were 33 months and 38%, respectively. Morphologic and histologic factors that adversely influenced the length of disease‐free interval and survival following curative surgery included 1) primary tumor 8 cm or greater in diameter, 2) tumor extension to serosa of the stomach, and 3) low grade of differentiation of the primary tumor. Seventy‐seven percent of recurrences in patients with curative tumor resection occurred within two years of resection of the primary. Of the 17 patients with advanced disease who received chemotherapy, two (15%) had objective tumor regression of greater than 50% and survived 16 and 24 months, compared with a median survival time of eight months for patients who failed to respond. AbstractSkin cancer, the most common malignancy in white patients, is rare in black populations. Seventeen black patients have been diagnosed and treated for basal cell carcinoma in the past 20 years at the University of Mississippi Medical Center. Ten of them have died, six of various types of cancer. Of the seven living patients, one had two cancers at the time of study: a new basal cell carcinoma and generalized lymphoma. The majority of patients had some degree of mixed racial ancestry, with medium to light brown skin, a history of heavy sun exposure, and lesions appearing on the head or neck. Highly significant depression of cellular immunity was demonstrated in these patients by T‐cell assay. AbstractThe aim of this study was to investigate the compound 4‐nitro‐1‐cyclohexyl‐3‐ethoxy‐2‐oxo‐3‐pyrroline (NOPYE) and some related compounds for skin sensitization in guinea pigs, as the first step in a search for more effective skin sensitizers for immunotherapy of cutaneous tumors. In guinea pigs, NOPYE and NOPYE‐L‐alanine produce far milder delayed hypersensitivity reactions than DNCB. Both NOPYE and DNCB fail to act as adjuvants for skin sensitization to tuberculin purified protein derivative (PPD) and ovalbumin (OV). This suggests an explanation for the lack of effectiveness of DNCB in immunotherapy of metastases: DNCB may be relatively ineffective as an adjuvant for production of specific antitumor immunity. Such adjuvant activity may be essential if the action of the immunotherapeutic reagent is not to be confined to its site of application but is to be effective at the site

AbstractIncubation of blood mononuclear cells with sheep erythrocytes at 29°C identifies “high‐affinity” rosette‐forming T‐cells (RFC). The proportion of highaffinity RFC was more frequently depressed in patients with cancer than the total number of RFC identified under optimal rosetting conditions. Our purpose was to determine the effect of chemotherapy and/or immunotherapy on the level of RFC and to determine whether trends in serial values had prognostic significance.Two hundred thirty‐seven of 270 normal individuals tested had 29°C RFC levels greater than or equal to 47%, while 21 of 41 untreated cancer patients had higher levels. Cancer patients with depressed 29°C RFC usually normalized within four weeks of operation. Serial studies of 29°C RFC were performed on 32 stage I and II lung cancer patients. Eight patients have developed recurrent tumor. We noted a pattern of falling rosette values (two or more consecutive values within normal limits followed by two consecutive depressed values) in five of these patients an average of five months before clinical relapse. Fifteen patients who have remained disease free either maintained normal levels of rosettes or increased into the normal range. Four of 13 (31%) stage I and four of nine (44%) stage II breast cancer patients had depressed rosette levels preoperatively. In seven stage II patients with no further therapy after mastectomy, only two of 27 serial tests showed low RFC. Twenty‐six stage II patients receiving L‐phenylalanine mustard (L‐PAM) therapy after mastectomy were depressed in 18 of 90 (20%) serial tests. Twenty‐one patients receiving cyclophosphamide, methotrexate, and 5‐fluorouracil (CMF) after mastectomy were depressed in 25 of 73 (34%) serial tests. In contrast, seven patients treated with CMF and additionally with Corynebacterium parvum after mastectomy were depressed in eight of 27, or 29%, of serial tests. The 29°C E‐rosette assay gives reproducible counts of a T‐cell subset. Serial monitoring of patients with “high‐affinity” rosette‐forming cells may predict relapse earlier than clinical evaluation. AbstractPhytohemagglutinin‐induced blastogenesis is used clinically as an estimate of immune competence but quantitative application is problematic since there is considerable variability. We sought to define for five normal subjects the degree of variability in repetitive testing using a constant culture methodology. A sixday growth response was determined as was the dose response to seven concentrations of PHA. Growth time to peak3H‐thymidine (3HTdR) incorporation was not constant and that PHA dose stimulating maximum3HTdR incorporation varied by 2–16 fold for the retesting of subjects. A pooled dose response was not stabilizing. There were statistical differencesamongsubjects for PHA dose optimum (P<0.01), maximum evoked3HTdR incorporation (P<0.05) and with varied plasma lots (P<0.001).
