AbstractTwo patients in whom the development of a malignant pericardial effusion lead to the diagnosis of an unsuspected bronchogenic carcinoma are presented. Pericardial involvement has rarely been described as the first manifestation of an underlying lung tumor. Histologic type in both cases was adenocarcinoma, the most commonly described pattern in this situation. In concurrence with previous reports, prognosis of patients with malignant pericardial effusion remains poor.

AbstractThe long‐term results were investigated in 46 patients of bladder cancer treated by cystotomy and peroperative irradiation. This combination therapy appears to produce grossly comparable long‐term survival results with surgery and radiotherapy. AbstractLiver metastases due to the more common neoplastic diseases such as colorectal, breast, or bronchogenic carcinoma are a frequent occurrence and are associated with an ominous prognosis. Earlier detection followed by appropriate therapeutic interventions might have a decided effect on the subsequent course of disease. Controversy exists over the selection of tests with the greatest sensitivity, specificity, and potential utility. Preliminary evidence suggest that γ‐glutamyl transpeptidase and 5′‐nucleotidase may be of particular significance. Four enzymes — γ‐glutamyl transpeptidase, 5′‐nucleotidase, leucine aminopeptidase, and alkaline phosphatase plus carcinoembryonic antigen — were compared in the same blood samples from selected patients with breast and small cell carcinoma of the lung. γ‐Glutamyl transpeptidase was the most sensitive test with 28/29 (97%) patients with hepatic metastases having elevated enzymatic activity in their sera. For patients with small cell carcinoma of the lung followed serially, γ‐glutamyl transpeptidase activity was increased an average of 5 months before liver metastases were detected by clinical means. Two factors are important in the interpretation of the results of γ‐glutamyl transpeptidase analysis: (1) Hepatic dysfunction due to diseases other than metastatic tumor involvement can cause a rise in enzyme levels as can (2) medications or ethanol which activate the hepatic microsomal drug metabolizing system. Of particular importance, however, is the fact that antitumor chemotherapy, even intensive and multiple agent, did not appear to effect the enzyme activity in the sera of patients with breast or small cell carcinoma of the lung. γ‐Glutamyl transpeptidase in combination with carcinoembryonic antigen may be of particular value in detecting liver metastases and in assessing s

AbstractSurgical procedures for breast cancer were compared in 468 cases with insitu and 4,967 cases with invasive lesions. The cases were obtained from the Michigan Cancer Foundation Registry, a Surveillance, Epidemiology, and End Results (SEER) participant, and were incident‐resident cases diagnosed from 1973–1978. For single, primary in situ or invasive cancer, the preferred mastectomy was modified radical. Partial and simple resections were used more often than radical procedures for non‐inflitrating, especially lobular lesions. For cases with bilateral primaries, with at least one in situ, the mastectomy of choice was the simple procedure for the in situ malignancy and modified radical for the invasive cancer. AbstractErythropoietin (Ep)‐dependent erythrocytosis was discovered in a 77‐year old man with angiosarcoma involving the liver and spleen. At autopsy, tissue from tumorous and normal sections of liver was obtained and extracts prepared for Ep assay in exhypoxic polycythemic mice. Neither tumor nor normal extracts had Ep activity; however, when the extracts were incubated with normal plasma prior to assay, significant amounts of Ep were generated. The normal liver tissue had three times the activity of the tumorous portion, 0.21 U/ml. These results suggest that the tissue contained an Ep activator, probably erythrogenin. It is proposed that the greater amount of erythrogenin activity in the normal tissue may reflect an attempt at liver regeneration following damage by an invading tumor, as regenerating liver has been identified as a stimulus for extrarenal Ep and erythrogenin product

AbstractCarcinoembryonic antigen (CEA) test was performed at the time of diagnosis in 136 patients with early operable breast carcinoma (stage I = 65, stage II = 71). Only 4% had CEA levels greater than 5 ng/ml. Patients with axillary metastasis (stage II) had higher incidence of abnormal CEAs than those with disease localized to the breast (stage I). Follow‐up CEA tests were performed on 243 patients after mastectomy. In this group, 50 patients developed recurrent disease and 48% had elevated CEA levels. Patients with chest‐wall recurrences rarely had abnormal CEAs, while those with visceral metastasis had higher levels. Among the 193 patients known to be without recurrence, 10% also had elevated CEAs. AbstractThe Rye histologic classification of Hodgkin's disease has been applied retrospectively to a series of 78 previously untreated cases of Hodgkin's disease. The patients have been followed for periods varying from 1 to 12 years. Mixed cellularity was the most common histologic type. Nodular sclerosis had an intermediate frequency, and lymphocytic predominance and lymphocytic depletion showed the lowest incidence. The sex incidence showed a male predominance in all histologic types except the nodular sclerosis, where prevalence of females was noted. The study confirms the better prognosis of the lymphocytic predominance and nodular sclerosis in comparison with the poor prognosis of the mixed cellularity and the very poor prognosis of the lymphocytic depletion. Nodular sclerosis presented with higher incidence in the mediastinum; it occurred mainly in young females and it exhibited a relatively good prognosis, findings which support the accepted view that nodular sclerosis comprises a distinct clinical and biological type of the disease. A marked difference in the survival rates in Stages I and II as compared to those of Stages III and IV was found.
