AbstractMice bearing transplanted glioma received 0.9% NaCl, 0.1 mg of BLM, or 200–250μCi of111In‐BLM (0.1 mg BLM) daily for 5 days intraperitoneally. After therapy, tumor sizes were in the order NaCl>BLM>111In‐BLM. On the 11th day after the first injection, tumor size (mm3) in the111In‐BLM group was 1,220; in the BLM group, it was 2,310 (P<.025). After intratumor injection of a total dose of 0.1 mg of BLM/gm tumor weight, or of 1 mCi/gm tumor weight of111In‐BLM (carried by 0.1 mg of BLM/gm tumor weight), the tumor size decreased in the111In‐BLM group more than in the BLM group. On the 5th day after the 2nd dose therapy, the tumor size in the111In‐BLM group was 2,020; in the BLM group it was 4,220 (P<.05). Host weights for these two groups were similar. The necrotic area in the tumor was much greater in the111In‐BLM group than in the BLM group. These results suggest the use for radiotherapy

AbstractWe investigated the occurrence of thyroid and parathyroid disorders in 100 women (age 66–70 years) irradiated for cervical spondylosis on average 25 years previously and in 100 control women of similar age. Hyperparathyroidism (HPT), proven by operation, was diagnosed in one patient of each group, and three additional cases were diagnosed biochemically among irradiated women. The difference in incidence is not significant. Nor was there any significant difference in incidence of thyroid disorders. No thyroid carcinoma was found in either group. AbstractIn a test of electromagnetic induction hyperthermia to deep viscera of a live dog model, we found that heating was not uniform to any depth, but was quite variable. In general, there was a thermal gradient between peripheral and central portions of the transposed spleen of about 1°C. Though heat generation within the abdomen was not uniform, its temperature pattern in the alive animal resulted in significant heating of that part of the organ that had been surgically placed at the center of the animal. This heating could not be explained by perfusion with regionally heated core blood. Our results indicate that extensive investigations in living systems and complex dynamic phantoms will be necessary before individual patient response can be predicted

AbstractLocal infiltration of adjacent anatomic structures and soft tissues of the neck from well‐differentiated carcinoma of the thyroid gland is a relatively infrequent occurrence. We report our experience with 21 such cases seen in our department over the past 20 years. All patients were treated by total thryoidectomy and total or partial excision of the infiltrated adjacent structures. Papillary carcinoma was the most frequent type of primary tumor seen. Following the definitive surgery, all patients were scanned with radioactive iodine (131I). In case of isotope entrapment, a curative dose131I was given. All patients in our series were required to receive thyroid hormone replacement. Four patients died as a result of their disease. Uncontrolled local disease and distant metastases were present at the time of death. Three patients died of unrelated causes. AbstractThirty‐eight patients with adrenal adenocarcinoma were reviewed. Two of 23 males and 8 of 15 females had a functioning tumor (P<0.005). The most striking finding in this review was the variation in survival according to histologic grade. The median survival from histologic diagnosis was for grade I tumors 54 months, for grade II tumors 26 months, for grade III tumors 1 month. The difference in survival between patients with grade I or II tumors and those with grade III tumors was significant (P<0.001). The majority of the patients with grade III tumors were admitted with clinical evidence of disseminated disease and died within 1 month. Among the patients with well‐differentiated tumors, five who received surgical treatment of their recurrence had a median survival of 51 months, whereas seven patients who received no surgical treatment had median survival of 20 months following first recurrence (0.05

AbstractAdjuvant immunotherapy has the theoretical attraction of augmenting the host immune response at a time when the tumor burden is low. We have previously reported that intralymphatic immunotherapy (ILI) augments the cytolytic humoral immune responses in melanoma patients. This study was undertaken to assess the effects of ILI on cell‐mediated immunity. As a model for the cellular effects of ILI, we investigated the natural killer (NK) cell activity in malignant melanoma patients. Fourteen patients were given an allogeneic cultured tumor cell vaccine (TCV) intralymphatically with concomitant administration of BCG. Natural killer cell activity was assessed sequentially using the single cell lysis and binding assay. Overall NK activity against the K562 target cell line showed a moderate increase over pretreatment levels as reflected by the increased number of target binding lymphocytes. However, the percentage of target binders mediating cell lysis remained unchanged during treatment. Assessment of NK activity against the NK‐resistant M14 melanoma cell line reflected similar findings. These results suggest the activation of NK cells by tumor antigens, BCG, and/or alloimmunization with TCV. AbstractFor some small rectal cancers electrofulguration can be an attractive alternative to more extensive surgical procedures. This report is a review of 49 patients who, after careful selection, were considered ideal candidates for curative fulguration in the period 1959–1982. All had rectal adenocarcinomas and were clinically staged as Dukes' A tumors. To put the results of this analysis into perspective, they are compared to the results of those patients (34) who also had early clinical stage rectal adenocarcinomas, but for a variety of reasons underwent abdominoperineal excisions (APE, 11 patients) or low‐anterior resections (LAR, 23 patients) instead of fulguration. Postoperative complications after fulguration were minimal, there were no postoperative deaths, and all patients retained anal continence. After APE or LAR there were two postoperative deaths (one myocardial infarction, one ruptured iliac aneurysm); postoperative morbidity was greater. All LAR‐treated patients remained continent. Of the patients at risk 55% remained disease‐free after fulguration, while 77% remained disease‐free after APE or LAR (P = 0.023). This is due to a higher percentage of loco‐regional recurrences occurring after fulguration than after APE or LAR (31% vs 9%, respectively, P = 0.021). The percentage of patients with distant metastases in both groups are similar. It seems that electrofulguration of rectal cancers, even in strictly selected patients in a specialized institution, can lead to an unacceptably high percentage of patients with loco‐regional recurrences. Nearly all local failures occurred in patients with tumors that measured more than 3 cm in diameter and/or occupied more than one‐third of the rectal circumference. Salvage operations for loco‐regional failure were performed in 13 patients, 5 of whom are alive with no evidence of disease. Since there seems to be a direct relation between tumor size and the chance of loco‐regional recurrence and since salvage operations for local failure are not uniformly successful, electrofulguration for cure must be reserved for the very rare patient with a very small early
