AbstractA case of pyomyositis, a deep muscle abscess, presenting as a soft tissue sarcoma is reported. Although common in the tropics, this disease is quite rare in the United States. The presentation, diagnosis, and treatment of the problem are discussed and reviewed.

AbstractThis is a retrospective study of 408 patients who had mastectomy for carcinoma of the breast during 1971–1980. Over these 10 years, we have had a significant increase in Hispanic patients. Although the mean size of breast tumors among the Hispanic patients was smaller than that of our black patients, Hispanic patients are more likely to have higher numbers of positive axillary lymph nodes. Similar to the nationwide trend, over 90% of the patients had modified radical mastectomy in recent years, and adjuvant systemic chemotherapy has replaced postoperative radiotherapy for patients with axillary metastasis. Postmastectomy actuarial 5‐year relapse rates of our patients with none or 1–3 positive axillary nodes were quite similar. Among patients with 1–3 positive axillary nodes, year of diagnosis and whether postoperative radiotherapy or chemotherapy was added or not did not affect disease‐free probability. AbstractThe effect of known chemotherapeutic programs in patints with breast cancer who developed metastasis during or after adjuvant chemotherapy with L‐PAM plus 5‐FU (PF) were switched. Thirteen patients failed while receiving adjuvant therapy at 3–22 months from the time therapy started. Three patients failed 6–28 months after 2 year of therapy. Thirteen patients received PF followed by cyclophosphamide, methotrexate, and 5‐fluorouracil (CMF) at the time of progression PF. Of these, ten received Adriamycin and vincristine (AV) at the time of progression on CMF. The response to CMF was 7%. No patint responded to AV after progression on CMF. Two of three patients treated with AV after failure on PF did respond, but both for only 6 responsive to further chemotherapy. AbstractAdenomatous polyps lead directly to carcinoma of the colon in patients with one of the familial syndromes of intestinal polyposis. Elective colectomy is prophylactic and life‐saving. A subgroup of patients will develop periampullary carcinoma, again arising from the presence of adenomatous polyps. Such a case is presented. These polyps should be surgically excised to ensure adequate pathologic examination. Yearly upper endos‐copic examination is an essential element of management. AbstractThe use of plasma albumin (ALB), transferrin (TFN), prealbumin (TBPA), retinol‐binding protein (RBP), triceps skin fold (TSF), and midarm muscle circumference (MAMC) as determinants of response to nutritional therapy (TPN) was investigated in 40 cancer patients during preoperative TPN. Thirty‐one patients received 90% or more of their anabolic caloric requirement (Harris‐Benedict equation) by means of TPN. During this study period (average 11.1 ± 4.7 days) nutritional assessments were completed before TPN and on the last day of TPN before surgery. Average weight loss based on usual body wt (UBW) and ideal body wt (IBW) was 19 ± 11% and 9 ± 15%, respectively (not significant, NS). Weight loss (UBW) correlated with ALB (P<0.001), TBPA (P<0.005) and RBP (P<0.02) but did not correlate with TFN (P<0.06), TSF, and MAMC. Weight loss (IBW) correlated with TSF (P<0.01) and MAMC (P<0.03) but did not correlate with plasma protein (PP). During TPN the average percent increases for PP were 0.1% (ALB, NS), 20% (TFN, NS), 60% (TBPA, P<0.02), and 116% (RBP, P<0.005). AbstractThe histogenic origin of squamous carcinoma of the stomach (SCS) is controversial. We report the case of a 55‐year‐old man who developed SCS following successful chemotherapy for generalized well‐differentiated lymphocytic lymphoma. AbstractPerineural metastasis from squamous cell carcinoma is quite rare. The treatment of such metastasis in the head and neck region is difficult due to base of skull or intracranial involvement by tumor. Surgery and radiotherapy may provide local control, but patients often fail distantly. We present a patient with squamous cell carcinoma presenting with perineural metastasis who was successfully treated with combination chemotherapy and definitive radiation therapy. Suggestions of therapeutic management are made of this uncommon clinical problem.

AbstractRecent experience with 108 Broviac catheters was reviewed. Sixty‐eight were percutaneously placed in the subclavian veins of adults receiving systemic chemotherapy. For subclavian catheters perioperative and late complications were unusual. There was no pneumothorax or serious bleeding associated with insertion, even in severely thrombocytopenic patients. Mean catheter life was 93 days. Infection occurred in only five cases (7%). We conclude that percutaneous placement of right atrial catheters is a safe procedure which provides reliable access for prolonged chemotherapy in cancer patient
