AbstractThis article describes the designated comprehensive cancer center in the Pacific Northwest section of the United States. The local financial, political, and medical problems associated with the initial establishment of this center are discussed. The general philosophy of the approach to cancer, cancer research, and care are included. The programs described differ very little from other similar centers with exceptions that are noted. These are the establishment of the major clinical facilities or units outside of the center proper. The utilization, not duplication, of already existing surgical, irradiation, and hospital facilities representing a departure from the usual custom is discussed. Active participation with real input by the physicians of our region through the extramural council is described.

AbstractCarcinoma of breast radical mastectomy specimens were examined. The tumors were classified in accordance with their morphologic appearance. The tumors were also graded on the basis of their cytological makeup and assigned a cytologic malignancy score (CMS). While the classification of tumors was based on the overall pattern of the tumor, the CMS was arrived at by examining the least differentiated portion of the tumor. The following conclusions were suggested by the study: Patients with tumors with high CMS have a poor prognosis, even in the absence of axillary lymph node metastases. It appears reasonable to suggest that patients without overt metastases, but with high CMS, should be considered for treatment with all therapeutic measures available. Such an aggressive approach to patients with low CMS does not appear to be justified.

AbstractThe in vitro blastogenic response of 76 patients with colon adenocarcinoma and 29 age‐matched normal healthy volunteers was compared. An age‐related decline in cellular immunocompetence was observed in both the patient and control group. Within each age interval, the patients' blastogenic responses were lower than the controls' and the rate of decline of the blastogenic response in the patient group was greater in magnitude, suggesting that their disease state also contributed to loss of cellular immunocompetence. Thus, both increasing age and presence of tumor are factors that contribute to the decline in immunocompetence in patients with colon adenocarcin

AbstractComplete regression of Walker 256 solid rat tumor was brought about by the subcutaneous implantation of a single sustained‐release cyclophosphamide capsule adjacent to the tumor mass. Untreated control animals died within 3 weeks after receiving the tumor with large necrotic masses. Rats with sustained‐release capsules experienced complete tumor regression within 28 days. Upon examination, the tissues around the previous tumor mass had regenerated, and no viable tumor cells could be found in the area where the previous tumor mass was. AbstractA comparative study of the inhibitory effect of cell‐free supernatants (CFS) from human lung cancer cell lines (ChaGo and PC‐1) and from a human embryo fibroblast cell line on lymphocyte blastogenesis indicates that both lung cancer cell lines produce a large amount of the inhibitory factor while the production of such a factor from a control noncancer cell line is essentially very minimal or absent. The inhibitory effect of CFS of the lung cancer cell lines is not due to direct cytotoxicity. The inhibitory effect is found to be partially sensitive to heat treatment. AbstractAn antiserum was developed in rabbits by injections of a virus concentrate from Moloney murine sarcomas. Passive transfer of this antiserum attenuated the growth of both primary and transplanted Moloney murine sarcomas. It also facilitated tumor regression and increased survival times of young mice with primary Moloney sarcomas.

AbstractPlasma carcinoembryonic antigen (CEA) assay was done in 30 patients with osteosarcoma. CEA was found positive (>2.5 ng/ml) in 17 of 21 patients who had active evidence of disease and negative (<2.5 ng/ml) in all 9 patients who were in complete remission resulting from previous amputation or chemotherapy. Serial CEA determinations demonstrated a fall to normal in 7 of 9 patients following successful surgery or chemotherapy and a rise and fall (fluctuation) of levels in 8 patients who had tumor progression while on chemotherapy. Clinical recurrence of disease in two instances preceded or coincided with CEA elevation. The CEA assay in osteosarcoma although nonspecific could be used as an inportant adjunct to experienced clinical judgment, periodic x‐ray examination, and laboratory study to prognosticate the course of osteosarcoma during therapy. AbstractA review of all patients with endocervical adenocarcinoma seen at the University of Kentucky Medical Center from 1962 through 1972 yielded 15 patients with tissue‐verifiable primary endocervical adenocarcinoma that had no associated sarcomatous or squamoid features. These patients varied from 17 to 73 years old with a mean age of 47 years. A malignant clear cell pattern was present in 8 patients and in 3 of these individuals that prominent morphologic feature was associated with hobnail cells. Five patients with clear cell carcinomatous changes in the endocervix were born prior to the use of diethylstilbestrol (DES) and 2 of the remaining 3 patients were exposed to DES during the first trimester of pregnancy. Other morphologic types of endocervical adenocarcinoma included mucinous adenocarcinoma in 9 patients (60%) and basaloid adenocarcinoma in one patient (7%). Cellular atypia of endocervical glands, which in some areas was consistent with adenocarcinoma in situ was found in 9 of the 15 patients with invasive endocervical cancers. AbstractA preclinical murine renal tumor model system is available for evaluation of the effectiveness of therapeutic agents. Clinical review reveals that objective response rates to hormonal agents reported since 1971 have been substantially lower than those reported prior to 1971. Many chemotherapeutic agents have not been adequately evaluated clinically or with the experimental model. Vinblastine to date may be the most active single agent with a 25% objective response rate. At the present time, it is uncertain whether combination chemotherapy or hormonal chemotherapy add to response rates obtained with single agents. There is some evidence that immunotherapy may be of some adjuvant benefit and, thus, may provide an additional avenue of investigation.

AbstractBlood group distribution in 264 Stage D relapsing patients from the National Prostatic Cancer Project was analyzed. No significant differences were found in distribution of blood groups in prostatic cancer patients as compared to the control population. This study does not support the observation of others of a relationship between blood group A and susceptibility to carcinoma of the prostate. Other patient groups may, however, be of value in searching for this relationship.
