AbstractA case of limited success with streptozotocin in overwhelming metastatic insulinoma is reported for illustration of clinical management and possible toxic effects of the drug. An initial dose of 1.5 g/m2and an interval of administration no more frequent than 1–2 weeks is favored. Acute renal failure and bone marrow suppression followed the last and largest intravenous dose of 8 g/m2. Insulin immunoassay levels and glucose or caloric requirements were reduced, but the patient did not survive. AbstractHyperlipemia developing in tumor‐bearing animals concomitant with depletion of carcass lipid prompted study of human cancers. Gross lipemia when liver function is normal is infrequent in human cancer. Mean serum triglycerides and unesterified fatty acids (UFA) are higher than in normal individuals, but no greater than in patients undergoing severe fat depletion from causes other than cancer. Total plasma fatty acids and cholesterol concentrations in human cancer are normal. Anin vivorat assay for fat‐mobilizing substances in the urine and plasma of cancer patients was negative. The mobilization of carcass lipid in human cancer appears to be in response to an energy deficit created by actively synthesizing tumor tissue. There is no significant difference between nonneoplastic surgery and resective surgery for cancer in regard to mobilization of fat for energy substrates. AbstractPre‐ and post‐therapy small bowel biopsies were obtained in 19 of 20 patients who received 5‐fluorouracil for treatment of their malignant disease. Three parameters were evaluated: clinical response to therapy, which occurred in five cases (26%); development of gastrointestinal toxicity, which occurred in 13 (68%); and histopathologic change of small bowel mucosa, which was noted in 13 (68%). There was no correlation between clinical response to therapy and the development of toxicity, nor was there any between toxicity and histopathologic change in small bowel mucosa. Definite histopathologic change in each of the patients who responded to therapy was the only consistent relationship noted, although the appearance of such changes did not necessarily indicate that tumor response could be exp

AbstractThe stilbestrol‐induced hamster kidney tumor retained hormone dependencein vivo. In order to elucidate the mode and site of hormone stimulation, tumors of the twelfthin vivotransplant generation as well as newly induced kidney tumors were explained to monolayer cell cultures. Various concentrations of diethylstilbestrol failed to stimulate cell growth or prolong viability, and cells died after two to eight subcultures. This was not secondary toin vitroselection of a hormone‐independent cell population, since hormone was required for growth of tumor recycled from cell culture to animals. AbstractSyngeneic thymic grafts in AKR mice were exposedin vitroto lymphomagenic virus, grafted subcutaneously or under the kidney capsule, and biopsied at intervals. No specific morphologic change was seen to precede the development of lymphoma in these grafts. Lymphoma was found to develop initially as foci of lymphoblasts in the cortex of a normal‐appearing thymus. Regeneration of syngeneic virus‐exposed thymic epithelial remnant grafts in AKR mice was compared to that of nonvirus‐exposed grafts in the same strain and to that in a low‐incidence lymphoma strain. All of the grafts were found to regenerate to the morphology of normal thymus with a characteristic sequence of cellular events. These results imply that lymphomagenic virus acts by a direct transformation of cells in the thymic cortex which is not dependent on any preceding structural alteration of this organ and that regeneration of grafts composed of epithelial reticular cells was not impaired or altered by the presence of th

AbstractThe effect of phytohemagglutinin‐P (PHA) onin vivoimmune responses was assessed in guinea pigs. PHA was administered daily, commencing 2 days prior to antigen. This regimen depressed the humoral antibody response to bovine gamma globulin (BGG) (p<0.005) and abolished the delayed cutaneous hypersensitivity (DCH) response to tuberculin (BCG) (p<0.005) after primary immunization with BGG and BCG. In guinea pigs immunized to BCG, PHA inhibited the DCH response to secondary challenge with the antigen. AbstractA successful right hepatic lobectomy for a large solitary liver metastasis of a leiomyosarcoma was performed 8 years after removal of the primary lesion in the right colon. An anomalous arterial supply to the medial segment of the left hepatic lobe was encountered during the dissection. Clues to the presence of the anomalous blood supply and the rationale and results of hepatectomy for metastatic neoplasms are discussed briefly. An aggressive approach to surgical therapy is advocated in selected cases.

AbstractWhen case follow‐up continues for 10 years or more after resection for localized bronchogenic carcinoma, the declining curve of relative survival rates levels out on a zero slope, indicating a normal life expectancy for long‐term survivors. This is the cure index. Among patients with tumors under 6 cm diameter, the smaller the tumor the greater the cure index. For tumors no larger then 2 cm, the cure index is about 80 percent; for tumors 2.5–4 cm, the cure index is better than for tumors 4.5–6 cm. Diligence in use of current methods to find and resect smaller lung carcinomas can be worthwhile. AbstractMast cells are frequently seen in and around a variety of malignant tumors. However, mast cells have not been seen in the transplantable estrogen‐induced renal tumor of the hamster. In this study the subcutaneously transplanted renal tumor of Kirkman was examined using a variety of fixation techniques. All tissues were stained in toluidine blue (0.5% solution at apK of 3.8). Mast cells were seen in great numbers only in tumor tissue frozen in isopentane cooled with liquid nitrogen at −155 C and cut on a cryostat at −20C. AbstractAn attempt was made to create an active antilymphocytic immunity in dogs. Pooled niesenteric canine lymph nodes were harvested, processed, and coupled viabis‐diazobenzidine to heterologous (rabbit, horse, cow, and human) gamma‐globulin. Mongrel dogs were immunized by serial injection of this material. There was a decrease in the absolute lymphocyte count in animals receiving lymph node tissue coupled with rabbit, horse, and bovine gamma‐globulin; however, this decrease was insignificant statistically (0.2>P>0.1). Twenty‐eight to thirty days following initiation of the immunization course, kidneys were exchanged in paired animals without any attempt at immunosuppression except for continuation of the immunization regimen. The average survival of the transplanted kidney and of the recipients was not significantly different from that of a control group of animals. Although there was no strong evidence of an active antilymphocytic immunity in the present study, modification of such a technique might prove effective as a means of prevention of homograft rejection or in creating autoimmunity to neoplasms of the leukocytes or solid
