AbstractLocal recurrence of rectal cancer following abdominoperineal resection is rarely amenable to limited resection. Computed tomographic (CT) study of the pelvis revealed recurrent tumor images in relation to the adjacent pelvic structures. Five patients with deeply invading recurrent lesions were submitted to pelvic exenteration combined with sacral resection. No postoperative deaths were encountered. The postoperative sharp drops of serum CEA levels are useful for judging the adequacy of this procedure. The follow‐up status of the 5 patients, respectively: 14 months alive with pulmonary metastasis, 9 months alive without symptoms but with slightly elevated CEA levels, and 8 months, 3 months, and 3 weeks alive with no evidence of disease. All patients who were relieved of perineal or sciatic nerve pain are now able to walk without any assistance or trouble. AbstractThe estrogen receptor (ER) profile of patients with breast cancer metastatic to bone marrow (BM) has not been widely reported. The charts of all patients having a diagnosis of breast cancer and undergoing bone marrow aspiration or biopsy at the Cleveland Clinic during the period of January 1980 through September 1982 were reviewed. Thirty‐nine patients were so identified; of these 39 patients, 28 had estrogen receptor determination performed on a primary or a metastatic tumor specimen. Of the 28 patients with known ER, ten (36%) had ER less than 5 fmoles/mg cytosol protein, three (11%) had ER or 5 to 10 fmoles/mg, and 15 (54%) had ER greater than 10 fmoles/mg. Of the 39 patients with BM involvement, 36 (92%) had cortical bone involvement documented on x‐ray or isotopic bone scan. Liver involvement was documented in 6/34 (18%) patients, pulmonary involvement in 14/37 (38%) patients, CNS relapses in 3/39 (8%), and locoregional recurrences in 19/39 (49%). The most significant hematologic finding was a hemoglobin of less than 12 gm% in 21/37 (57%). The most frequent biochemical abnormality was an elevation of the alkaline phosphatase in 30/39 (77%). AbstractThe selective affinity of hematoporphyrin derivative (HpD) for dimethyl‐hydrazine‐induced colorectal malignancies in mice was evaluated both visually and by a quantitative microfluorescent photometric technique. A significant corelation was found between higher concentrations of HpD in colon biopsies determined by microfluorescence spectroscopy and the presence of carcinoma cells in these biopsies as identified by histologic examination. AbstractTwenty‐two inflammatory, benign, or malignant parotid lesions were studied by means of the specific red cell adherence test (SRCA), a modification of the Coombs' mixed cell agglutination reaction. In 22 normal parotid tissues the acinar structures were devoid of red cell agglutination, but it was present in ductal epithelium. Findings were similar in 2 cases of parotitis, 11 benign mixed tumors, and 1 malignant mixed tumor. All lacked red cell agglutination in areas of neoplastic change. Benign Warthin's tumors (4 cases) demonstrated antigenicity in the co‐lumnar epithelial component of the tumor, but lacked red cell agglutination in areas of the lymphoid component. One malignant Warthin's tumor showed agglutination in areas of normal columnar epithelium but not in areas of malignant dedifferentiation. Undifferentiated carcinoma (1 case) and adenoid cystic carcinoma (2 cases) did not possess detectable ABO (H) antigens in neoplastic areas of the gland. The absence of ABO antigens in normal acinar glands supports their suggested myoepithelial or mesenchymal derivation, as the absence of antigen in benign and malignant mixed tumors supports their proposed mesenchymal derivation. Ductular epithelium and the epithelial components of benign Warthin's tumors have ABO (H) antigens, while the loss of antigen in the epithelial portion of the malignant Warthin's tumor is characteristic of epithelial neoplastic dedifferentiation. AbstractLeiomyosarcoma is a malignant tumor which rarely occurs in the rectum. Three patients with leiomyosarcoma of the rectum are presented. Treatment should include wide surgical excision followed by adjuvant chemotherapy. Emphasis is placed upon computerized tomography as the method of choice for the diagnosis of recurrent disease. Recurrent tumor should be resected whenever possible.

AbstractThe distribution of Coomassie blue‐stained proteins from uninvolved regions of 4 human livers, from 1 hepatocellular carcinoma, and from 4 samples each of uninvolved colon, primary adenocarcinoma of the colon, and colon cancer metastatic to the liver was analyzed by two‐dimensional protein electrophoresis. From a comparison of acidic proteins between pI 3.5 and 6.5, we conclude (1) that the majority (66 of 82) of denoted acidic proteins from 4 normal liver samples were represented in the hepatocellular carcinoma. Fifty‐one of 58 proteins denoted in the 4 colon samples were detected in each of the 4 primary colon cancers; (2) that the “normograms” of proteins from normal colon and normal liver differed in many details, and their dissimilar patterns identified the source of the sample; (3) that hepatoma and primary adenocarcinoma of the colon were easily distinguished by their distribution of proteins; (4) that colon cancer metastatic to the liver contained a majority (50/58) of acidic proteins enumerated in primary colon cancer. AbstractA retrospective analysis of 42 patients with stage III, IV, or recurrent epithelial ovarian carcinoma treated with monthly cisplatin, Adriamycin, and cyclophosphamide (PAC) was made. Of 36 patients with measurable disease, 18 (50%) achieved a clinical complete response (CR) and 12 (33%) achieved a partial response (PR) for an objective response rate of 83%. Six stage III patients remained without measurable disease after surgery and postoperative PAC and are included in the survival data. The median survival of all patients was 22 months (10 months for nonresponders), with a median duration of response of 15 months (19+ months for clinical complete responders). Of 16 patients who underwent second‐look laparotomy while in clinical CR, 8 were pathologically free of disease. Of these 8 surgically staged CRs, 2 have suffered CNS relapses, while the rest remain free of disease. One additional patient, who had been found to have only microscopic disease at the time of second‐look surgery, subsequently relapsed in the CNS, for a total of 3 patients with CNS relapse. AbstractThoracic cage tumors are infrequently encountered and may present a dilemma in diagnosis and treatment. A case of hepatocellular carcinoma metastatic to rib, an unusal chest wall tumor, is presented here, and an orderly and effective approach to chest wall lesions is described. Computerized tomography and biopsy are the most efficacious diagnostic modalities.

AbstractWe report the case of a primary malignant melanoma arising from the mucosa of the hard palate on a fibrovascular stalk. This is to our knowledge the first report of a pedunculated melanoma in this location. The possibility of a more favorable prognosis, because of the stalk, which shows no signs of invasion by the neoplasm, is considered.
