AbstractNeuroblastoma is a common solid tumor of childhood. Maturation to a ganglioneuroma or regression has been reported to occur spontaneously or following minimal treatment. Malignant degeneration of such a ganglioneuroma is extremely rare. We present a report of a malignant tumor arising in a ganglioneuroma 19 years following abdominal radiation therapy for neuroblastoma.

AbstractExperimental evidence suggests that regional lymph nodes (RLN) are important in the initiation and possibly the maintenance of tumor immunity. “Negative” nodes denote strong tumor immunity and “positive” nodes low. The latter also serve as markers of systemic disease. From histological and immunological studies, and mostly from recent clinical studies in breast cancer, the following practical recommendations are made: (1) Clinically positive axillary nodes are best eliminated by surgery. (2) Resection of positive internal mammary nodes appears to increase survival of patients with central and inner quadrant lesions; however, destruction of these nodes by irradiation, although improving local disease control, may decrease survival. (3) Negative RLN should be preserved, as they appear to prevent lymph node metastases and stimulate systemic immunity. Only a small fraction of unresected RLN harboring micrometastases will ultimately develop palpable disease, and their elimination at that late phase yields the same results as when these nodes are treated prophylac

Abstract566 stage‐II melanoma patients treated at the National Cancer Institute of Milan, Italy, were analyzed to evaluate the prognosis. Among the criteria considered, four were significantly associated with survival when considered as single factors: growth pattern, levels of invasion, the number of involved lymph nodes, and the extent of metastatic growth. As regards growth pattern, the observed 5‐year survival rates were 41.9% for superficial spreading melanoma and 20.5% for nodular melanoma (P= 10−3). As regards levels, the 5‐year survival rates were as follows: level II, 20.9%; level III, 33.1%; level IV, 43.2%; level V, 10.2% (P= 10−3). Patients with a partial node metastasis had 64.5% 10‐year survival, while those with extension beyond the capsule had 32.6% 10‐year survival (P= 10−9). Patients with one metastatic node had 43.4% 10‐year survival, and patients with three or more positive nodes had 26.0% 10‐year survival (P= 10−9). AbstractA patient with unexplained low back pain, weight loss, and pulmonary infiltrate underwent an inguinal herniorrhapy. Histologic examination of the grossly normal hernia sac revealed metastatic adenocarcinoma. The patient died of cachexia 10 months later. This case observation suggests the necessity of histologic examination of grossly normal hernia sacs.

AbstractThis study is a review of 39 patients with testicular seminoma, Stage I, treated at the Department of Radiation Oncology, James Graham Brown Cancer Center from 1959 to 1978. The age of the patients ranged from 16 to 70 years with a median of 37. Thirty‐two (82%) patients presented with swelling or mass in the testis, four patients with pain, and three patients had seminoma diagnosed incidentally. Twenty (51%) patients had the tumor on the right side and 19 (49%) patients had the tumor on the left side. All patients received irradiation to the ipsilateral inguinal, iliac, and bilateral para‐aortic nodes with “hockey stick” type fields. The majority of the patients received a midplane dose of 3,200 to 3,600 rad in 3–4 weeks time. None of the patients received prophylactic irradiation to the mediastinum and supraclavicular region. The 5‐year actuarial survival rate is 96%. There is no additional benefit in giving prophylactic irradiation to the mediastinum and supraclavicular regions in Stage I testicular seminoma. AbstractThe role of cryptorchidism in testicular carcinogenesis has been of concern for many physicians for quite a long time. The probability of a malignant neoplasm developing in an undescended testicle is about 20 to 48 times greater than a normally descended testicle. In a review of 74 patients treated with a diagnosis of testicular seminoma at the Department of Radiation Oncology, University of Louisville, Louisville, Kentucky, from 1959 to 1978, six cases were identified with a history of cryptorchidism. A detailed review of these patients along with literature review is done in this paper. The different etiologic factors for tumorigenesis in this group of patients is also discussed. AbstractExperimental evidence indicates that specific sex hormonal imbalance, deficiency, and excess may be causes of tumors or at least contribute in some way to their development. Clinical observations show that the prognoses of patients with various malignancies differ among males and females, and some cancers can be alleviated and partially controlled by altering the accustomed hormonal environment. Although beneficial effects usually are only temporary, there is no doubt that some cancers are hormone‐dependent to a degree. A significant number of prostatic carcinoma in males and breast carcinoma in both sexes have been treated with various additive or ablative endocrine manipulations. The detection and quantitation of specific steroid binding proteins in hormone‐sensitive tumors have enhanced our understanding of the mechanism of endocrinal therapy. Excluding carcinoma of the breast and of the sex organs (ovary and uterus in females, testis and prostate in males), many other solid tumors have been tested for the presence of estrogen and other steroid receptors. A fair number of solid cancers contains estrogen and progesterone receptors (ER, PR), even those from male patients. Thus, the better prognosis of females with sarcoma, melanoma, liver, colorectal and other cancers cannot simply be explained by the presence or absence of estrogen or progesterone receptors. AbstractTwo unusual presentations of perforated mucoid adenocarcinoma of the appendix are described. One patient presented with a large periappendicular abscess that extended into the gluteal region, the other with a retroperitoneal abscess and abscesses in the right flank and groin. Recognition of the source of the abscess may be difficult but is essential for the prevention and elimination of the sepsis which may be life‐threatening. AbstractThirty‐one patients with neurogenic sarcomas treated at Roswell Park Memorial Institute (RPMI) during a 10‐year period were studied. The mean follow‐up is 35.5 months. A specific nerve of origin could not be identified in 61.3% of patients. The most frequent site was the proximal lower extremity (38.7%). The only presenting symptom was enlarging mass in 67.7%. There was association with Von Recklinghausen's disease in 42% of the cases. Survival was significantly worse in tumors with Von Recklinghausen's disease (25.6%) compared to patients with solitary malignant schwannomas (50.9%). Twelve of 18 patients who had adequate surgical treatment initially remain disease‐free, whereas only 2 of 11 patients referred following partial excision or recurrence remain disease‐free (P<0.02). Eleven of 18 patients with grade I or II tumor are disease‐free, whereas 2 of 10 patients with grade III tumor are diseasefree (P<0.05). AbstractA case is presented of prolonged survival following multiple thoracotomies performed for metastatic adrenal cortical carcinoma that was refractory to other treatment modalities. In selected patients with isolated metastases to the lungs, repeated surgical resection may offer the only possibility for cure or extended palliation.
