AbstractTAG72 is a well‐characterized, human tumor‐associated antigen present in>85% of human colonic cancers. In this study, we established an animal model of hepatic metastases of human colonic carcinoma. The high‐mucin variant cell line, designated HM7, was derived from the human colonic carcinoma cell line LS174T. Following intrasplenic injection, HM7 was able to induce much greater hepatic metastases in SCID mice compared to its parental cell line LS174T. Numerous hepatic metastases were evident 18 days subsequent to the intrasplenic injection of tumor cells. Using the chimeric anti‐TAG72 antibody ccM4, immunohistochemistry demonstrated strong expressions of the TAG72 antigen in these metastases. Our biodistribution and imaging data also showed that the radiolabelled ccM4 antibody was able to localize hepatic metastases in the SCID mice. Based upon these findings, w anticipate that the herein described SCID mouse model will prove most useful in studying hepatic metastases of human colonic carcinoma by using anti‐TAG72 therapeutic immun

AbstractBetween 1971 and 1992, 42 children (median age = 2 years) with histologically proven Wilms' tumor were evaluated at the Northern Israel Oncology Institute. Most patients were staged according to National Wilms' Tumor Study (NWTS) criteria by which 18 were clinical stage I‐II and 24 were III‐IV. Combined therapy (surgery, chemotherapy, and radiotherapy) was given with a change to lower dose radiotherapy (<2,000cG) after 1981. The pre‐1981 group (13 patients) suffered six relapses, most of which were pulmonary, whereas the 1981‐1992 group (29 patients) had three relapses. The actuarial 5‐year survival rate using Kaplan‐Meier tables was 79% for the entire group but 100% for the 1981‐1992 group and 38% for the 1971‐1980 group. One case of immediate hepatic toxicity and one sudden death, unknown cause, of a patient in remission occurred in the group. Late deleterious effects were seen in five other patients; four had scoliosis and one developed osteoid osteoma of a rib. AbstractDNA nuclear content and PCNA index (proportion of PCNA reactive cells) have been studied by flow cytometry in eight pancreatic lesions producing hyperinsulinemic hypoglycemia to assess DNA ploidy and tumoral growth fraction. Adult nesidioblastosis had a diploid DNA index, two adenomas were near diploid, and four of the adenomas and the carcinoma were aneuploid. The median value of PCNA index (6.056% ± 6.76) was significantly correlated (p<0.05) with the mean tumor diameter (2.43 cm ± 1.96). Tumors with a PCNA index<6.056% showed a diameter less than the mean tumoral diameter and a benign clinical course. Tumors with PCNA index<6.056% generally displayed a diameter bigger than the mean tumoral diameter, being associated with lymph node metastases in one case. AbstractApproximately 80% of breast biopsies are performed for what proves to be a benign process. The patients who undergo these procedures should continue screening with breast physical examination and mammography. The long‐term impact of breast biopsy on these screening modalities has not been well studied. We performed a prospective, follow‐up evaluation in 63 patients who underwent needle localization biopsy with benign histology at our institution between 6 and 7 years ago. This evaluation consisted of a directed history, breast physical examination, and follow‐up mammogram. Two patients (3%) had undergone mastectomy for an interval breast cancer; 17 others (28%) had undergone subsequent biopsies. No patient had changes on physical examination of the biopsy site. All mammograms were evaluated as normal or as having benign abnormalities. AbstractNeuroblastoma, a tumor of the sympathetic nervous system, is one of the most common solid malignancies in infants and represents 7% of all cases of childhood cancer outside of the central nervous system. Thirty‐five samples of neuroblastoma from 31 patients were obtained from Duke University Medical Center between 1979 and 1991 and studied to determine the relative prognostic value of a number of clinical, histologic, nuclear, and oncogenic features. The features studied were: stage, Shimada classification, DNA ploidy, MIB‐1‐proliferation index and status for HER‐2/neu, p53 and epidermal growth factor receptor (EGFr). Only age (P= .03), HER‐2/neu (P= .01), and p53 (P= .02) reached statistical significance as prognostic indicators. The median survival for patients with no HER‐2/neu expression was 12 months; median survival for patients with no HER‐2/neu expression was 138 months. Similary, the median survival for patients with p53 expression was 12 months; patients with no p53 expression had a median survival was 144 months. AbstractThe clinical and pathologic characteristics of 100 consecutive men with clinically localized prostate cancer and staged by radical prostatectomy were reviewed. Men with impalpable prostate‐specific antigen (PSA) detected cancers (T1c) were contrasted against men with palpable disease (TB). Lesions were clinically staged as T1c in 53 men and TB in 47 men. Mean serum PSA for men with T1c cancers was 11.8 ± 0.7 ng/dL (normal: 0>4) and 14.1 ± 1.7 ng/dL for men with TB disease. Histologic evaluation revealed a mean Gleason's sum of 6.4 ± 0.2 (scale: 2>10) for T1c lesions and 6.6 ± 0.2 for men with TB cancers (P= NS). DNA content was diploid in 67% of T1c cancers and 62% of TB lesions (P= NS). An overall 43% clinical staging error was observed. Extraprostatic extension was demonstrated in 36% of T1c cancers and 51% TB lesions (P= NS). PSA‐detected cancers were indistinguishable from palpable lesions by all measures evaluated. Prostate cancers detected in asymptomatic men by an elevated PSA should be considered biologically significant

AbstractErythrocyte fatty acids were determined in patients with gall stones and carcinoma of the gallbladder. Significantly low levels of myristic acid (p<0.01) and stearic acid (p<0.001) and significantly high levels of palmitoleic acid (p<0.05) and oleic acid (p<0.01) were observed in cancer patients. Arachidonic and linolenic acid were significantly high in gallstone patients, but there was no significant difference in the lauric acid and palmitic acid levels. Thus an altered lipid metabolism in cancer patients suggest existence of a possible association between gallstones, fatty acids, and carcinoma of the gallbladder.

AbstractImmunotherapy (IT) has become an accepted therapeutic modality for a limited number of tumor types. One of the limiting factors in the use of interleukin‐2 (IL‐2) has been dose‐related toxicity. We undertook these studies to study the effects of combined therapy on a murine melanoma. The B 16 melanoma was implanted in the right hindlimb of C57BL/6 mice and therapy begun on day 3 (microscopic tumor model) or day 10 (macroscopic tumor model). Animals were divided into four groups: No therapy, local hyperthermia (HT) alone (45°C ± 15 minutes on days 3 and 6 or days 10 and 13), HT + IL‐2 at 300,000 IU ip tid, and HT + IL‐2 at 600,000 IU ip tid. We have shown in multiple previous experiments that IL‐2 alone at these doses has no effect on tumor growth; these groups were omitted. In the microscopic model, tumors in the no treatment group were an average of 400 mm2. Animals treated with HT alone had a mean tumor size of 300 mm2. However, tumors in animals receiving both therapeutic modalities measured a mean of 100 mm2(300,000 IU IL‐2 ip tid) and 80 mm2(600,000 IU IL‐2 ip tid). In the macroscopic tumor model, tumors in animals receiving no treatment were an average of 7.5 times larger than on day 10, in animals receiving HT alone were an average of 5 times larger, animals receiving IL‐2 were 2.95 times larger (both dose levels). These results show that combined IT + HT therapy resulted in significantly (p<.05) reduced growth with both microscopic and macroscopic tumors compared to HT alone or no therapy in a murine subcutaneous melanoma model using doses significantly lower than those usually needed to observe a therapeutic response with IL‐2 used alone. AbstractIntrahepatic cholangiocarcinoma is a rare disease. The prognostic factors of resectable cholangiocarcinoma have not been previously reported. Nineteen patients (10M, 9F) with resectable cholangiocarcinoma were investigated to find factors that might influence prognosis. The cumulative survival rate was measured using the Kaplan‐Meier method. The log‐rank test was used to compare two survivals. Age, sex, tumor size, and positive tissue carcinoembryonic antigen were factors that had no influence on prognosis. Patients with a positive HLA‐DR, a well‐differentiated carcinoma, and a clear resective margin had better prognoses but were statistically undifferentiatable from the other patients. Patients with positive hilar lymph nodes had poor prognoses (p<0.01), whereas patients with positive mucobilia had good prognoses (p<0.05). Positive staining for HLA‐DR on tumor cells was observed in nine of 19 cases of intrahepatic cholangiocarcinoma. The positive HLA‐DR staining correlated with a better prognosis, but no significant difference was noted between the positive and negative HLA‐DR staining group. A positive hilar lymph node was a grave sign, as almost all patients of positive lymph node died within 9 months after operation.
