AbstractPeritoneovenous shunting for the treatment of malignant ascites has become increasingly popular. This technique can be complicated by tumor embolization, congestive heart failure, and disseminated intravascular coagulation. Arterial thromboembolism has been encountered in two patients following LeVeen shunt insertion. Recurrent bilateral femoral artery thromboemboli and a cerebrovascular accident occurred in one patient and cerebrovascular thromboembolism developed in a second patient. Major arterial embolization is potentially a serious, although infrequent, complication of peritoneovenous shunting in patients who have malignant ascites.

AbstractHepatocellular carcinoma can be complicated by obstructive jaundice with tumor growing into the extrahepatic bile duct. This complication is an autopsy finding in most reported cases and, rarely, is recognized ante mortem. We report two patients with hepatocellular carcinoma who presented initially with obstructive jaundice. Clotted blood and tumor, which caused bile duct obstruction, was removed operatively and the biliary tract was drained in both patients. We conclude that 1) blood clot and fleshy debris removed from the common bile duct at operation for obstructive jaundice suggests the possibility of hepatocellular carcinoma; 2) the differential diagnosis of jaundice and fever in patients known to have hepatocellular carcinoma should include intrabiliary tumor causing obstruction and cholangitis; and 3) the association of obstructive jaundice and hepatocellular carcinoma may occur more often than previously recognized.

AbstractA 55‐year‐old woman with an ovarian carcinoid presented with intermittent facial and cervical flushing for 10 years, watery diarrhea for 4 years, and abdominal pain without hepatomegaly. Markedly elevated systemic venous and arterial serotonin levels (830 ng/ml; nl = 50‐200 ng/ml) were found. The highest serotonin levels were observed in the superior venal caval system, but serotonin as a marker for tumor localization was inaccurate and led to an unproductive neck exploration. The histological pattern of this tumor contained purely insular elements. No hepatic or nodal metastases were identified and the lesion was unilateral. Substance P levels were elevated in the venous drainage of the left ovary and in retrospect correctly localized the ovarian tumor. This peptide may prove to be another carcinoid tumor marker in addition to serotonin and 5‐hydroxyindolacetic acid. AbstractWe have previously demonstrated that the presence or absence of ABO(H) cell surface antigens correlated with stage, grade, and prognosis of patients with bladder cancer. However, because of the weak antigenecity of 0(H), certain investigators have reported a high rate of false‐negative results utilizing specific red cell adherence (SRCA) test of Davidsohn. Since approximately 45% of the population is blood group 0(H), this high rate of false‐negative result renders the test unreliable in an ordinary laboratory. The present study is comparing the results of SRCA to that of peroxidase antiperoxidase (PAP) technique in a double‐blind fashion in 29 patients with bladder cancer having 0(H) blood group. In terms of grade, stage, frequency of recurrent tumor, and quality of slides, the results of PAP is superior over SRCA. Furthermore, the convenience, permanence, and ready availability of PAP will enhance the utility of this test in a conventional histopathologic laboratory with a high rate of accuracy and reproducib
