AbstractDNA ploidy patterns were measured by flow cytometry in 7 human carcinomas and compared with the DNA histograms of 3 cell lines and 25 xenografts and derived from them. Four of the carcinomas showed DNA ploidy changes on initial xenografting or early passaging, while 2 carcinomas showed stable ploidy patterns over several xenograft passages. One xenograft derived from a cell line showed a marked, but stable, increase in DNA content over the original human carcinoma. Two of the 3 cell lines were DNA aneuploid, and at least 1 line was apparently derived from a minor subpopulation in the original carcinoma. The possible mechanisms and implications of these changes are discussed.

AbstractTo gauge the impact of screening mammography and opinions regarding surgical procedures on the pattern of primary surgical care of breast cancer, tumor registry statistics from 1980–81 were compared to those from 1987 at a large community hospital. AbstractFrom November 1981 to July 1985, 124 women with International Federation of Gynecology and Obstetrics (FIGO) stage III ovarian cancer were treated in prospective studies of surgery and chemotherapy in our institution. Patients with no macroscopic cancer after primary surgery (n = 16) received five cycles of adjuvant cis‐platinum; those with residual cancer after primary laparotomy (n = 108) underwent a second surgical debulking after three or five cycles ofcis‐platinum‐based cytoreductive chemotherapy. Total macroscopic tumor clearance was achieved in 26 of these 108 patients. Fourteen patients with total tumor excision at primary laparotomy remain in complete clinical remission a minimum of 36 months after diagnosis, but the median progression‐free interval for the other two groups was 9 and 17 months, respectively. AbstractThirty‐six cases of synovial sarcoma (13 biphasic and 23 monophasic) were subjected to a clinicopathologic study that included electron microscopy and immunohistochemistry. The group consisted of 21 males and 15 females ranging in age from 2 to 63 years. The majority of tumors (27 cases) were found in the hip and lower extremity. Immunohistochemical study revealed that keratin, which was detected in 92% of the biphasic and 57% of the monophasic tumors, was a more sensitive marker of epithelial differentiation than EMA or CEA. The overall 5‐year survival of the patients was 64%. Male sex, older age, presence of tumor necrosis, monophasic pattern, and absence of keratin positivity had an unfavourable effect on survival but lacked statistical significance. AbstractThe follow‐up results of surgical procedures for cancer of the pancreas at three affiliated hospitals during the past 15 years (1974‐1989) were ret‐rospectively analyzed to evaluate the merit of pancreatectomy in surgical treatment of advanced stages of this disease. Included were 4 cases of stage I, 14 cases of stage II, 19 cases of stage III, 43 cases of localized stage IV, and 35 cases of generalized stage IV. Pancreatectomy was performed in 67 cases; 100%, 92.9%, 89.5%, 67.4%, and 11.4% of the stage I, II, III, localized IV, and generalized IV cases, respectively. For the localized stage IV cases, in which the cancerous lesions were advanced but limited to the peripancreatic region, 29 pancreatectomies, 12 bypass operations, and 2 exploratory laparotomies were performed. This group included 17 curative and 12 noncurative pancreatectomies. The 50% survival periods were 257 days after curative pancreatectomy, 226 days after noncurative pancreatectomy, 120 days after bypass operation, and 33 days after exploratory laparotomy. The difference in overall survival rate between curative and noncurative pancreatectomies was not significant. The overall survival rates after both curative and noncurative pancreatectomies were significantly higher than the rate after bypass operation. The postoperative physical performance status after pancreatectomy was significantly better than after the palliative procedures. No significant difference in the status was found between patients after standard and extended pancreatectomies. There was no signifigant difference in the survival rates or the physical performance status between the pancreatectomy group and the palliative surgery group for the generalized stage IV cases, in which the cancerous lesions extended beyond the peripancreatic region. On the basis of these findings, it is concluded that pancreatectomy extends the postoperative survival period without impairment of the physical performance status in patients with advanced cancer of the pancreas. Even when the pancreatectomy proves to be a noncurative resection, this aggressive surgical approach may be of benefit to this group of patients. AbstractNinety‐four patients with malignant pleural mesothelioma were treated at Southern California Permanente Medical Group Facilities between 1965 and 1988. This retrospective analysis of survival in these patients is compared according to surgical and/or supportive management. Group I patients received supportive care only, including pleurodesis as needed. This group included the majority of patients. Group II patients were managed largely with debulking procedures including decortication and pleurectomy. Group III patients received extrapleural pneumonectomy. This group included the two long‐term survivors of the entire group. AbstractIn the past 27 years, 75 patients were found to have separate primary carcinomas of the head and neck and the esophagus. Head and neck tumor was discovered first in 41 patients, synchronously with esophageal tumors in 17 patients, and esophageal tumors were recognized first in 17 patients. Reconstruction by graft after resection of head and neck cancer was nec‐ essary in 18 patients and esophagectomy was performed in 62 patients. In eight patients, pharyngeal and esophageal reconstruction was necessary after resection of synchronous and metachronous pharyngeal and esophageal carcinoma. AbstractThirty‐four cases of phyllodes tumor of the breast were studied to evaluate the clinical and pathological features, which correlate with local recurrence. Histologically, local recurrence correlated with the finding of more than ten mitotic figures per ten high‐power fields, marked cellularity, prominent nuclear pleomorphism, and stromal overgrowth. Older patients and patients with larger tumors had a higher probability of local recurrence.
