AbstractDuring an 18 month period, 41 patients who had previously had a primary tumor removed, underwent 60 operations for resection of newly detected radiographic pulmonary nodules. Primary tumors included 16 osteogenic sarcomas, 9 malignant melanomas, 12 soft tissue sarcomas, 1 Ewing's sarcoma, 2 adenocarcinomas of the rectum, and 1 testicular teratocarcinoma. One hundred and eight‐one pulmonary specimens were submitted for pathologic examination and 149 discrete nodules were found in these specimens by the pathologist (83%). Metastases represented 76–100% of nodules removed in 68% of the operations, 51–75% of nodules in 12%, 26–50% of nodules in 14%, and less than 25% of nodules in 6% of the cases. Metastatic disease was found in 90% of cases where nodules were greater than or equal to 0.5 cm in diameter, while metastases were found in only 33% of cases where the largest nodules removed were less than 0.5 cm. In only two cases did the histology of the pulmonary metastases differ from that of the primary tumor. AbstractSixty patients with renal adenocarcinoma have been treated with five different immunotherapy trials consisting of 1) Transfer Factor (TF), 2) TF and Bacillus Calmette‐Guerin (BCG), 3) TF, BCG, Chloroethyl‐cyclohexy‐nitrosurea (CCNU) and megestrol acetate (Megase), 4) BCG, CCNU, and Megase, or 5) BCG. Using strict response criterea for measurable disease, objective responses were seen in 14–22% of cases. While this nonspecific immunotherapy of renal adenocarcinoma has been associated with documented regression of metastases, response rates are similar to that obtained with hormonal therapy alone. AbstractFrom 1965 to the present, 287 patients with advanced and metastatic carcinoma of the breast have been treated according to a uniform philosophy of sequential therapy. Surgical castration was the initial procedure for premenopausal women and for postmenopausal women with clinical or laboratory evidence of endocrine responsive tumors. Tumor progression following castration was treated with major endocrine ablation, either adrenalectomy or hypophysectomy. Patients who relapsed following major ablation were treated with antiestrogen therapy as it became available. Nonresponders to major ablation and patients relapsing after antiestrogen therapy were treated with combination chemotherapy including cytoxan, methotrexate, 5‐fluorouracil, and vincristine followed by adriamycin alone or in combination. Progression following chemotherapy was treated with additive hormonal therapy. Radiation therapy was used throughout for the control of localized disease, usually following complete hormonal ablation, except in cases of brain and spinal metastatic disease. The median survival for the entire group was 40.5 months from the onset of metastatic disease. Patients who responded to both oophorectomy and major ablation had a median survival of 61 months, which compares favorably to survival of 14 to 22 months reported in major combination chemotherapy trials. AbstractThree cases are presented in which pericardial tamponade was the first indication of malignant disorder. The diagnosis and therapy of malignant cardiac tamponade was revealed by emergency pericardiotomy. In all cases the tumor was an adenomatous carcinoma and the site of primary tumor was unknown until death. The median survival time after onset of symptoms was 4 months. The primary site was the lungs in two and remained unknown autopsy in one case.

AbstractBased on reports of synergism between adriamycin and radiation therapy in experimental systems, a trial was initiated testing this combination in 53 adult patients with various advanced malignancies, especially sarcomas. Since studies have suggested a selective sensitization of hypoxic cells when this drug is given prior to radiation, an injection of low‐dose adriamycin was given 90 min before radiation. This treatment was repeated every 7 days. The combination was most effective in the 30 patients with soft‐tissue sarcomas, with 62% of these patients having partial or complete responses. AbstractHemorrhage resulted in a large retroperitoneal mass in an adolescent male. Surgical exploration revealed only hematoma. Serum markers were useful in leading to the correct diagnosis. This presentation has not been previously reported in embryonal cell carcinoma.

AbstractA case of primary malignant duodenocolic fistula is presented. Colonic carcinoma is the most common etiology of this entity. Clinical features include diarrhea, weight loss, anemia, and feculent vomiting. Review of the literature indicates that the treatment of choice is a right hemicolectomy with a pancreaticoduodenectomy or segmental duodenectomy if the disease is localized. Forty‐six percent of patients undergoing this procedure have been reported to be alive more than 2.5 years later. If the disease is disseminated, the best palliative procedure appears to be a right hemicolectomy with a partial duodenectomy. AbstractSixteen women with a clinical diagnosis of inflammatory breast carcinoma had estrogen receptor analysis performed. Eleven of 16 were premenopausal. Median age of all patients was 46 years.
