Purpose: To better define the spectrum of limbal and corneal dermoid characteristics and to obtain data on the visual outcome following surgery. Methods: We reviewed the ocular records of 12 consecutive patients with solid dermoids which underwent surgery in our institution from 1985 to 1993. According to tumor size and location, limbal dermoids can be managed with a simple excision associated with a conjunctival flap or corneoscleral lamellar keratoplasty. Results: Surgical techniques, cosmetic and visual outcomes are documented. Limbal dermoids were more noticeable cosmetically, and they often induced oblique astigmatism. In 6 cases, they could easily be shave off the cornea and adjacent sclera to improve the appearance of the eye; visual acuity was unchanged. In 3 cases, unfortunately a degree of refractive amblyopia remained after late surgery and visual acuity was inferior to 20/200. In 2 large-diameter limbal dermoids, early corneolamellar keratoplasty allowed to improve best corrected visual acuity. In the corneal dermoid, amblyopia was the indication for early surgical removal. Conclusion: We believe that early surgery with simple local resection (combined with a conjunctival flap in order to cover the bare area) may be preferred to lamellar keratoplasty. Nevertheless, corneoscleral lamellar grafts may be required in advanced cases, especially when the whole thickness of the corneal stroma is involved or when the tumor deeply extends around the limbus.

To investigate the effect of intraoperative mitomycin C (MMC) application on muscle insertion migration after adjustable surgery, hang-back recession was bilaterally performed for 4 mm on the inferior rectus muscle in 30 rabbits. The scleral wound of the insertion site of one eye of each pair was treated with a sponge soaked with a solution cotaining mitomycin at a concentration of either 0.4 or 1.0 mg/ml for 5 min. As a control, the contralateral eye was treated with a distilled-water-soaked sponge. Three, 6 and 12 weeks later, the distance from the anterior border of the reattached muscle to the original insertion was measured with in vivo and microscopic examination. Anterior migration of the muscle insertion was observed in both the MMC-treated and control eye. The measurement by the in vivo and histologic examinations revealed that MMC treatment did not significantly reduce the anterior migration relative to the control eye. Analysis of the time course in the MMC-treated eye revealed a significant increase during the observation period in anterior migration as measured by the histologic examination (p = 0.0267 for the 0.4 mg/ml group and p = 0.0408 for the 1.0 mg/ml group). Exposure to MMC has no significant inhibitory effect on the muscle migration compared to that in the control eyes.

Transforming growth factor β (TGF-β) plays an important role in anterior segment wound healing, by controlling the cell proliferation and differentiation, angiogenesis, extracellular matrix composition and mediating the immunosuppres-sive properties of the aqueous humor. The present study was undertaken to clarify the possible changes of aqueous humor TGF- β1 levels after excimer laser photoablation. Twenty-eight New Zealand rabbits were divided into four groups of 7 rabbits each. Group 1 served as control, the central 7 mm of corneal epithelium was removed in groups 2, 3 and 4. We performed 50-µm corneal photoablation in group 3, and 100-µm ablation in group 4. After 48 h we measured the TGF- β1 levels of the aqueous humor by ELISA method. The mean TGF- β1 value of the aqueous humor was found to be 162.94 ± 13.73 pg/ml in the control group. Mechanical deepithelialization did not change the TGF- β1 0.05), but the TGF-β1 levels of the 100-µm photoablated group were found to be significantly higher than those of both the control group and 50-µm photoablated group (p < 0.05). Many factors and cytokines may induce corneal haze and myopic regression after excimer laser photoablation; our study demonstrated that TGF- β1 is one of these factors and there is a positive correlation between the depth of corneal photoablation and aqueous TGF- β1

Angiotensin II type 1 receptor (AT1) is expressed in a variety of tissues. However, the localization of the specific ATI mRNA in several ocular tissues has not been fully elucidated. In the present study, we investigated the expression and distribution of AT1 mRNA in the several ocular tissues of Wistar-Kyoto rats (WKYs) using the technique of reverse transcription with polymerase chain reaction. The 5 cornea, sclera, iris, ciliary body, and choroid samples tested were negative. When we analyzed 5 retina samples, 4 were positive. AT1 mRNA was more significantly expressed in the retina of WKYs than in the other ocular tissues of WKYs (p = 0.0238 < 0.05). Our study provides the first direct evidence that the AT1 is abundantly present in the WKY retina portions. This is a strategic location for an ocular renin-angiotensin system to influence retinal cicrulation.

Upper-eyelid retraction is a common sign of thyroid-associated eye disease (TAED), and these patients are highly bothered by the appearance of their eyes. In this study, botulinum toxin A (BTA) was injected into the levator palpebrae superioris muscle in 8 eyes of 4 patients in an attempt to control the abnormal elevation of the upper eyelid. BTA provided control of the upper-eyelid retraction, and the cosmetically acceptable effect lasted for 3–4 months. It was concluded that BTA is an effective method of treatment in this condition. Since it has a temporary effect, it can safely be used to provide relief of symptoms related to upper-eyelid retraction during unstabilized periods of TAED, which may last as long as several years in some patients.

摘要:
We observed a unilateral free-floating vitreous cyst in a 6-year-old boy. The ophthalmologic examination was otherwise normal. The cyst, which was pigmented, had a diameter of approximately of 2.5 mm and was localized in the anterior vitreous of the left eye. In its movements it occasionally affected the visual axis, causing only transient and sporadic blurring of vision. Visual acuity in the affected eye was equal to that in the other eye (20/20). Follow-up is by periodic examinations.

摘要:
The use of topical corticosteroids following photorefractive keratectomy (PRK) is widespread. The major complications of potent corticosteroids are glaucoma and cataract formation; in order to decrease these complications, 0.1% fluoro-metholone administration is usually preferred after PRK. We report here a case of lens opacification which was induced by 0.1% fluoromefholone administration after PRK in a period of 4 months. To our knowledge, this is the first reported case of 0.1% fluorometholone-induced cataract after PRK.

We evaluated a unilateral retinal vascular abnormality in a 34-year-old woman receiving bromocriptine mesylate for the suppression of postpartum lactation. After using bromocriptine, 2.5 mg twice daily for 1 week, the patient complained of metamorphopsia in the left eye. A central retinal vein occlusion was found. We believe that central retinal vein occlusion in patients receiving bromocriptine, as demonstrated in our patient, is rare.

A 12-year-old female patient suffered a horse kick to her face, resulting in various orbital fractures and blunt eye trauma. Upon ophthalmoscopy, an extensive subretinal hemorrhage was revealed under the macula, reaching to both vascular arcades. The visual acuity was 20/300. Five days after the accident, the patient was referred for vitreoretinal surgery. After standard vitrectomy, a small retinotomy was made at the temporal raphe. A second infusion was directed under the retina, thereby separating it carefully from the clot. The blot clot was exposed to tissue plasminogen activator (0.012 mg/0.1 ml) for 30 min. The liquefied blood was then washed out by continuous irrigation with buffered saline solution. The procedure was terminated by gas tamponade. Eleven days after surgery, the patient’s vision had returned to 20/60. At 10 months after surgery, her vision was 20/30. No new hemorrhage was observed, nor did any fíbrovascular proliferation occur from choroidal rupture. Continuous infusion instead of use of syringes for irrigation reduces the frequency of instrument passages through the pars plana and vitreous base, which may help reduce the risk of iatrogenic ora lesions. Surgery should be performed within 7 days to avoid toxic or metabolic retinal damage.

Initial clinical observation of a 2-year-old boy whose right eyelid had been pierced by a glass thermometer 3 days earlier showed a scab-covered cicatrix and slight swelling in the eyelid. X-ray examination showed the broken thermometer tip, fine glass fragments, and mercury droplets beneath the eyelid and extending along the superior orbital margin. On surgical incision, glass fragments and the thermometer tip were found and extracted from the eyelid and orbital fatty tissue with forceps, and 70.3 mg of mercury droplets were readily removed from the same regions by suction via an elastic needle connected to an injection syringe. Blood mercury gradually rose from the normal range to a peak of 28.4 μg/dl 3 months after surgery, then declined during the following 7 months to a level of 15.1 μg/dl. No abnormality has been found in liver or renal function to present.
