In congenital cases, ocular toxoplasmosis often presents as a focal whitish fluffy lesion in the retina adjacent to an inactive chorioretinal scar. We examined a girl who has visible floaters in the right eye. The patient had focal active retinitis in the right fundus, a focal chorioretinal scar in the left fundus, a positive enzyme-linked immunosorbent assay (ELISA) for IgG anti-Toxoplasma antibodies and a negative ELISA for IgM antibodies. We believe that active focal retinitis in one eye and a focal chorioretinal scar in the fellow eye in congenital toxoplasmosis, as demonstrated in our patient, may be rare.

Flavobacterium indologenes has recently been implicated in nosocomial or opportunistic infection. It has been isolated from lids, conjunctiva, and lacrimal system, and is resistant to most antibiotics. No previous cases of F. indologenes corneal ulcer have been reported in the literature. The natural habitat of Flavo-bacteria is soil, water, plants, and foodstuffs. In the hospital environment, these bacteria exist in water systems and on wet surfaces. They are not part of the normal ocular flora.

A severe case of conjunctival dyskeratosis was resolved by treatment with high doses of vitamin A administered both topically and parenterally. This result, which was confirmed by cytology, and the lack of adverse effects suggest that vitamin A administration migh be a valid alternative to surgery in cases of dyskeratosis.

The term junctional scotoma of Traquair is used to describe a unilateral temporal visual field defect due to pathology of the ipsilateral optic nerve where it joins the chiasm. We report a 26-year-old woman who presented with blurred vision of her left eye. Examination revealed visual acuity of 20/20 OU, AO-HRR color plates 6/6 OD and 4.5/6 OS, and a 0.3 log unit relative afferent pupillary defect on the left side. Computer and Goldmann perimetry revealed a normal visual field OD and a paracentral temporal scotoma OS. Magnetic resonance imaging scan disclosed a sellar mass, expanding mainly into the left suprasellar area. Laboratory findings and pathological studies were consistent with a prolactin-secreting pituitary adenoma. While involvement of the temporal visual field in only one eye (junctional scotoma of Traquair) is found in about 10% of patients with pituitary adenoma, involvement of only the paracentral temporal visual field in one eye is rare (1–2%). Recognition of this rare syndrome is important because careful visual field analysis discloses a vertical step and helps to avoid misdiagnosing optic neuritis.

A case involving a 14-year-old girl having clival chordoma with symptoms of diplopia, acute esotropia, and bilateral abducens palsy is reported. Magnetic resonance imaging revealed a clival tumor invading the posterior edges of the bilateral cavernous sinuses. After removal of the tumor, no ocular manifestations were noted. The importance of acute esotropia as a sign indicating a possible brain-stem mass is emphasized.

Pigment epithelial detachment is usually associated with disorders of the underlying Bruch’s membrane and choroid in a context of age-related macular degeneration. In a very few cases, lesions resembling pigment epithelial detachment have been reported after retinal reattachment surgery. We observed an additional case who presented deep lesions several weeks after a scleral buckling procedure for retinal detachment. The lesions, all located posteriorly at the edge of the reat-tached retina, appeared to be subretinal, well circumscribed and yellowish with a normal overlying retina. All exhibited the typical biomicroscopic features of focal detachment of the retinal pigment epithelium (RPE) but failed to show, throughout the angiogram, the typical fluorescein pooling of the classic pigment epithelial detachment. This suggests that these lesions, though simulating RPE detachments, represent focal pockets of subretinal fluid.

We examined 3 patients with isolated retinal macroaneurysms and secondary hemorrhage by standard fluorescein angiography (FA) and indocyanine green (ICG) angiography. In contrast to standard FA, ICG videoangiography clearly demonstrated the lesion to be pulsatile, contiguous with the arterial wall and pathognomonic of an isolated retinal artery macroaneurysm. ICG with its absorption and emission peak in the near-infrared range penetrates hemorrhage to a greater degree than does blue or green light of conventional FA. ICG can be essential in delineating the specific cause of the bleeding and allow the physician to choose the correct specific form of treatment needed.
