Oculopression tonometry according to Ulrich can be used for the judgement of the facility of outflow like the tonography according to Grant. As shown by us for the first time, oculopression tonometry can also serve to evaluate aqueous humor formation. The results of a planned prospective study in 30 glaucoma patients and in 30 healthy volunteers are shown. It can be concluded that the recovery time of the intraocular pressure seems to be a better criterion for the diagnosis of glaucoma than the intraocular pressure measured immediately after the artificial pressure enhancement.

The aim of the present study was to show the effect of different topical beta-blocking agents on aqueous humor formation with a clinically practicable method. In a clinical, randomized, prospective double-blind study, 60 healthy volunteers were examined by means of oculopression tonometry according to Ulrich. The placebo was applied to 29 subjects; a nonselective beta-blocker (timolol maleate 0.5%) was given to 14 volunteers and 17 subjects received a cardioselective, beta-1-blocking agent (betaxolol-HCl 0.5%). On the 1st day, intraocular pressure (IOP) was lowered by the suction cup method and then we measured the time required for IOP to return to the initial value. The examination procedure of the 1st day was repeated on the 2nd day 3 h after application of the ophthalmic solution. After evaluation of the different groups, we could show that the recovery times in the placebo groups were comparable. Furthermore, we showed that both beta-blockers slow down the increase of IOP to the initial value. There was a statistically significant difference in recovery time between timolol and placebo. This difference was statistically not significant between beta-xolol and placebo.

After 20 years of extensive practical experience in the management of glaucoma, beta-adrenergic blocking agents have gained a continuously increasing importance. The reason is a combination of effective reduction of intraocular pressure even during chronic treatment and few adverse effects. For many years, substantial effort has been focussed on the elucidation of the cellular mechanism inducing the intracellular pressure reduction. In this respect, mainly three different hypotheses have been controversially discussed: beta-blockers reduce aqueous humor formation (1) by an unspecific membrane-stabilizing process, (2) by impeding the local circulation of the ciliary body or (3) by specifically blocking adrenergic beta-receptors intimately involved in the regulation of aqueous humor production. Recently, substantial progress has been achieved by different working groups getting highly purified melanin-depleted fractions of ciliary epithelia which allow accurate receptor-ligand binding studies. The most important results, which all favor the explanation that the mechanism of action is due to specific inhibition of adrenergic receptors of the beta-2-subtype, are presented here. The consequences which result for the development of more specific pharmacologic agents in the future are discussed.

This report of papillary conjunctivitis induced by a corneal foreign body suggests that papillogenesis may result from physical/mechanical tissue trauma. A case is presented with an epithelialized corneal foreign body in the peripheral cornea during a 9-month period. The overlying corneal epithelial surface was elevated, and corresponding upper tarsal conjunctiva revealed localized, elevated papillae. One month after the foreign body was excised, the papillae resolved. An epithelialized foreign body, elevated cornea, and coincidental papillae support the role of physical trauma for the development of papillae – a feature of the syndrome of contact-lens-associated giant papillary conjunctivitis.

Visual loss in birdshot retinochoroidopathy has been reported to be produced by cystoid macular edema, optic atrophy, epiretinal macular membranes and subretinal neovascularization. We present a patient with this syndrome in whom the visual loss was due to an acute anterior ischemic optic neuropathy. The possible common pathogenesis of both diseases is discussed.

A specific carbonic anhydrase activity inhibitor (methazolamide) was injected into one vitreous body each of 4 New Zealand White rabbits. Electroretinograms (ERG) were recorded before and several times after the methazolamide injection. The stimulus levels maximized the rod and cone response characteristics of the rabbit ERG. The effects of methazolamide were followed over 5 h. During this time, the electroretinograms showed a decline in amplitude of both a and b waves at both stimulus levels. The data support the involvement of carbonic anhydrase in the excitatory physiological events in the retina. Preliminary evidence indicates a slow recovery of the carbonic-anhydrase-inhibited ERG.
