Two brothers with symptoms of connate ophthalmic lymphatic oedema are reported. The combination – connate generalized lymphatic oedema with antimongoloid eyelid, ‘cow-eye phenomenon’, euryblepharon and conjunctival lymphatic oedema of two brothers with healthy parents – suggests the existence of a new entity of this syndrome with recessive heredity.

The authors present the results obtained in the treatment of oedematous diabetic retinopathy of 70 patients, i.e. 129 eyes. Follow-up was from 6 months to 6 years. Argon laser photocoagulation of the posterior pole is simple and without risk at the early stages of oedematous retinopathy, and has also proved to be effective in the later stages. Out of 115 eyes exhibiting oedemas of the posterior pole, 84% were stabilized or improved. The same encouraging results were obtained in the case of macular oedemas. Ideally, regular follow-up and treatment as required should prevent cystoid degeneration of the macula.

A 26-year-old man with sex-linked juvenile central and peripheral retinoschisis showed on fluorescein angiography an avascular lower temporal periphery, bordered at the central side by abnormal capillaries in a dendrite-like structure. In dominant exudative vitreoretinopathy an identical picture without schisis can be seen in the periphery. This feature seems to be a rare phenomenon in sex-linked juvenile retinoschisis. However, routine peripheral ophthalmoscopy and fluorescein angiography (especially inferotemporal) may reveal more cases.

The action of silicone oil is due especially to two of its properties: density and surface tension. Its density is less than 1: for this reason, the silicone oil bubble aways presses upon the upper part of the eye, i.e. most of the time at twelve o’clock; so there is a constant support upwards which closes the tears and reattaches the retina. The action of silicone oil is different on the lower part of the eye, even if the cavity is filled with silicone oil: a buckle is then required. The surface tension being weak, a vitrectomy is necessary to obtain a single bubble which gives a higher pressure than several small bubbles. The cumulative effect of the density below 1 and the low surface tension explains that the recurrences always occur in the lower part of the retina. Retinotomies or retinectomies are presently trying to counteract this ineffectiveness of silicone oil.

524 persons showing pathological ocular symptoms (hypertony, transient amaurosis, macular alterations, retinal vascular alterations, pigmentary retinopathy) underwent a cervicoencephalic Doppler test with echotomography. The results confirm the frequency and importance, in many cases, of latent cervicoencephalic degeneration: 12–27% of the subjects had important or very important atheromatous vessels, 25–42% with carotid endarterial plates, 1–10% with carotid stenosis (higher than 80%), 12–24% with alterations of the ophthalmic arteries. Such a noninvasive investigation is a very useful means to explore the hemodynamic and physiological status of the vascular surroundings of the eye and of the central arteries of the retina; it could be a very valuable tool for preventive and curative therapy.

Videoangiography is capable of identifying the different phases of a fiuorescein angiogram exactly in terms if time elapsed. We employ slow-motion television techniques to determine the rate at which fiuorescein flows through the retinal vessels. By this means the arterial, capillary and venous circulatory phases can be isolated at will for study. The way for this method is opened by a division of the overall circulation interval of a bolus through the retinal vessel into arterial, early venous and late venous circulation times. In the study described here the normal parameters of these circulatory times were determined for a group of healthy subjects (n = 100).

The study included 30 aphakic eyes half of which had received implants and the remaining half contact lenses. The examination showed abnormal laboratory findings in the tears of these patients, with signs of inflammation in the frontal segment in a high percentage of implants: albumin showed pathological levels in 93% of the implanted eyes and total IgG was abnormal in 87% of the implanted eyes. For the contact lens group, the corresponding values were 69% and 50%. However, considering the advantages of implants, one should accept this even if it implies more frequent postoperative follow-ups. Careful attention should be paid especially to lysozyme, which was pathological in 27% of the implanted cases, but completely normal in the contact lens group.

The ultrastructure of the iris muscles and innervating nerves in patients with diabetes mellitus and approximately age-matched controls was examined by electron-microscopy. The specimens were obtained during cataract surgery. There were definite histopathological alterations at the nerve terminal innervating the dilator muscle, e. g. the presence of mitochondrial abnormalities, dense bodies and lamellar structures. No change was noted at the nerve terminal to the sphincter muscle. Moderate involvement of the muscle, especially at the sphincter, was observed in the specimens of diabetics. The control specimens had no change at the nerve terminals, while moderate change was observed at the sphincter muscle. Therefore, the change in the sphincter seen in the specimens of diabetics seemed to be mainly the result of the aging process, though the existence of early sympathetic neuropathy was confirmed.

Corneal and vascular endothelial cells in sectioned human embryonic eyes were studied with respect to factor VIII expression. This was assessed by the use of two different antibodies which bind to factor VIII Rag with high specificity. It was found that whereas human vascular endothelium expressed factor VIII Rag as significant levels, corneal endothelium as well as all other ocular cells were found to be factor VIII-negative. This finding suggests that endothelial cells lining the inside of blood vessels are functionally different from those in the cornea.

The aqueous humor and lenses of rabbit eyes contain inhibitors of photodynamic destructions. Preparations of vitreous, however, more likely seem to contain stimulatory compounds. The anticataractic drug Pherajod® significantly blocks the photodynamic formation of destructive oxygen species. Inhibition by 65% of photodynamic destructions, measured as ethylene release from S-methyl-a-ketobutyric acid in the riboflavin-driven oxygen photo-activation, is observed with aqueous humor preparations isolated 5 min after start of incubation of rabbit eyes in Pherajod solution.
