In a group of patients with keratoconjunctivitis sicca (KCS) associated with Sjögren’s syndrome (SJ) a significant decrease in tear fluid lactoferrin concentration and tear film break-up time was demonstrated in comparison with a matched group in which KCS was not associated with any systemic disease. The Schirmer values were also lower in the SJ of patients and these differences almost reached statistical significance. None of these parameters in itself appeared to be of any clinical value to differentiate between KCS that was or was not associated with SJ. The combination of the tear fluid lactoferrin concentration, the Schirmer I test values and the tear film break-up time, however, provide a basis for differential diagnosis that is clinically of some value.

The possible causative relationship between exposure to house dust mite allergen (HDMA) and symptoms of vernal keratoconjunctivitis (VKC) was investigated. VKC patients were evaluated for an average of 18 months using patient reports of symptoms, physicians’ observations and assays of sera and tears for total and specific IgE to 10 inhalant allergens common in Israel, including HDMA. Tear and serum samples from a group of 8 patients reacted only with HDMA, and were compared with the tear and serum samples of a group of 11 patients not reacting to any of the above allergens. House dust samples from the homes of two VKC patients in the HDMA-reactive group were examined monthly for a year. While mites were detected throughout the year, they were approximately 3 times more abundant from June through September than at other times. 96.6% of the mites collected belonged to the family Pyroglyphidae; 91.0% were Dermatophagoides pteronyssinus and 5.5% Dermatophagoides farinae. Both the severity of VKC symptoms in HDMA-reactive patients and mite population levels peaked simultaneously in the summer. This relationship was not seen among the non-HDMA-reactive patients. Our results suggest that exposure to HDMA plays an important role in the aggravation of VKC symptoms in HDMA-reactive patients.

Central corneal endothelial photographs were taken with a specular microscope. The cell density was calculated with a computerized image analyzer. No difference between the treated and control eyes could be found. The laser energy level delivered in trabeculoplasty seems to be too small to induce any permanent changes in the endothelial cells.

We treated two patients with herpes zoster ophthalmicus in whom hyphema and hemorrhagic glaucoma occurred. Case 1 complained of facial skin eruption, and was given intravenous acyclovir for 7 days. Hyphema and high intraocular pressure occurred in the left eye 10 days after the onset of the skin eruption. Case 2 had severe pain and blisters on her face, and was given intravenous acyclovir for 7 days. An intracameral hemorrhage and glaucoma developed in the right eye 15 days after the onset of the skin lesion. Intravenous acyclovir may be necessary for longer than 7-day periods if the iridocyclitis remains.

We examined a 63-year-old woman with retinal telangiectasia, aneurysms, a ring of fatty exudate, and pigmented chorioretinal atrophic lesions in the right eye. Leber’s miliary aneurysm was diagnosed, although this disorder is known to have a high incidence mainly in young boys. The authors present the results of a transmission electron-microscopic study performed on a tissue sample obtained by an intraocular fine-needle aspiration biopsy in a case of retinoma. The ultrastructural study showed that the tumor was composed mainly of granular osmiophilic material which contained some tumor cells. These cells showed peculiar ultrastructural characteristics, which demonstrated their neural character, and appeared in different degenerative stages. Apparently, this is the first case in whom ultrastructural techniques have been applied to study an intraocular tumor sample obtained by intraocular fine-needle aspiration biopsy, and the first case of retinoma studied by such a diagnostic approach.

Nine cases of metal intraocular foreign body are presented. This study investigates the use of electrodiagnostic techniques in the diagnosis of metallosis and as a prognostic indicator. The electro-oculogram (EOG) and the electroretinogram (ERG) indicate that there are two mechanisms of metallosis, one of which involves cytotoxic damage by siderosomes, and the other lipid peroxidation of photorecepter outer segments and retinal pigment epithelium. The evidence suggests that the location of the foreign body in the eye determines whether either or both of these mechanisms occur. We propose that these tests can help the ophthalmologist decide when a foreign body has to be removed and give information as to the mechanism of the pathological process.

Nd:YAG laser surgery is performed by inducing optical breakdown in the interior of the eye. The optical breakdown is accompanied by shock waves which expand throughout the entire eye. In our experiments, we measured the amplitudes of shock waves at the walls of artificial eyes caused by single and multiple pulses of optical energy originating from an Nd:YAG laser at a wavelength of 1.064 μm. We have shown that the effects observed are not caused by mechanical resonance. We believe that similar results can be expected for human eyes. The peak pulse amplitudes in artificial eyes of different length were found to increase linearly as a function of the energy transferred into the eye. The peak amplitudes of pulse sequences were found to be only half of those for single pulses of comparable energy. In laser surgery, such sequences can be preferable if they achieve the same effect.
