A new type of corneal dystrophy with various oculocutaneous symptoms and other signs is described. Snowflake dystrophy is characterized by hundreds of small, round, oval or cork-screw-like white opacities in the endothelium and Descemet’s membrane. The length of the opacities is 5–20 μ and they form an even layer throughout the posterior membrane. 44% of the patients above the age of 70 years had also pseudoexfoliation of the lens capsule. Corneal endothelial pigmentation advance with the age but are not even in elderly patients necessarily present. A wide range of cutaneous disturbances of melanin metabolism was noted in 4/5 of the cases: intradermal nevi, lentigines, nevus spilus, melasma, vitiligo, early alopecia and early graying of the hair. Photosensitivity reactions like solar urticaria were noted in 5 cases. The skin was often wrinkled, dry and inelastic. Conjunctival wrinkling and Bitot’s spots, ovarial cysts, frequently recurrent tonsillitis and several cholecystectomies suggest a generalized involvement of mucous membranes in this syndrome. Degenerative joint disease was constated in 2/5 of cases. The genetic analysis of 59 persons revealed an autosomal dominant mode of inheritance. The prevalence of the gene was high in the province of Satakunta in western Finland.

In an 18-month-old girl affected by mucolipidosis IV (ML IV) with mild corneal clouding, normal retinal activity was documented by electroretinogram (ERG) and slightly delayed optic pathway conduction was revealed by visual evoked potential (VEP). Re-examination 9 years later disclosed severe retinal alterations resulting in atrophy with reduced photopic and missing scotopic ERG components and flat VEP. These fundoscopic and electrophysiologic deteriorations in ML IV suggest progressive rod-cone impairment similar to tapetoretinal dystrophy.

An 87-year-old woman, with known atrophic senile macular degeneration in one eye, had isolated decreased reading ability while Amsler grid testing was normal. This led to the early diagnosis of macular subretinal neovascularization in the other eye. Thus patients at high risk for neovascular macular degeneration should be made aware of possible subtle changes in vision as well as abnormalities in the Amsler grid. Regular visual acuity check and careful biomicroscopic examination of the macula should be part of each follow-up examination.

23 patients with proven herpetic dendritic keratitis have been treated with combined blunt spatula debridement and trifluorothymidine. The treatment was remarkably well tolerated. Average epithelial healing time was 2.50 days. Our results suggest that at present the described treatment seems to be one of the best available treatments in herpetic dendritic keratitis.

To find out the most sensitive parameter of early toxic ocular changes, a group of patients was extensively examined at regular intervals during therapy with ethambutol. Colour vision abnormalities could be detected using the desaturated panel of Lanthony in the presence of normal visual acuity, normal visual fields, normal visual-evoked potentials and a normal panel D-15 test. Maj or blue-yellow errors were found in treated patients without visual complaints as well as in a group of healthy volunteers, but there was a significant difference between both groups. In a later stage of intoxication, blue defects, red-green defects or tritanomalous defects can be observed, together with other symptoms of ocular intoxication.

In several clinical studies involving superior oblique surgery it was noticed that the posterior insertion of the superior oblique tendon can be moved outside from the undersurface of the superior rectus muscle. This was achieved by rotating the eye in a maximum nasal and downwards direction while avoiding intorsion or retraction of the globe. Traction was applied on the superior temporal quadrant of the globe.

With a newly developed photoplethysmographical measuring device it is possible to record the conjunctival pulse of the human eye. The device consists of the self-constructed photoplethysmographical amplifier unit, the sucking electrode from silicone hard-rubber with photo-detector and luminescence diode, and a 3-channel recorder (3-NEK). Electrocardiogram and the conjunctival pulse are recorded simultaneously. Parameters that are derived from the curves allow to judge on the circulation in the tissue region investigated.

Investigations on 114 glaucoma patients with the computerized perimeter ‘Peritest’ showed that treatment with Cosaldon A + E resulted in a decrease or elimination of visual field defects. Favourable results were also obtained with statistical significance for severe visual field defects of defect class≥2.0 logU. Freatment time was generally 4–5 months. During the first 6–10 weeks of treatment strong fluctuations were observed regarding individual sensitivity of light perception as well as number of defect positions.

Two cases of teleangiectatic hamartoma of the iris and ciliary body are reported, one of them with its histopathological study. No previous cases have been found, as such, in the current literature. The most important descriptions of primary vascular lesions of the iris and ciliary body are briefly reviewed.
