Ultrasonography of the lacrimal gland is a harmless method of examination. Depending on reflectivity, sound attenuation, interior echo patterns and borders, a variety of diseases can be diagnosed echographically. Histological findings and corresponding ultrasonographical criteria are compared.

A case of iris malignant melanoma, macroscopically classified as tapioca melanoma, was studied by electron microscopy. The tumor originated in the left eye of a patient, and a specimen was obtained during iridociclectomy. Two main types of cells were found: the differences between them concerned mainly the Golgi complex, mitochondria, RER and free ribosomes. The melanotic bodies, scantly represented, were carefully examined. The ultrastructural findings enabled us to classify these two types of cells as spindle subtype B and epithelioid types. A comparison of ultrastructural features observed in our case with those of other melanomas of the uveal tract and the skin allowed us further to stress the hypothesis of a nevoblastic origin of the tapioca tumor cells.

Intra- and postoperative complications of iris-fixated 4-loops Binkhorst intraocular lense implants were studied. The horizontally placed lens implants, iris-fixated with two prolene sutures, were found to be the best placed, considering complications like lens subluxation or dislocation. Iris angiography, done 9 days to 18 months after lens implantation, revealed no local iris damage at the suture sites.

8 patients with clinically diagnosed Behçet disease were immunologically investigated. Uveitis was present in 7 of these patients. Although uveitis is a rare presentation of Behçet’s disease, it tends to be bilateral when it does occur and to involve both the anterior and posterior segments repeatedly. Nowadays it is rare to observe a hypopyon, thanks to early initiation of therapy. In the late stages, recurrent vitreous haemorrhage complicates the course of the disease and can lead to blindness. HLA-B5 can frequently be demonstrated in patients with Behçet’s disease. Detailed serological investigations, as measles hemagglutination inhibition test + complement fixation test (CFT), rubella hemagglutination inhibition test, hepatitis HB2 antigen, adenovirus antigen, varicella zoster CFT, mumps CFT, herpes simplex CFT, anticytomegalovirus IgM and IgG, toxoplasmosis immunofluorescence, tuberculosis Middlebrock-Dubois, were unable to uncover a connection with past or present infections. Determinations of haptoglobulin, lactate dehydrogenase, albumin and ceruloplasmin showed nothing worth remarking. Electrophoresis can indicate the duration of the illness. In 1 patient the lymphocyte adherence inhibition test was abnormal.

Retinal detachments caused by holes in the posterior pole are rare among rhegmatogenous detachments, but the central position of the holes causes considerable difficulties on surgery. The use of intraocular gas bubbles affords new possibilities for the treatment of these cases. Among 1,800 consecutively treated retinal detachments, we found only 21 which were due exclusively to one or more holes at the posterior pole; of these, only 7 exhibited a hole directly in the macula. The operative treatment of these 21 detachments consisted of an intraocular gas tamponade effected through injection of SF6 combined with cryo- and laser coagulation of the periphery of the holes. Reattachment of the macula and improved visual acuity were achieved in 19 cases. The postoperative follow-up ranges from 4 to 60 months, with a mean of 29 months.

Retinal folding appeared in young chicks fed a commercial starter diet. The degree of this folding was increased when diets deficient in vitamin E and enriched with polyunsaturated fatty acids (encephalomalatic diets) were fed. The unique response of the retina and the cerebellum to the encephalomalatic diets is pointed out.

The authors report an atypical case of Goldenhar syndrome characterized by hemifacial and cranial hypoplasia associated with severe microtia and anophthalmia on the right side, antimongoloid palpebral fissures, epibulbar epidermoid, corneal anesthesia and preauricular tags on the left side. The bilateral presence of characteristic features of Goldenhar syndrome is rare and lends support to the possibility that the patient presents an intermediate form of developmental defect of the first branchial arch. The differential diagnosis is discussed.
